3 1.0. Introduction to LimeSurvey v2.05 LimeSurvey is a survey creation and management tool allowing for the easy design and administration of participant self-reports and assessments. LimeSurvey uses Common Data Elements (CDEs) based on the NCI Thesaurus, when possible, to ensure the standardization of data. This guide is designed for end users of surveys in LimeSurvey LimeSurvey FAQs How can I tell which version of LimeSurvey I am using? In most cases there will be two versions of LimeSurvey used for your project: 1. The test version is where you will test and enter dummy data. The test version will have the word test in the URL. 2. The production version is the finished survey; its features will depend on your needs. The word Production will be in the title bar and title tab of LimeSurvey once you have logged in. Which browser is the best to use? Different browsers have different capabilities that can affect the appearance of templates and some functionality. LimeSurvey performs best with Firefox or Chrome. You can use other browsers, but these sometimes do not function as well. Is our LimeSurvey data being stored through LimeSurvey or within the UAMS system? The survey data is securely stored on UAMS servers. Who can edit responses? Permissions that specify who can and cannot edit responses may be granted to users. In older versions of LimeSurvey, only the survey owner (the person who created the survey) and super admins are able to edit responses. If you need to make changes you don t have permissions to make, submit a ServiceNow ticket, and IT Research will contact you. 3
4 3.0. Accessing Surveys You may access the surveys you have permissions for in a few different ways: Method 1: From the Administration Page List You may access surveys from the Administration page (see Figure 1), which you can always return to by clicking the Default administration page icon ( ). Just click the name of the survey from the list to open it. Figure 1. You may access surveys from the list on the Administration page. A survey may be Active or Inactive, as indicated by the Status symbol: indicates a survey is Active. indicates a survey is Inactive. Be aware that these icons work as toggle switches. Clicking them will change the status of the survey. Method 2: From the Surveys Drop-Down List You may also open a survey from the Surveys drop-down list (see Figure 2). The list will include both active and inactive surveys for which you have permissions. 4

Method 3: From the Detailed List of Surveys Icon You can also access surveys by clicking the Detailed list of surveys icon ( clicking a survey name from the list. ) then 4.0. Reviewing & Testing Surveys Once your survey has been created in the test instance of LimeSurvey, you will want to do vigorous testing to ensure it meets the needs of your project. IMPORTANT: Only enter test data in the test version. There are a couple of ways to open a survey for testing: Method 1: Open from the Administration Page If the survey is active, use the survey URL link to test it. You can ask the survey creator to provide you the link or open it from the survey Administration page that displays when you access the survey (see Figure 3). (See 3.0. Accessing Surveys for details on locating and opening surveys.) 5
6 Figure 3. You can review and test a survey by clicking its URL link. Method 2: Open Using the Execute This Survey Icon Click the Execute this survey icon ( ) from the survey Administration page. (See 3.0. Accessing Surveys for details on locating and opening surveys.) If the survey is inactive, the icon will say Test this survey Activating Test Surveys You must activate the test survey in order for other people to test it. Simply click the Activate/Deactivate toggle button from the Administration page to activate the survey. Click to Deactivate a test survey. Click to Activate a test survey. Only the survey administrator can Activate/Deactivate a study that is in production. The following sections apply to both activated test surveys and surveys in production. 6
7 6.0. Entering Data into a Survey You can enter data (survey answers) in two ways: Launch the Survey (preferred method) To directly enter into the survey, click the Execute the survey icon ( ) or use the survey URL link provided. This method allows for logic and other calculations to function fully. Using the link does not require an admin login and is therefore ideal when working with students or others who need to perform data entry but do not need access to the results. Use the Data Entry Screen The data entry screen is an option for easily and quickly entering data if, for example, you are doing a retrospective study and have paper forms to enter. This method is not preferred because it does not allow for logic or equations. It also requires an administrator login. To get to the data entry screen, click the Responses icon ( ) drop-down menu and choose Data entry screen, or click the Data entry icon ( ) from the Responses menu bar Viewing Data Once you have gathered your data, you will be able to view and extract responses. To view responses, click the Responses icon ( from the drop-down menu. ) and choose Responses & statistics You will be taken to the main responses page. The Responses bar has many options (Figure 4), several of which will be discussed here. Figure 4. There are many tools on the Responses menu bar, several of which will be explained here. To view the data, click one of the following: to display all responses to display the last 50 responses 7
8 The table you see includes the questions and coded answers. The codes are assigned during survey creation. You will also see any hidden or equation-type questions when viewing the responses Exporting Data There are multiple options for exporting surveys from the main Responses page, including export to applications and SPSS/PASW. Exporting to an Application You can export LimeSurvey data into one of these applications/formats: CSV Microsoft Excel PDF HTML Microsoft Word Follow these steps to export to an application: 1. Click the Export results to application icon ( ). 2. Choose an application/format, then select the options you would like (Figure 5). 8

Exporting to an SPSS/PASW Command File Follow these steps to export to an SPSS/PASW command file: 1. Click the Export results to a SPSS/PASW command file icon ( ) and fill out the required fields (Figure 6). Figure 6. 10 2. Click the Step 1 and Step 2 buttons and upload the exported files. LimeSurvey also provides Instructions for the impatient as an alternative method (Figure 7). Figure 7. LimeSurvey offers an alternative method for exporting to SPSS/PASW command files Extracting Statistics You can also extract statistics from the Browse responses page. LimeSurvey has a builtin tool for viewing the data. You can use it to filter your data then view charts and percentages or export the specific fields. 1. Click Get Statistics from these responses ( ). 2. Choose options from the General Filter box (Figure 8). 10

To save the graphs from the Statistics page, right-click your mouse and choose Save image as. Most questions show a pie chart. Multiple choice questions display as bar graphs Managing Tokens There are many options on the Token control menu bar, but this guide will focus on adding tokens. Follow these steps to add tokens: 1. Click the Token management icon ( ). 11
12 2. Click the Add new token entry icon ( ) to add a token entry. (For testing purposes, you can also create a dummy token by clicking the Create dummy tokens icon ( ).) Fill out the participant's information and assign a token number. C3PR will automatically generate tokens through the broadcast from C3PR. To have LimeSurvey generate a token, click the Generate tokens icon ( ). 3. Click Add token entry. 4. You will have the option to Display tokens or Add another token entry. 5. To view the tokens table, click Display tokens. 6. Now you can launch the survey, edit the token, or delete the token. 7. Click on the Return to survey administrations icon ( ) to return to the main survey administration page. 12
