1 Refresher Training Survey Results July 2008 STATE QUESTION 1 QUESTION 2 QUESTION 3 Does your state / territory / commonwealth require providers to have some sort of refresher training / continuing education / skill verification, etc. after initial training / certification / licensure has been done? If yes, how often? How are student competencies (cognitive / affective / psychomotor) verified in the refresher cycle process? Alabama Gary Mackey (334) gmackey@adph.state.al.us Arizona Joel Bunis (602) Bunisj@azdhs.gov Arkansas Norajean Miles Harrell (501) Norajeanmiles@arkansas.gov Colorado Marilyn Bourn Mbourn@cdphe.state.co.us Connecticut Kevin Brown (860) kevin.brown@ct.gov For EMT-B, we require a 24-hr. Refresher. For EMT-I and EMT-P, we require a 48-hr. Refresher, all levels except paramedic (licensed versus certified, with expectation that the medical director will set credentialing expectations) Once every 2 years Once every 2 years is our certification period with recertification 3 years for three cycles (total 6 years) and at 3- year intervals thereafter. We also require Protocols (standing orders) review, there are done by oral scenario testing and handson demonstration Arizona requires a 150- question written exam which has to be passed with a 70% or better, plus we use the NREMT skill sheets. Formal refresher requires testing of cognitive and practical skills. Continuing ed. (in lieu of formal refresher) is a variety of courses. At the advanced level (Intermediate- 85 & Paramedic) the medical director must sign off on the recertification paperwork and verify skills. Programs are approved by the State of Colorado and each program does their verification independently. Cognitive competencies are assessed through regionally administered written exams; psychomotor competencies are assessed through locally administered skill tests; affective competencies are not formally assessed. 2 District of Columbia Robert W. Austin (202) prior to renewal of certification Florida Rickey Stone (850) , Ext Cognitive Idaho Tawni Newton (208) Iowa Joe Ferrell (515) First Responder & EMT-B 3 year certification periods Advanced EMT & Paramedic 2 year certification period 2 years At the BLS level, students are required to attend a 24-hr refresher course and obtain 24-hrs of CME. At the conclusion of the refresher course, providers are given a 50-question test. At the ALS level, the provider must obtain 72-hrs of CME. During their two year certification period they must be successfully evaluated by a Dept. of Health certified evaluator. The evaluations must be an ALS incident, and are typically performed in the field. We recently began the process of accepting two evaluations performed using a simulator (such as SimMan). The provider cannot talk their way through, but must perform the actual skills on the manikin. The evaluations are placed into a package and reviewed by the Paramedic Review Board a board of seven members (nurses, physicians and paramedics) make recommendations to the Dept. of Health concerning recertification. This will be changing somewhat in the near future as we are going to National Registry certification for both initial certification and renewal. Attested to by training officer or medical director for BLS (FR and B), and by agency medical director for ILS (Advanced) and ALS (paramedic). Not done by state. Competencies must be assured by services during licensure. Page 2
3 Kansas Carman Allen (785) Maine Jan Brickman (207) Massachusetts Russell Johansen Minnesota Debra Teske (651) Biennially Every 3 years For Basic EMT 28 hrs of continuing education and 24- hr DOT refresher with skills/exam. There is no verification of competencies. Providers must submit copy of certificate from an approved (DOT curriculum) refresher training program; or, if using CEH s, the CEH class must be pre-approved either through the Maine EMS Office or one of our Regional Offices. Each provider must complete a prescribed number of CEH s in the categories designated for their licensure level. Skill labs and exam FR US DOT 16-hr refresher and CPR for the HealthCare provider. EMT-B US DOT 24-hr refresher, practical skills exam and CPR for the HealthCare provider or 48 hrs of continuing ed. in the National Standard Curriculum modules 4 hrs in each of the six modules remaining 24 in any/all modules, practical skills exam at an EMSRBapproved training program and CPR for the HealthCare Provider or current NREMT certification if living in another state. EMT-I 48 hrs of continuing ed. in National Standard Curriculum, CPR for the HealthCare provider (no practical skills exam, unless required by employer) EMT-P 48 hrs of continuing ed. in National Standard Curriculum, ACLS course (no practical skills exam, unless required by employee). Dec Nebraska will be changing from 3 yrs to 2 yrs for the provider to receive the required continuing ed hrs. Every 3 years Skills evaluator during mandatory refreshers, training officer and medical director statement of competencies. EMT-B 100 hrs and BCLS EMT-P 144 hrs and ACLS Ground, air, and nontransporting agencies have a requirement for annual skills. Skills sign off (as part of the required refresher) by lead instructor approved by State Agency. FR from 20 to 14 hrs EMT-B from 30 to 20 hrs EMT-I from 45 to 30 hrs EMT-P from 60 to 40 hrs Nebraska requires that all continuing ed must cover information that is found in the (1) Nebraska Approved Protocol, or (2) within the Nebraska EMS Rules and Regulations or (3) found in the DOT Curriculum. The continuing ed must be taught by a certified EMS instructor or person who has expertise in the topic. All instruction must stay within the scope of practice of EMS as defined in Protocols, Rules and Regulations or Curriculum. Per NREMT + Protocols Review Students must recertify in one of two ways: 1) Take a Refresher course that includes a State Practical and Written Exam. Or 2) Recertify using our CME program, skills are verified and CME s documented. They are not. New Mexico Lauren Pena (505) Lauren.Pena@state.nm.us Page 4
5 Pennsylvania Beth McAteer (717) Texas Brett Haart (512) Vermont Mike O Keefe (802) MOkeefe@vdh.state.vt.us Virginia Warren Short (804) Warren.short@vdh.virginia.gov Texas has five options to recertify personnel must recertify using one of these options. (1) Written Exam Recertification Process (2) Continuing Ed Recertification Process. The certificant shall attest to accrual of dept. approved EMS continuing ed as specified in of this title (relating to Continuing Ed) (3) National Registry Recertification Process. The applicant shall attest to and hold current National Registry certification at the time of applying for recertification. (4) Formal Course Recertification Process. The applicant shall attest to successful completion of a dept. approved recertification course. (5) CCMP Recertification Process. (5) CCMP Recert. Process An applicant affiliated with an EMS provider that has a dept. approved Comprehensive Clinical Management Program (CCMP), for all levels. Paramedic annually. FR and EMT every three years Every 4 years BLS every 4 yrs ALS every 3 yrs We offer continuing ed, 18 hrs annually for Paramedic, 24 for EMT, and 16 for FR. You can also retest but most opt for the continuing ed ours is very vague we only split the continuing ed into med trauma or other. In refresher courses, the EMT course coordinator, through the approved education program verifies proficiency. In the CCMP course the EMS agency medical director verifies proficiency. FR & EMT-B Practical & Written exam, EMT-I-90 & EMT-I-03 Renew EMT-B plus get signature of district medical advisor recommending renewal. EMT-P NREMT requirements. CE Program includes skills conducted by state-approved courses. Then the provider must either take and pass the written exam or the provider can be waived from taking the written exam by signature of the Operational Medical Director whose signature indicates competency by provider.
