Manufacturer Introduction to MacroPATH pro-x MacroPATH pro-x has been designed to carry out complete, safe and reliable image handling of gross histological samples. The system has been conceived for laboratory use and can therefore withstand the harshest laboratory conditions; equipment is intended for laboratory use (indoor use only). Because the MacroPATH pro-x auto-sizing tool is strictly related to the calibration done by the operator and is not controlled by the software of the unit, sample measurements obtained with MacroPATH pro-x are NOT TO BE CONSIDERED AS DIAGNOSTIC values. It is in fact necessary to position a ruler by the sample (see picture below) or to use a caliper to get a diagnostic value of the tumor growth, lesion extension etc It is necessary to calibrate the system (see chapter for calibration procedure) when making a measurement with MacroPATH pro-x. 6

3.6. LIVE functions In LIVE it is possible to create/retrieve a case, acquire images, take a video and modify extra camera settings. When the camera has not been used for about 6 hours, it turns off automatically to preserve its life, as explained below. At this time, the procedure of camera wake up starts: the live screen is stopped and the following message appears. The maximum time is 90 seconds, during this period do not press any button, just wait. Then it is possible to use again the camera. During the normal use of the camera, sometimes a fast buffering is automatically executed to extend its life. In this case the following message appears, for a maximum of 20 seconds. During this time do not touch any button. Then it is possible to use again the camera. In particular this procedure may be applied also when the camera is switched off by pressing its icon and then switched on and when entering in edit mode. In these cases the message Buffering showed above may appear for a few seconds, do not touch any button and wait. To extend the life of the camera, by default under Settings screen (see the chapter ), it is set a standby of the camera after 1hour. So when Macropath will not be used for 1hour, the camera turns off and the live screen becomes black. If you want to switch on the camera, press its icon, as shown below. When the camera is switched on, the message Buffering showed above may appear for a few seconds. 30 Camera Tab Select Camera to open the page, as shown below. Skip focus before snapshot: enable this option to maintain the showed focus. It allows a faster image acquisition. Snapshot quality: allows to set the acquisition quality. Follow also the chapter settings to decide the best compression quality for images. To set the image quality taken it is required to set properly two parameters as follows: 1. Compression Quality: the amount of compression applied to the image file (see chapter 3.8.4). 2. Snapshot quality: the dimension of the image. From these two parameters depends the size of the image file. The table below lists all possible combinations. Milestone suggests setting cameras to get Snapshot quality 16 Mpx and Compression Quality Standard (see chapter 3.8.4). As this is a good compromise between file size (disk consumption space) and quality of the image to report. Should images be used for publications (articles, papers etc), Milestone suggests to choose quality 16 Mpx and Full Data compression. Compression Quality Standard Full Low Snapshot Quality File dimension 16 Mpx 1.2Mb 7.2Mb 719kb 8 Mpx 516kb 3.6Mb 363kb 3Mpx 204kb 1,4Mb 133kb Note that the space required for each image is the double that which is reported on the table above. In fact, the system automatically saves each image in both JPEG and m5p formats. The changes will be saved in the configuration in use (see chapter 3.8.5) AWB Tab (auto white balance) Select AWB as shown below. White Balance: (Light Source Selection) During the installation it is NOT necessary to perform the white balance camera setting. The digital camera will auto-balance the white according to the light conditions where the MacroPATH PRO-X has been positioned. Therefore user has to select the type of light source in the laboratory among the options reported in the list showed in the image below, in order to perform the best color contrast. 30

By default Auto is selected and it is suitable when you are not aware of the type of light source of the grossing room. These modifications take place when the user press yes to the message that asks to confirm the saving of the modifications, and it is not necessary to restart the software. If several cams are present, start each one and modify settings for each cam. The changes will be saved in the configuration in use (see chapter 3.8.1) Areas Select Areas as shown below. By default, the camera applies a focus in the center of the screen; it is done automatically and it is suggested to use it. Anyway if you want to resize the focus area applied into the center, you can check Enable Focus area showed above. If enabled, the Focus area will use the values showed under Focus area that are: 500 as width and 400 as height. It is possible to modify these fields, if required. If you want also to visualize the focus area into the live screen, select also Show Focus Area (a green square will show the focus area before to get the snapshot). As explained above, the Area Focus is automatically centered in the middle of the screen. However it is possible to click on any point of the live window: the area will be automatically re-centered on the selected point and it will be resized to fit to the values indicated under Area Focus Click, showed in figure above. For 31

example below the green Area Focus is shown and it is centered on the lower right part of the screen, after a click in that zone. This function can be very useful: in case of big organs with different high levels of field: just select different parts of the live screen to get accurate focus and brightness of the area of interest, when a high level of zoom is used Camera firmware version In this section is shown the camera Firmware version. 32

To obtain (in case after a problem, you wish to receive a new license file): Insert the Product Key printed on the MacroPATH CD (see the arrow in the previous picture) Press Regenerate (indicated by the circle in the figure above), Press Copy to Clipboard button, to copy the Machine ID (indicated by the circle in the figure above), Paste in a text file or directly in the message, Describe the reason why it is necessary to have a new license, Send to customersupport@milestonemedsrl.com, On receipt the reply mail save the file you received from Milestone customer support, Press Import file (indicated by the circle in left figure below) and select the.lic5 file saved, Press Save button of settings screen (indicated by the arrow below). Restart MACROPATH and verify in Settings that the license level is Unlimited, as shown below. MacroPATH unit is provided with a full License file already installed. 42

Storage settings Click on Storage button to either change or add new the storage folders (up to 3). The system automatically saves cases in each selected folder. In order to perform an adequate and proper installation, in particular in case of multistation (more than one MacroPATH unit installed), it is strongly required that the units store the data on to a share folder stored on a server. To correctly perform the installation, as the software allows to store the data to more than one path, the 1st path has to be the shared folder present on the server. The shared folder has to be set with FULL CONTROL for each account and for each PC running the MacroPATH software. So the 2nd path should be the local folder (present on the internal hard disk drive of the PC-panel), then we can suggest the path, for Microsoft Windows 7: C:\Users\Public\Documents\Milestone srl\macropath5\images As shown in the image below (that shows an example for the 1st path): \\SERVER\Pathological Anatomy\Laboratory\MacroPath\IMAGES In the local folder will storage ONLY the cases created and opened by the local MacroPATH that you are using. The shared folder on the server will collect ALL the cases created and managed by all the stations/units. Sql Server icon not enabled. Data compression: it is possible to set three different compression qualities: Full Quality, Standard and Low file size. Refer to chapter to set the best compression according with the snapshot quality. High definition: if selected, a filter is applied on the image to have more constrast. By default it is not checked. When used, more seconds are required to make a picture. This function is not needed for camera pro-x (developed for previus models of camera). 43

Enabled shared folder management: The shared folder management is useful when more MacroPATH stations are working on the same cases. In fact, with this function activated, when a case is opened by a station and another MacroPATH tries to open it, a warning message appears, communicating that the case is already opened by another system and displaying its IP address and when the case has been opened. Below an example. Before using this function, please contact our Service Department at: customersupport@milestonemedsrl.com Camera settings Click on Camera button to modify camera settings. Select Cam1 or more such as Cam2, Cam3, Cam4 and Cam5 depending of number of cameras present. Click on Camera short name field to modify the name of the camera. Select: Trust external calibration: MacroPATH pro-x keeps the calibration factor from previous case Force calibration so that MacroPATH pro-x requires to calibrate every image (useful if camera is moved before capturing the image). Auto measure function not available. Vertical Flip Image: vertical mirroring. Horizontal Flip: horizontal mirroring Priority: in case of two or more cameras connected to MacroPATH pro-x, choose the selection sequence. Zoom mode: choose either Extended mode (default and recommended: 100% optical zoom) or standard mode (reduced optical zoom for optimal focus in special cases). 44

Foot Pedal: enable foot pedal (if connected) by choosing the serial number in the list shown. Keypad: not enabled Calibration Procedure: by pressing this button it is possible to calibrate the Unit, as explained in the following chapter Calibration Click the Calibration Procedure button and position the ruler on the work surface. Press Focus if the image is not well focused and press Go. Insert the distance (in mm) between the two edges of the image (see picture) Repeat the operation for different zoom levels (7 times). At the end press Save. The calibration procedure is required whenever the position of the camera or the type of the lens are changed. Each camera has its calibration, then each camera must be calibrated independently of each other Third party settings It is possible to create a shortcut icon to recall an external software (up to 3 different external applications, not included): Select the External application checkbox, as show below. In the image is reported an example to recall Notepad. To do that it is required to fill the fields under External application1: - Application : it is the address, in this case C:\Windows\System32\notepad.exe - Process : it is the name of the application, in this case Notepad - Image : it represents the image of the icon that will be visualized to open the application, it is required to browse an image.png, - Label: in this case Notepad. 45

By clicking Shutdown MacroPATH after launch, at the starting of the external application, MacroPATH prox closes down. By clicking Disable MacroPATH after launch option, at the starting of the external application, MacroPATH pro-x is disabled. When Disable MacroPATH after launch box is selected, a dedicated program (not included, to be provided by the customer/final user) must also be installed to enable user to return to MacroPATH. On the contrary, return to MacroPATH is not possible and user will have to restart the application MacroPATH trough the red cross CLOSE of windows system. DO NOT thick if not sure. Customization of the Exit button is also possible: add label name and icon path. Use MacroPATH pro-x camera and software as TWAIN devices of an external software (not included) is also possible: enable Install virtual driver and choose: Mode Volatile Shutdown Volatile Minimize Permanent Shutdown Permanent Minimize Description To remove the case data and close the MacroPATH after importing the images To remove the case data and maintain active (reduced to icon) the MacroPATH after importing the images To maintain the case data in the TWAIN folder and close the MacroPATH after importing the images To maintain the case data in the TWAIN folder and maintain active (reduced to icon) the MacroPATH after importing the images 46

4. MacroPATH pro-x AS A MULTISTATION VIEWER 4.1. How to install the MacroPATH pro-x as viewer MacroPATH pro-x as viewer allows simple retrieval of MacroPATH pro-x images, using either barcode or manual entry of patient identifying number. Images can only be viewed and printed (generating report as explained in the previous chapter), but no modifications i.e. annotations are possible. To carry out any modification on images (such as add notes, section and so on), an extra license of MacroPATH pro-x software must be available. For this, contact your local distributor. MacroPATH pro-x as viewer is the same software installed on the MacroPATH pro-x unit and it does not need a license. Therefore, user is authorized to install MacroPATH pro-x as Viewer software on any PC. For license activation see chapter MacroPATH pro-x Viewer can be installed on any computer: using the MacroPATH pro-x CD and open the MacroPATH pro-x folder. Click on Set Up installer and follow the instructions to proceed with the installation of the MacroPATH pro-x software Settings for MacroPATH pro-x Viewer PC Milestone strongly suggests that the following procedures are carried out by your local Network/IT Manager. Before proceeding with the instructions below, it is necessary to connect the MacroPATH pro-x PC and the MacroPATH pro-x as viewer PC to a local network, setting it with appropriate parameters. Settings the MacroPATH pro-x viewer PC: 1. Turn the MacroPATH pro-x viewer PC on. 2. Open the MacroPATH pro-x software 3. Click on Settings icon on the screen 4. In Storage field click on browse button 5. Select My Network Places folder, 6. Search the MacroPATH pro-x unit (the standard name of the PC is MACROPATH or MILESTONE, 7. Select the shared folder IMAGES then press OK 8. At the end, the full path is displayed (for example \\MACROPATH\images) It is now possible to work from both (or more) working stations. When a new case is added from histological laboratory, every additional station is able to view and print the same images. Milestone strongly suggests storing the cases on a server with an automated backup procedure. Contact your local Network/IT Manager. 53

4.3. Integration of MacroPATH pro-x with LIMS systems The images created with Macropath pro-x are 100% compatible (open system). Images are stored in.jpg formats which is universal formats. Any commercial software, pathology reporting, LIMS that are image capable are suitable to retrieve and view images collected by Macropath Pro-x. The system also produces a session file in each specimen directory detailing the Patient file ID, opening and closing time for the specimen gross file How MacroPATH saves the images Through the software interface, for every specimen case it is necessary to insert the ID code, by typing it through the keyboard or using the barcode reading. MacroPATH pro-x doesn t generate automatically any codes, so to start the system the operator is required to enter the specimen identifier file number. The MacroPATH system can also receive the ID number directly by the LIMS system Taking image Take image of specimen by zooming in/out with the foot pedal control until you have the desired area of interest, then press the Save pedal switch. The save feature is also possible by using the icons on the left side of the screen. The image saved will appear on the right of the screen in the film library. (Thumbnails) Where and how the images are saved: With Windows 7 all the specimens directory created by the user by typing the specimens codes are saved (as default) in the directory path C:\Users\Public\Documents\Milestone srl\macropath5\images While Windows XP the images are saved (as default) in the folder path C:\Documents and settings\all Users\ Documents\Milestone srl\macropath5\images. As default this directory is created into the hard-disk C:, but the folder Images can be set on any different location on the local network. The example below shows the way as 5 images taken under the case ABC are saved into the directory C:\Users\Public\Documents\Milestone srl\macropath5\images All the images are saved in.jpg format. The MacropathD/PRO-Xsoftware automatically saves the images with the ID name: ABC_0001.JPG ABC _0002.JPG ABC _0003.JPG ABC _0004.JPG ABC _0005.JPG 54

For every specimen case the software generates also an XML file with the information on the saved session. All the images are then available to be retrieved and visualized by any other software Interface command between LIMS and MacroPATH pro-x MacroPATH Pro-x can work as a standalone unit or interfaced to a LIMS system. Trough simple string commands the LIMS system can initialize MacroPATH pro-x with the ID code of the specimen. The command that the LIMS can send to MacroPATH to open the selected case is: macropath abc (name of the case e.g case: ABC) With this command the LIMS can open the selected case directly in the MacroPATH pro-x software 55
