1 BD309.doc of 8 1 Introduction All partners of IQNet have signed either the IQNet Multi-Lateral Agrement (MLA) or the IQNet Franchising Contract; requiring them to mutually recognise partner management system certificates (listed in Annex C) as being equivalent to their own. The validity of this recognition is based on partner cooperation and confidence established the harmonisation and peer evaluation processes. This establishes the basis for: partners issuing a Declaration of Equivalence on the request of any third party relative to any certificate issued by another partner co-certification (upon customer request), i.e. partners issuing their own certificates on the basis of valid audit and certification results from another partner (to whom the customer belongs). 2 Purpose Issuing IQNet certificates on the basis of standards and specifications approved by the GA (refer Annex C). This is done in the form of co-certification and shall be provided to all customers who hold or will receive a certificate of a partner of IQNet, provided that the requirements laid down in this document are fulfilled. 3 Refereced documents IQNet SR 10 Rulebook 4 IQNet certification 4.1 Basis of IQNet certification IQNet certification bases on the assessment and certification carried out by a partner of IQNet and is therefore a co-certification. The IQNet certificate is a certificate on which both, the original certifying IQNet partner and IQNet, certify jointly that the relevant organisation s management system conforms to the defined standard or specification. The certification decision is taken: by the partner for its own partner s certificate (if any) and for its own part of the conformity statement on the IQNet certificate, and by the partner in its capacity (as part of IQNet) for the IQNet conformity statement part on the IQNet certificate. The IQNet certificate is to be issued, modified or withdrawn by the original certifying partner on behalf of IQNet. 4.2 Conditions concerning the tri-lateral relationship The customer shall be informed and have access to the rules on IQNet certification. This may be done either in the certification contract (between the IQNet partner and the customer) or by special written notice (refer Annex B). The information shall include details explaining that the: a) IQNet certificate is issued and will be modified in parallel to the issue and modification of the partner s certificate b) main responsibility for issuing the IQNet certificate lies with the partner c) applied procedures and conditions for issuing, modification and withdrawal of the IQNet certificate are described in the Management Handbook of the partner and the Management Handbook of IQNet d) IQNet certificate is to be withdrawn by the issuing body within 30 days after termination of its membership or franchising body status in IQNet e) IQNet Management System certification Mark shall be used according to the criteria of the relevant IQNet Basic Document.

2 BD309.doc of 8 The partners have the responsibility to monitor the correct use of the certificates and to take appropriate action in case of misuse. The same applies for IQNet Ltd in case of certificates issued directly. The partner shall make all efforts to the maximum extent possible to indemnify IQNet and all its partners from any claims arising from the existence of the IQNet certificate. 5 IQNet certificate content and design According to clause 3.1 the IQNet certificate is a joint certificate issued by the original certifying partner on behalf of IQNet. It shall be in English. However, IQNet partners, who are not using the Latin alphabet in their local language are allowed to issue the IQNet certificate only in their local language with the following statement in English appearing on it This certificate is available in English upon request. The issue of an IQNet certificate does not depend on a status of accreditation of IQNet. The IQNet certificate may contain the reference to the partner s accreditation only in an annex to the certificate. The statement may express that the certificate is based on the assessment and/or certificate of the IQNet partner which is accredited by the accreditation body. If this information is given in an annex to the certificate, the partner could abstain from issuing its own (accredited) certificate in parallel, if it decides to do so. The IQNet certificate shall also contain the signatures of the IQNet President and of the issuing partner. The IQNet certificate shall be printed on white paper of either A4, A3 or US Letter size. However, the proportions shall remain the same. Any other type of certificate (e.g different size, different material or media, different colour etc.) is regarded as a facsimile certificate. Facsimile certificates are permitted provided the: correct proportion is maintained correct colours are used (monochrome is permitted as an alternative) use is approved in writing by the appropriate IQNet partner The certificate design is given in Annex A. As indicated only the design of the top and bottom areas are specified by IQNet. The text of the middle area may follow the partner s text of its own certificate but printed using similar types as used for the IQNet design areas. The date of issue of the IQNet certificate equals the date of issue of the partner s certificate. The date of the original issue of the partner s certificate may be given in brackets below the validity date as decided by the partner (e.g. MM certified since yyy-mm-dd ). If in a certification project several IQNet partners are involved, the resulting Leading Partner s certificate and the IQNet certificate (or the relevant annex) may indicate this involvement. It is acceptable to reference more than one standard on the same IQNet certificate. An update of the individual partners listing at the bottom of the IQNet certificate may be made by the partners individually. 6 Costs of IQNet certification It is strongly recommended not to charge for the IQNet certificate. 7 Criteria for determining which circumstances IQNet Certificates can be issued. If an IQNet partner wants to issue an IQNet certificate in conjunction its own certificate for any Standard which is not included in Annex C, the following rules apply: A. If the Partner has a formal accreditation for this Standard from an accreditation body (AB) which is a signatory to the IAF MLA, the IQNet partner can issue an IQNet certificate in addition to its own certificate. Formal accreditation means that the related Standard must be in the scope of IAF MLA of the AB.

If there is no formal accreditation from an AB available, or there is an accreditation from a non-iaf member or there is an accreditation from an AB but this Standard is not in the scope of MLA of AB, then; B1- The Standard must be issued by a National/Regional/International Standardization Body. By National Standardization Body, it is meant a full P-member of ISO. B2- The IQNet partner must have minimum of 5 clients certified according to this Standard. B3- The IQNet partner must have minimum of 3 auditors/lead auditors according to this Standard. B4- If the Standard is a national/regional one and it has no English version, then the following information in English language must be send to IQNet head office: - The scope of the standard - A simple flow chart for the principle steps to follow according to this standard - CB s major certification document, ie. procedure, instruction, flow chart, or the brief abstract of this document concerning - Competence of Personnel involved in this particular new scheme - Qualification of Auditors involved in these certification activities for the new scheme - The lay out of the certificate for the new scheme (a sample of the Partner s own certificate, to verify the contents) - The survey process cycle if applicable (three years cycle, two years cycle, one year cycle) - provide to IQNet a formal approval coming from : steering committee or administration board or directive council or any other top management institution in our organisations (like to prove the strong commitment of the Top Management) - Audit man day calculation rules After reception of the information Head Office, this topic is put in the Agenda of SCPM meeting. After discussion by the SCPM, the proposal is put to ballot by the GA, which decision is final. The constraints B1, B2 and B3 are not applicable for non-accredited IQNet certification schemes (such as IQNet SR 10). For the IQNet SR 10 scheme, the following conditions have to be fulfilled by partners: a) A legal enforceable agreement shall be signed between the partner and IQNet association for IQNet SR10 certification. This contract shall include the clauses established in paragraph 4.7. of the IQNet SR 10 RuleBook. b) At least one auditor shall be fulfilled the qualification requiremens established on paragraph 5.2. of the IQNet SR 10 Rulebook. c) The IQNet certificate for this scheme shall not make any references to ISO 26000

7 BD309.doc of 8 ANNEX B Instructional leaflet for Organisations receiving the IQNet certificate 1 The IQNet certificate is based on the audit and certification carried on by a partner of IQNet or under its lead and is issued by this partner on behalf of IQNet. IQNet certification has been enabled because confidence between IQNet partners has been established by peer evaluations results, by cooperation and by having signed the IQNet Multilateral Agreement (MLA) or the IQNet Franchising Contract regarding mutual recognition of certificates. 2 The IQNet certificate is issued and will be modified or withdrawn in parallel to the issue and modification or withdrawal of the IQNet partner s certificate. 3 The main responsibility for issuing the IQNet certificate lies with the IQNet partner. 4 The IQNet certificate will be cancelled by the issuing body within 30 days in case of its termination of its membership or franchising body status in IQNet, and holders of an IQNet certificate must be informed in writing (letter, ), with copy to the IQNet Head Office, of the communiqué announcing that the validity of the IQNet certificate has expired.
