1 Page 1 of 13 HOW TO ENROLL FOR EMPLOYEE HEALTH CARD THROUGH ONLINE Get ready with the following documents: Self:- a. Photo: Scan a 45 mm x 35 mm ICAO compliant passport size colour photograph of 200 Kb size. b. Service Register (two pages): Scan the pages 1 and 2 of old service register (or) pages 4 and 5 of new service register with your name etc. clearly visible. c. Aadhar Card/Receipt: Scan the Aadhar card with your name and number clearly visible if you are giving the Aadhar number (or) scan the Aadhar enrolment receipt with your name and enrolment number clearly visible if you are giving the Aadhar enrolment number. d. Disabled Certificate: Scan your disability certificate if you are disabled. Dependant Family members: (Scanned copies) a. Photo: 45 mm x 35 mm ICAO compliant passport size colour photograph of 200 Kb size. b. Aadhar Card or Aadhar enrolment number Receipt (scanned copy) c. DoB Certificate: Date of birth certificate if the dependent family member is less than 5 years of age. d. Disabled Certificate: Disability certificate if family member is disabled Procedure for login and filling the application at (EHF PORTAL) 1.Click on sign-in 2.Enter your user ID : (your Treasury code) Password : (your Treasury code) Select Login as: Employee If you are a pensioner, sign-up with a PPO id and password. ***contact your DDO for your employee details

2 Page 2 of 13 if you are unable to Login using your employee treasury Id and password, you may try the following. Click on forgot password link in the sign in page The following windows will appear. Then click on button Click here to Proceed Enter your treasury Id select login as Employee

3 Page 3 of 13 Enter your mobile number & mail id then click on update The following screen will be appeared saying that password is sent to your mobile number and mail id. Then try to login with your treasury Id and Temporary password. (After login for first time please change your password. New password will be sent to your mobile)

4 Page 4 of 13 Your login screen will appear as below 4.select Initiate Health card/ view application by clicking on Registration as shown below 5. Now select radio button Adhar Number or Adhar enrolment Number then click on Retrieve Details button as shown below

Enter the Details in the Application form Select your head of the department, Drawing and Disbursing Office unit, Category of your post from the dropdown lists in the application, submit the data, and attach the needed documents. and save it. Note: DON T CLICK ON SUBMIT APPLICATION UNTIL ALL THE DATA IS FILLED AND ALL THE ATTACHMENTS ARE ADDED.

6 Page 6 of To add attachments (scanned copies of photo, 1& 2 pages of old service register and 4,5 pages of new service register) click on Add attachments button. A new window as shown below will be opened. Attach files and click on upload button 7. To add the dependents click on Add Beneficiary button as shown below which appears in employee application. 8. Then a new window will be opened as below. (please ensure that pop-ups are not blocked) Enter the D.O.B of the dependent & click on submit

7 Page 7 of Now select radio button Adhar Number or Adhar enrolment Number and enter the same, then click on Retrieve Details button as shown below 10.The following window will be opened then Enter the Dependent details and attach the scaned copy of adhar card/ adhar enrollment receipt and photo of Dependent and click on submit. 11. Verify the accuracy of data. If there are errors, correct and save. 12.Afeter entering all the required data and uploading the attachments click the submit application button. After submission, you cannot make any changes to the form 13. Submit the application by clicking on Submit the Application button.

8 Page 8 of You will get an alert message as below. Click on OK BUTTON. 15.Your submitted Application will be opened. Then Take a print out of the filled up application and sign it.

10 Page 10 of Upload a scanned copy of the signed application form in the EHF portal & press Submit for Approval button. 17. Take the physical copy of the signed application form and give it to your DDO 18. Note that employee (incumbent) is responsible for furnishing correct details of dependent family members in the online enrolment application. Any false declarations will entail disciplinary action against him. 19. You will receive an sms and acknowledgement (if the is provided in the e-form) as soon as the application reaches the DDO. 20. If your DDO rejects your application, you will receive an sms and notification (if the e- mail is provided in the e-form). In such case Resubmit your application with corrections 21.You will receive a notification through sms/ when your card is printed and ready for issue. 22.Approach the Card Issue Centre (CIC) in your district with all your dependents. The CIC locations will be notified by your District Collector. 23.The card will be issued after taking your biometric fingerprint. The fingerprint will be your acknowledgement of receipt of card.

11 Page 11 of 13 Personal Details Data needed for filling Enrollment Application Form Employee ID* PPO number* Name* Aadhar Number* Aadhar Enrollment Number* DOB* Sex* Marital Status* Date of Retirement* Community* Disability Details* Nationality* Obtain the employee code given by DTA If you are a pensioner obtain the PPO number Write down the name as it appears in your service register Write the Aadhar Number as it appears in your card Write the Aadhar enrollment ID as it appears in the acknowledgement Slip given at the time of Aadhar enrollment Write DOB as it appears in your service register Write your Gender Write your present marital status If you are retired,write down the date on which you retired Write the community to which you belong to If you are disabled, write down the type of disability and percentage of disability Write you Nationality Contact Details Write your complete residential address, location, address and mobile Address* phone number.note that your and mobile phone number have to be Identification Details carefully written as you will be communicated through these modes. Ration Card number Identification marks* If you hold a ration card, write down your ration card number. Ration card number will make tracking your cases easier and quicker for us. Therefore don't forget to inform the number in case you have one. Write two visible identification marks such as moles with exact location on your body. Posting Details

12 Page 12 of 13 HOD* District* DDO code* Post* STO/APPO* Write your Head of the Department Write the district where you are currently posted Obtain the DDO code of DTA where you are currently receiving your pay. Write your current category of post. Ex: Civil Assistant Surgeon, Kamati, Driver, etc. If you are a pensioner, obtain the STO/APPO code and name of office from where you are receiving your pension. Pay Details Current Pay Write down you current pay. Attachments needed Service Register* Obtain a scanned softcopy of the first two pages of the service register Obtain an ICAO compliant passport photo of self and each dependent family Photo* member in softcopy. ICAO compliant photo is one which we submit while applying for Passports. Tell your photographer beforehand about this requirement. Aadhar card* DPB certificate* Disabled certificate* Scan a copy of either your Aadhar Card or the Aadhar acknowledgement slip with the details clearly visible. If any of your dependents is less than 5 years of age and does not have an Aadhar number, then prepare a scanned soft copy of his date of birth certificate. If any of the members is disabled, then prepare a scanned soft copy of the disability certificate. Family member details Name, relationship, aadhar number* Write the name, relationship with your, Aadhar number or Aadhar enrolment ID number of each of your dependents. Declaration Signed application After you enter all the details in the e-form, you need to print it and sign the

13 Page 13 of 13 form with declaration* form. Scan the form and prepare a soft copy. The declaration says that, you declare that (1) you agree to the entire Aadhar data of your family to be transferred to AHCT, (2) you are liable for disciplinary action for declaring ineligible family members as dependents, and (3) the information submitted is as per your service register and true to the best of your knowledge.
