1 BLUE MOUNTAIN BREWERY BUS & LARGE GROUP REGISTRATION POLICY Are you planning to visit Blue Mountain Brewery with a large group or bus tour? Please note that we can ONLY accommodate groups of 25 or more with PRE- REGISTERED approval. Bus groups that are not pre- registered cannot be accommodated. To register your group, please review the attached policies and send the registration form back by to josh@bluemountainbrewery.com. Or you can fax it to A confirmation letter will be sent to you by if your registration is accepted.

2 Bus & Large Group Registration Policies Large Groups are accommodated Monday-Thursday & Sundays from 11AM to 9PM and Friday & Saturday from 11AM-1pm only. You must arrive within 15 minutes of your approved registered time to ensure we can accommodate your group. If you are late, there is a very good chance we will have used your space. Registration requests must be sent to josh@bluemountainbrewery.com and must include a completed registration form. We strongly recommend that you plan well in advance to ensure we can accommodate your group. Just returning this form does NOT guarantee that Blue Mountain Brewery can accommodate your group. You must receive registration approval first. If you plan to visit multiple breweries on the day of your visit, Blue Mountain should be an early stop on your itinerary. Intoxicated guests will not be allowed onto the grounds or into the restaurant, regardless of prior reservation. Please be sure to have a valid form of ID if you plan to purchase alcohol. Guests must be 21 years of age to purchase or consume alcohol. Acceptable IDs include: valid US driver s license, passport, military ID, or US identification card A credit card must be on file to officially register your group. Failure to show up or cancel your reservation at least 24 hours before your scheduled visit will result in a $ charge.

3 PLEASE SHARE THE FOLLOWING RULES OF CONDUCT WITH YOUR GROUP: Virginia law prohibits persons from consuming any alcoholic beverages not purchased at Blue Mountain Brewery on our property. If any member of your group brings any outside open alcoholic beverage to our property, we will not serve anyone in your group, and we will require that your group leave our premises. No refund of fees will be made. Keep the open alcohol containers on the bus. Do NOT bring them on the grounds, in the parking lot or in the buildings. Under Virginia law, we cannot serve anyone that is intoxicated. We will assume you are intoxicated if you are disrespectful or do not follow the directions of our staff, or you are otherwise loud or disruptive to the staff or other patrons. You will be charged $10 per occurrence if any member of your group attempts to take glassware off the property, i.e., back on the bus or car or into the parking lot. It is ILLEGAL to take open alcohol into the parking lot or onto the bus/car at Blue Mountain Brewery.

You will have the opportunity to use another form (or forms) of payment on the day of your visit.
