1 UNCLASSIFIED MSG DTG Z JAN 17 FROM COMMANDER, AMCOM, REDSTONE ARSENAL, AL //AMSAM-SFA// SUBJECT - AVIATION SAFETY ACTION MESSAGE (ASAM), MAINTENANCE MANDATORY, AIRFRAME STRUCTURAL PARTS, H ASAM-04 This message is issued per AR to all Army Commands (ACOM), Army National Guard (ARNG), Army Service Component Commands (ASCC), and Direct Reporting Units (DRU). Commanders must retransmit this message to all subordinate organizations. Notify the AMCOM SOF compliance officer at "usarmy.redstone.amcom.mbx.safety-sof-team@mail.mil" when complete. Per AR 750-6, commanders/directors in the grade of major general or civilian equivalent may authorize temporary exception from message requirements. These exceptions may only occur during combat operations, matters of life or death in civil disasters, or other emergencies when aircraft operation is required. Commanders unable to comply with the requirements of this message within the time frame specified will change the affected aircraft status symbol to a Grounding //X//. Commanders, facility managers, and contractors at all levels will not issue aircraft until they are in compliance with this message. Aircraft transferred to the U.S. government via DD Form 250 after the message DTG are in compliance with message requirements. A listing of all messages and any required reports, supplements, or addendums can be downloaded at: " 1. END ITEM / MISSION DESIGN SERIES (MDS) AFFECTED - All H-60A/L and HH-60M (Black Hawk,) series aircraft. 2. SUMMARY Background - During depot level maintenance, a quality escapement was discovered on vendor supplied airframe parts used on aircraft undergoing rebuild, recapitalization, and battle damage repair. The suspect parts may not have the proper hardness value per the design specifications. The affected aircraft were identified through a records search, and each suspect PN was individually evaluated for its effect on airworthiness. Not all of the vendor supplied parts are substandard, but all are suspect until tested. Page 1 of 5
2 2.2. Message Purpose Require inspection of aircraft with suspect parts Schedule Depot Field Teams as necessary. 3. INITIAL AIRCRAFT TAMMS-A (THE ARMY MAINTENANCE MANAGEMENT SYSTEM - AVIATION) ENTRY - When complying with the message requirements, complete forms and records entries per DA PAM ULLS-A units will use appropriate "E" forms. Upon receipt of this message, make the following entry on the DA Form Enter a Horizontal Dash //-// status symbol with the following statement: "Comply with H ASAM-04, Airframe Structural Parts, by 11 JAN 17." 4. SPECIAL PROVISIONS TO MESSAGE REQUIREMENTS (AIRCRAFT) - For message purposes, aircraft away from home station are defined as not having access to a maintenance facility capable of meeting all message requirements Aircraft in Transit (surface/air shipment/ferry status/aircraft away from home station) - Unit commanders unable to comply with the requirement specified in paragraph 3 may defer making the Initial Aircraft TAMMS-A entry until arrival at final destination. Adjust the date in the entry to be 7 days after arrival at destination Exempt Aircraft - The inspection and correction procedures in this message are not required for the following categories of aircraft Maintenance Trainers (Cat A and B) Aircraft entered into the Black Hawk Exchange Sales Transaction (BEST) Program Activities/units with these aircraft will take the following actions to meet the message requirements Enter the required Initial Aircraft TAMMS-A entry on the DA Form per paragraph Note message compliance on the applicable DA Form per paragraph 8 by entering "H ASAM-04 not complied with" Compliance reporting via AMTRACKS is NOT required. 5. MAINTENANCE APPLICATION Level of Maintenance - Field at an Aviation Maintenance Company Estimated Time Required Time to complete inspection Verification of aircraft serial number - Total of 0.5 man-hour using 1 person Inspection/Testing of suspect components - Aircraft dependent. The time stated below does not include time for Maintenance Operational Checks or test flights. Page 2 of 5
3 Time for repair/replacement - Aircraft dependent. 6. ASSEMBLIES/COMPONENTS/PARTS AFFECTED - The affected parts list is maintained at the depot level. The parts do not have NSN s and can only be located on the aircraft drawings. 7. SPECIAL TOOLS AND FIXTURES REQUIRED - As determined by the Depot Field Team. 8. TECHNICAL PROCEDURES/INSTRUCTIONS - Unless otherwise stated, all message requirements shall be completed by the suspense date listed in paragraph 3. This message includes a required addendum. If not received with this message, the addendum may be viewed/downloaded at " Requests for exceptions (waivers) to this message will be submitted per GEN-14-AMAM-01, Message Exception (Waiver) Process, and AR For help, contact the AMCOM Safety POC in paragraph Referring to Part 1 of the addendum, determine if the assigned aircraft tail number is listed If the aircraft is listed, proceed to paragraph If the aircraft is not listed, the inspection is complete. Proceed to paragraph If the aircraft is listed in the addendum, make the make the following entry on the DA Form Enter a Horizontal Dash //-// status symbol with the following statement: "Airframe Inspection required per H ASAM-04." When a unit contacts the applicable Project Management (PM) POC, a schedule will be developed and coordinated for the Depot Field Team to inspect the aircraft at the unit location Units with suspect aircraft will contact the applicable PM POC as listed below to schedule the aircraft inspection. When contacting the POC provide the following information - POC Name, address, phone number, Unit designation and location/installation Active Duty, Reserve, and Other Government Agencies (OGA) will contact the PM POC listed in paragraph National Guard units will contact the PM POC listed in paragraph Clear the initial entry from paragraph 3 and note compliance on DA Form INSPECTION PROCESS Aircraft listed in Paragraph A of Part 1 of the Addendum A Depot Field Team will conduct the inspection and any required repairs. It is anticipated the inspection should be completed in one (1) day. Any part found to be discrepant will be replaced during a follow-on visit by the Depot Field Team. Dependent Page 3 of 5
4 on the parts identified, flight restrictions and/or periodic inspection instructions will be provided to the unit Prior to the Depot Field Teams arrival, the unit will be required to prep the aircraft before inspection can occur. Refer to Part 2 of the Addendum for the aircraft preparation instructions Aircraft listed in Paragraph B of Part 1 of the Addendum - These aircraft will be inspected by the unit using instructions which will be provided by the PM POC. The instructions will include the information needed to locate and test any suspect parts. If a component is found to be deficient, follow-on actions will be determined dependent on the component identified. Coordination with depot may be required depending on the component. 9. PROCEDURES/INSTRUCTIONS FOR ASSEMBLIES/COMPONENTS/PARTS IN WORK OR IN STOCK (AT ALL LEVELS INCLUDING WAR RESERVES) - N/A. 10. SUPPLY/PARTS (REQUISITION/DISPOSITION) - As determined by Depot Field Team. 11. COMPLIANCE REPORTING REQUIREMENTS - Report compliance with this message via the AMCOM Message Tracking System (AMTRACKS) at " Unit personnel designated to submit reports must establish a profile in AMTRACKS. AMTRACKS compliance reporting does not apply to aircraft operated by non- Department of Defense, Other Government Agencies (OGA). Those aircraft operated under "bailment" will report compliance per paragraph Aircraft Initial Compliance Report - All reporting requirements will be accomplished as part of the "Final Compliance Report" Aircraft Final Compliance Report - Submit the report via AMTRACKS by 17 JAN 17 per AR This report will consist of the Aircraft SN, MDS, Date of Initial TAMMS-A Entry, and check the box "Entered on DA Form " Retail Stock Inspection Compliance Report (Installation level and below) - N/A Wholesale Stock Inspection Compliance Report (including Depot Stock, Depot Maintenance, and Overhaul/Repair Facilities) - N/A OGA Bailment Aircraft Compliance Reporting (Homeland Security, Customs and Border Protection, FBI, etc) - Submit the reports via to "usarmy.redstone.peo-avn.mbx.avn-uh-oga-safety-reports@mail.mil".
