1 STATEMENT OF WORK INSTALLATION SERVICES OF EMC AVAMAR SOLUTION LOS ANGELES COUNTY DEPARTMENT OF PUBLIC HEALTH RFB_IS_ INTRODUCTION May 21, 2013

2 This document defines the requirements, deliverables, tasks, and scope of the work to be delivered by CONTRACTOR to the DEPARTMENT OF PUBLIC HEALTH (DPH). CONTRACTOR shall provide the services necessary to accomplish all of the tasks and subtasks in this Statement of Work (SOW) and to successfully install, configure, test, and implement all of the components listed in EXHIBIT A (Component Listing) including, but not limited to, all associated software and/or firmware, patches/fixes, and interfaces, post implementation support, technical and end-user training. CONTRACTOR is responsible for all acts, and services required to provide DPH with a fully functional System in accordance with the Manufacturer s Specifications, whether or not the acts or services required to do so are specifically identified as a task, subtask, or deliverable in this Statement of Work, and unless otherwise expressly stated, all work described in this Statement of Work shall be performed by CONTRACTOR. EXHIBIT A (Component Listing) to this Statement of Work identifies all of the hardware, software, and their corresponding manufacturer part numbers that shall be installed by CONTRACTOR. Software and or other bundled items not listed in EXHIBIT A that are deemed as bundled items by the Manufacturer shall be installed and implemented in accordance with Manufacturer s Specifications, whether or not they are specifically listed in EXHIBIT A. DPH is responsible for procuring and taking delivery of these components prior to commencement of installation and engineering services. TASK 1 INSTALLATION AND ENGINEERING SERVICES CONTRACTOR shall be responsible for unpacking, installing, installation testing and integrating of all components listed in EXHIBIT A including integration into existing DPH Data Center. CONTRACTOR shall be responsible, at its expense, for obtaining any and all governmental licenses or permits that may be required for carrying out the installation at DPH Facilities. CONTRACTOR shall deliver and transfer any and all Documentation and licenses for software, firmware, utility programs, and any and all other software provided by the manufacturer as well as warranties for all of the components listed in EXHIBIT A. All configuration and integration work must be completed by CONTRACTOR with the following credentials. CONTRACTOR must be an authorized EMC VELOCITY PREMIER LEVEL certified partner, EMC VSI (VELOCITY SERVICES IMPLEMENT) certified partner with EMC certified implementation engineers on staff. In the event technical support or other services are required during or after the end of the project due to problems resulting from this project, one of CONTRACTOR s EMC certified VSEs (Velocity Systems Engineers) must be able to respond on site within four for diagnosis and problem resolution or escalation. All equipment must be supplied in brand new, unused, condition. All equipment must come from the manufacturer or an authorized EMC Reseller with full USA manufacturer warranty and USA manufacturer technical support. Equipment shall not be gray market. SUBTASK 1.1 INSTALL, AND CONFIGURE ALL COMPONENTS

3 CONTRACTOR shall unpack, install, configure, connect, power-up, and test, in accordance with Manufacturer s Specifications all components listed in EXHIBIT A. Upon completion of installation, CONTRACTOR shall submit to DPH the documents listed below. These documents are to be known as the Post Installation Documents. A listing of all components that were installed including equipment type, model number, serial number, location, name, version number, and license number. The manufacturer's valid USA warranties. Certification that all components are installed and operating properly according to Manufacturer s specifications. All reference and operating manuals and all other Documentation pertaining to the operation of the equipment. Any and all manufacturer maintenance and technical support availability and telephone numbers. Satisfactorily prove that all provided equipment is not black market or gray market. SUBTASK 1.2 PROVIDE INSTALLATION/CONFIGURATION SERVICES FOR EMC AVAMAR SOLUTION CONTRACTOR shall provide all engineering services related to configuration and implementation of the new EMC AVAMAR SOLUTION to properly function with DPH s existing EMC storage arrays, fibre channel switches, Cisco network, and attached x86 servers currently installed in the DPH Network Data Center and integrate them with all required components listed in EXHIBIT A. All configuration and integration work must be completed by CONTRACTOR with the following credentials. CONTRACTOR must be an authorized EMC VELOCITY PREMIER LEVEL certified partner, EMC VSI (VELOCITY SERVICES IMPLEMENT) partner with EMC certified implementation engineers on staff. One year experience with large scale AVAMAR deployment in healthcare IT is required. Any disruptive work that affects access to the production servers will be scheduled for weekend hours. Activities under this subtask shall include, but not be limited to, the following services: Attend weekly meeting between the time the order is delivered and installed at DPH data center. Recommend optimum configuration settings based on DPH s existing environment as well as anticipated future needs. Install the latest firmware levels on EMC AVAMAR SOLUTION, as well as verify and/or install the most current levels of firmware/software required on associated equipment (switches, servers, Management Appliance, HBA/FCA cards, etc) Install and configure EMC AVAMAR SOLUTION at DPH Primary and DR Site

4 Provide upto 120 hours of expert onsite technical resource to DPH with EMC AVAMAR certification credentials Discuss, demonstrate, and implement these procedures under the supervision of DPH staff Before project begins, CONTRACTOR should become or already be familiar with DPH Active Directory setup and components. Additional Information Location of installation and configuration: DPH Data center Hours of installation: TBD. Site preparation can be completed 8-5 Monday-Friday with scheduling approval from DPH staff. Any disruptive work that affects access to the SAN or attached production servers will be scheduled for weekend hours. Expected Completion Date: December 2013
