3 As an ALSTAR state participant, you will receive a complimentary subscription to NSPIRS which provides unlimited access to state pesticide product data and all associated federal data. All versions of state labels, for participating ALSTAR companies, are accessible through the complimentary NSPIRS subscription. You may add up to 20 NSPIRS user accounts. Please copy and complete this subscription application for each NSPIRS user. Only one user may be designated as the primary contact for NSPIRS. CHECK if this person is the primary NSPIRS contact who will be responsible for sending changes as NSPIRS users change within the state. CHECK if this person is a pesticide regulator and should have access in NSPIRS to the ALSTAR labels that are intended for Regulators Only. First Name Last Name

4 There is no cost for states to participate in ALSTAR. However, each state agency must submit a signed Subscription Authorization agreeing to the following standards. Standards for ALSTAR Participation s participating in the Accepted Labels Tracking and Repository (ALSTAR) project at Purdue University agree to the following: manage pending label acceptance in a timely manner maintain state profile to accurately reflect state requirements request inclusion of documents not currently identified in the ALSTAR list of supporting documents through the Standards Committee move away from paper labels towards electronic submission within one year, unless statutorily prohibited provide state registration data to NPIRS on at least a weekly basis The primary contact for a state will submit a signed subscription form agreeing to the above Standards for ALSTAR Participation on behalf of their state ALSTAR participants. s who cancel their ALSTAR subscription agree that all links for electronic labels for their state will be removed from the ALSTAR database and removed from the NPIRS websites. The Standards Committee reserves the right to remove any state not adhering to the ALSTAR Standards for Participation. Purdue University will invoice annually for the NSPIRS upgrade subscription fees. Please do NOT send fees with your subscription application(s). Purchase orders, contracts, agreements, etc. have to be reviewed and approved by Purdue University s Contract and Grant Business Affairs office prior to start of a new subscription.
