3 REQUEST FOR QUOTATION I. INTENT The purpose of the RFI is to request information regarding your company and your products and services. The same exact information will be gathered from different companies. The information that is received will be evaluated and used to select which companies will be invited to participate in a RFP. II. SNOQUALMIE VALLEY HOSPITAL S BACKGROUND The Imaging department serves between 4 and 5 thousand patients per year. We serve the needs of the Emergency department, inpatients, and outpatients. We operate a general x-ray room, Portable X-ray, CT scanner, Ultrasound department and MRI. Today we are looking to upgrade our entire X-ray room and transition from CR to DR. The criterion for the portable X-Ray machine that we are looking for is described in Attachment A. Please be sure that the information on the machine that is submitted functions on 52k of power. III. SCOPE Specific information is requested in Attachment A. IV. CONFIDENTIALITY RFI documents must contain sufficient information to be evaluated without reference to any confidential or proprietary information. Please do not include any information that you would not want to be released to the public. Any information submitted that is marked Confidential or Proprietary, or that contains material so marked, will be returned. V. VI. SUBMITTAL REQUIREMENTS In order to successfully answer the RFI please fill out Attachment A in its entirety. If you need more to augment the form to fit response, please do so in an effective and efficient manner.. All submittals will be accepted in two different forms. SVHD will accept electronic submittals which shall be sent to jessew@snoqualmiehospital.org. Secondly, proposals will be accepted by hard copy on 8-1/2 x 11 paper bound. 4 All submittals are required to be time stamped and received by September 27, 2013, by 4:00 p.m. The SVHD reserves the right to reject any proposals received later than the specified time. VII. COSTS There shall be no obligation for the SVHD to compensate a Company for any costs responding to this Request for Information. VIII. PUBLIC RECORDS The SVHD is a Public Agency as defined by State Law, and as such, all of the SVHD records are public records (unless otherwise declared by law to be confidential) and are subject to inspection and copying by any person. However, a proposal that requires negotiation or evaluation by the SVHD may not be disclosed until the proposal is recommended for award of a contract. 2
