1 Known Issues Report v /10/2017 Purpose This report is a compilation of known issues users and testers have reported to the MnCHOICES help desk and is intended to inform MnCHOICES mentors and end users regarding: Problems that were reported and documented Location in MnCHOICES where the issue occurs Function of MnCHOICES that is affected Description of the issue Steps to take to support the user NOTE: When we fix a known issue, we will place it in the fixed issues section at the end of this document. We will remove items from the fixed area of this list periodically. Date 2/23/17 Loss of free text in when timing out Session timeout resulting in logout of MnCHOICES Instead of all data entry being saved when a timeout occurs, some text is lost when the following sequence of events occurs exactly: 1. Login to MnCHOICES online mode 2. Create a new person record 3. Complete all fields in the new record, including text fields 4. Select a text field (that already had text in it) and edit it 6. Disposition the intake 7. Create the 8. Assign the Now: The user may do one of the following: 1. Avoid letting MnCHOICES idle for more than 35 minutes. 2. Navigate to the field where the additional text was input 17. Observe the most Session timeout from inactivity recently entered text was lost When the user logs into MnCHOICES for the first time, the session timeout works as expected. However, when the user logs in again and waits to be timed out, the session timeout message appears again for five more minutes before having a session timeout. This is true both on- and offline. The following sequence of events occurs: 1. Login. 2. Let MnCHOICES idle for more than 35 minutes. Now: The user may do one of the following: 1. Avoid letting MnCHOICES idle for more than 35 minutes. 2. Respond to the pop-up message within 5 minutes. 3. If logged out, close the bowser and open a new browser session before logging back in.
3 1/27/17 Connection to server interrupted while working remotely 1/23/17 Unable to run PCA Printout For Providers Report 1/23/17 Offline eligibility summary report queue Reports Reports 3. Receive pop-up message and do nothing. 4. Wait 5 minutes. 5. User is automatically logged out. 6. Login without closing browser from previous MnCHOICES session. 7. View an. 8. Let MnCHOICES idle for 35 minutes. 9. Receive pop-up message and do nothing. 10. Wait 5 minutes until the pop-up disappears. 11. Let MnCHOICES idle another 35 minutes. 11. Receive a second pop-up message and do nothing. 12. Wait 5 minutes. 13. User is automatically logged out after 80 minutes Taking s offline Running the PCA Printout For Providers Report Eligibility summary report total instead of 40. Occasionally, when a user selects to take (s) offline, and the server connection experiences a degradation or interruption, the system presents error messages. The format of the eligibility summary report is different Now: The user must re-establish a stable connection to the DHS servers, via a stable connection to the internet with sufficient bandwidth to transmit data efficiently. Note: If the internet connection is insufficient to cleanly stream video, issues may also exist with transmitting data via MnCHOICES. Now: No work-around exists to restore the ability to run the PCA Printout for Providers Report. Users with this problem should contact the SSIS help desk. Now: The user may view, download or print either version of the
4 differs from online eligibility summary report 1/23/17 Error messages are reported 1/23/17 Infinite loading when running eligibility, no error message 1/23/17 Items are logged online but offline log record is blank General Triggering ILOG determine or save eligibility in online mode versus offline mode. The online version is the newest format, whereas the offline version is a previous format. Occasionally, when a user selects to view the eligibility summary of a closed document (e.g. eligibility update, HRA full, and new ), the eligibility summary is blank and the error message "IBlog file does not exist on your computer or on the Server" appears briefly and then disappears. Also, occasionally when a user creates HRA Full with all of the counters at zero, errors appear upon selecting a disposition. When the person is over 65, IBlog reports needing a "Parent/Guardian" question answered. This problem is related to a lost or degraded connection to IBlog, the application's rule engine. Eligibility summary tab Running eligibility When a user selects to run eligibility and the counters are not all at zero, MnCHOICES attempts to run eligibility and presents the barber pole indefinitely. Event log Viewing the event log entries Occasionally, after an has successfully run eligibility online, is then taken offline, and its eligibility is run offline, the event log shows no record of eligibility having been run offline or online. eligibility summary report in its respective mode of operation. Now: The user must wait a few minutes and attempt to view the eligibility summary, or disposition of the HRA again. If waiting a few minutes does not help, the user should log out and log back in, and make another attempt. If the problem remains unresolved, the user should contact the SSIS help desk. Now: The user must log out, and log back into MnCHOICES. Then the must be completed and eligibility run with all counter values reading zero. Now: No work-around exists to restore the event log data on offline mode. Users should return to online mode and select to view the event log data.
5 1/23/17 Deselecting your Unsaved Offline Selections. Please Re-Enter your Date 1/23/17 User encounters infinite loading when clicking the view button on any type queue queue Opening a new Opening an eligibility update Occasionally, when the user selects an date for a new, MnCHOICES presents an error message that reads "Deselecting your Unsaved Offline Selections. Please Re- Enter your Date." This error message in no way represents anything done by the system; nothing was changed with any offline selections. Occasionally, when the user first assigns an date for an eligibility update at the queue, and selects "View," MnCHOICES presents the barber pole indefinitely and does not navigate to the eligibility update. 1/23/17 Conflicting answers Selecting answers Occasionally, when a user selects one answer for a question, MnCHOICES saves more than one answer to the database, which creates an error when running eligibility. These errors are displayed on the eligibility summary tab when they occur. 1/23/17 s can be pre-dated if was assigned before today s date 1/23/17 Fetching Questions barber pole remains on- Assigning an date Completing an A user can set the " Date" in queue before the today's date for an existing or re. Now: The user must deselect responses to the erroneous questions, and reselect intended answers. If the problem persists, the user should contact the SSIS help desk. 7 1/23/17 Updates to demographic information do not always populate 1/23/17 User encounters A Server Error Has Occurred while entering data 1/23/17 User encounters Error saving data to the server while adding an intake 1/23/17 Person search does not work after changing a password Demographic information General error extends to other data fields, or how frequently it Viewing the intake or Navigating within the happens is unknown. Occasionally, demographic information entered into a person record does not copy to the intake record, or the demographic data updated in the intake record does not fully copy over to the. This occurs when the user selects [View] on the document history grid, on the persona and activities page to open the intake record or, immediately after the "barber pole" finishes spinning after selecting to update the person's record. When navigating between domains and groups in the, MnCHOICES loses its internet connection, fails to navigate to the selected domain or group and presents an error message. General Adding an intake The user creates a person, populates the person s data fields fully on each tab. Then, the user selects to add an Intake. The system experiences an error saving the person data as an intake. General Changing passwords When a user changes a password, the system may not be able to perform a person search. Now: After updating a person's demographic data in the person record or the intake record, and selecting the "Update Person" command button, the user should wait a few seconds after the barber pole disappears from view before selecting to view the person's intake record or from the document history grid, on the person and activities page. Now: Log out and log back into MnCHOICES. Now: This error has not been reproducible on command. Should this error occur, log out of MnCHOICES, close the program, reopen the program, and log back into MnCHOICES. User may have to re-enter person data. If a user encounters this error, it should be reported to the MnCHOICES help desk. Now: The user must log out of MnCHOICES, and close the program. The user should clear the computer's cache. Then, the user should reopen MnCHOICES and log back in.
8 1/23/17 Browser reports that "co.dhs" is not responding 1/23/17 Event log tabs are missing events for s 1/23/17 Pressing [F5] logs a user out of MnCHOICES 11/17/16 Question counts are not clearing General General Running eligibility, or restoring a previous eligibility Occasionally, when a user selects to run eligibility, the spinner continues indefinitely and the browser reports that "co.dhs" is not available. The eligibility does not run. This can also be reproduced in the "Eligibility Log" tab on a record that has had eligibility run at least once. The events of assigning an and opening may not appear in the event log. General Pressing [F5] on the queue page to update the page sends the user to the "About MnCHOICES" page with the error message, "You are not authorized to view the Person Search." The page has no menu options. Running eligibility Occasionally, when a user opens an, the counter for required questions shows one or more remaining. However, when the user views each group, all the required questions appear to be answered, but the counter still reads greater than zero, which prevents determining eligibility. This is caused by changing answers to display additional required questions, then changing answers back again to hide those required questions. The required number of questions changes, but the system fails to adjust the counter correctly. Now: When the issue happens, select "Recover Webpage." The system will recover and the user may attempt to run eligibility again. Now: To confirm that an was assigned or opened, the user must reference the assignment queue or queue to see the with related data. Now: Do not press the [F5] button. If the [F5] button is pressed by accident, the user must log back into MnCHOICES. Now: The user may do the following: (1) exercise caution by not doubleclicking radio buttons, (2) deselect and reselect question answers with mandatory indicators, (3) go to Person and Activities screen and select the Update Person command button, or (4) contact the SSIS help desk for data-fix within the.
9 6/4/16 The report generated from the person and activity screen is missing data 4/1/16 Getting an error message when running eligibility 3/1/16 No view button in offline mode 2/29/16 The Check Out for Offline Use button does not always appear on-screen 2/24/16 Launching from SSIS and the top MnCHOICES main menu bar does not display report Generating the report The report generated from the person and activity screen has less data than the same report generated from the print tab at the screen. Eligibility Summary tab Running eligibility When a user runs eligibility, goes back to the, changes nothing, and tries to run eligibility again, an error message sometimes appears. No further work can be done in MnCHOICES. queue (offline) -queue screen in online mode Login Viewing and completing s in offline mode Selecting s to go to offline mode Navigating from SSIS to MnCHOICES User logs in to offline mode, goes to queue and does not see the view button. User is unable to open the in order to edit it. After selecting one or more s to take to offline mode and selecting Save Selections, the system fails to present the Check Out for Offline Use button. The user cannot take MnCHOICES offline. Occasionally, when launching from SSIS when there is new content to download, the MnCHOICES main menu bar does not display. This happens with initial and all subsequent launch attempts. Now: The report should only be generated from the print tab at the screen. Now: Close out the error message and log out of MnCHOICES. Close the browser. Then, reopen the browser and log back into MnCHOICES. Now: Return to online mode. Clear browser cache and Silverlight Isolated Storage. Click the Increase Storage button, and download the content again. Check out for offline mode and confirm that the View button is now visible. Now: The user must select another within the queue to go offline and select the Save Selections button. The Check Out for Offline Mode button should then appear on-screen. If this work-around does not work, the user may contact the help desk to reset the s in the queue so the user may start over. Plan: Try to duplicate and identify the cause of this problem to fix it. Now: Delete the downloaded content using the user profile page. Then close the browser and re-open MnCHOICES directly from the URL and not through SSIS. The user will be prompted to update the content again. After updating the content
10 2/24/16 Launching from SSIS when the user s password has expired 2/24/16 Launching from SSIS when the user s password is going to expire in seven days or less 2/17/16 An error message appears when running eligibility when no error exists 8/25/15 Address information not Login Login again, the menu should be visible when launching from SSIS. If the menu still does not display, the only work-around at this time is always to launch MnCHOICES directly from the URL and not SSIS. Navigating from SSIS to MnCHOICES Navigating from SSIS to MnCHOICES Launching from SSIS when the user s password has expired causes the login to fail, and MnCHOICES does not display an error message indicating the password has expired. Launching from SSIS when the user s password is going to expire in seven days or less causes the login to fail and MnCHOICES does not open properly. - Eligibility tab Running eligibility When the users runs eligibility, an error message appears on-screen that says, ORA-00001: unique restraint error occurred. Demographics Updating person information Occasionally, when a user removes the address field Plan: Try to duplicate, identify and fix cause. Now: User should consult his or her MnCHOICES mentor to reset the password through SSIS. There may be up to a 20-minute delay in the new password synchronizing between SSIS and MnCHOICES Plan: Change the password expiration warning to display when the user is launching through SSIS. Now: User should close the browser and open MnCHOICES directly from the URL and not through SSIS. After logging in, the user will see the password-expiration warning. The user must use the Change Password feature under the User Profile menu or change it through SSIS. Either way, there may be up to a 20-minute delay in the new password synchronizing between SSIS and MnCHOICES. Plan: Change the password expiration warning to display even if the user is launching through SSIS. Now: Ignore this error message (click OK to take it off-screen). The system is calculating eligibility correctly. Now: There is no work-around at this time. The development team

Fixed Issues These issues have been fixed since the last release of MnCHOICES, and/or the last release of this document. Date 1/23/17 Slow performance when opening large domains and groups 1/23/17 Intake data is not saved as the user navigates from domain to domain 1/23/17 Fetching Questions box remains on screen during intake Navigating inside the When the user selects to navigate from one domain or group to certain others, the system takes longer than expected to present the domain or group on-screen. The user experiences impatience, and selects to navigate again, which makes the system take even longer. The slowness is caused by the system taking more time to save and present larger domains and groups. Intake Completing an intake Occasionally, while completing an intake, answers are lost when navigating to a different domain and then returning to the previous domain. Intake Completing an intake Occasionally, when navigating from the "Reason for Contact & Referral Source" domain to "Demographic Information" domain by selecting the "Go Now: Wait 20 to 30 seconds for domains "General Health," "Symptoms, Conditions & Diagnosis," "Treatments & Monitoring" and "Behavior/Emotion/Symptoms." Wait six to 10 seconds for other domains and groups. Now: When the user observes the issue, the user must navigate to the queue; then return to the intake queue; and reenter the intake. MnCHOICES will retain entered data in the intake domains correctly. Now: The user should select a domain name on the left of the screen. MnCHOICES will navigate the user to that domain, and the barber pole will disappear.
