5 DEAR CUSTOMERS RE: LIFELINE SERVICE AND LINK-UP PROGRAM The Federal Communications Commission has established two programs to make basic telephone service more affordable to eligible residential customers. The Lifeline and Link- Up programs of the federal Universal Service Fund ensure that all Americans can get basic telephone service by providing limited discounts to consumers who might not otherwise be able to afford service. Lifeline involves discounts on the monthly charges, and Link-Up involves a discount on the cost of initiating telephone service for residents of Tribal lands. Lifeline benefits are also now available for subscribers of broadband Internet access service at minimum service standards defined in FCC rules. FCC rules also prohibit more than one Lifeline service per household. Who Qualifies? To participate in the program, subscribers must either have an income that is at or below 135% of the federal Poverty Guidelines or participate in one of the following assistance programs: Medicaid (SoonerCare) Supplemental Nutrition Assistance Program (SNAP) Supplemental Security Income (SSI) Federal Public Housing Assistance (FPHA) Veterans or Survivors Pension Benefit Subscribers who live on Tribal lands and who qualify under one of the programs above or one of the following programs, are eligible for additional discounts under Tribal Lifeline: Bureau of Indian Affairs General Assistance Tribally administered Temporary Assistance for Needy Families (TANF) Head Start Programs (only applicant or customer who satisfies the income qualifying. eligibility provision) Food Distribution Program on Indian Reservations ( FDPIR ) What does Lifeline service include? Lifeline service includes the following functions: Voice grade access to the public switched network Local usage at no additional charge Access to emergency services, such as 911 and E911 Toll limitation for qualifying low-income consumers Lifeline customers may also subscribe to optional services at the same rate offered to other customers. How to Apply for Lifeline and Link-Up Program? Residential customers may request self-enrollment forms from the Shidler Telephone Company business office. Lifeline is limited to one benefit per household. (This form must be renewed yearly.) How to Apply for Telephone Service? If you do not have telephone service and wish to obtain telephone service and you are in the Shidler Telephone Company serving area, or if you have telephone service and you move and you qualify for the Link-Up Program and you are in the Shidler Telephone Company serving area, please call business our office number and we will be happy to assist you.
