4 Creating Your Submission File Your submission file consists of records extracted from your registry database in a particular manner based on the parameters listed below. Every registry software system is different, so you will need to consult your software provider for specific instructions. When you create the submission file, be careful to note its filename and path (the drive and folder) where it is located. Most software systems offer a function to automatically select the appropriate cases for you; NCDB recommends using that capability. Required Diagnosis Dates All analytic cases diagnosed from 2004 through 2016 PLUS Cases diagnosed from 1985 (or your Reference Date if it is later) through 2003 must be submitted if they were changed by the registrar on or after December 1, Full details are available at Cases diagnosed before your Reference Date or outside the range will be rejected. Cases must have a Date of First Contact between 1985 and 2018, inclusive. Class of Case Analytic cases (Class of Case 00, 10, 11, 12, 13, 14, 20, 21 and 22) are required Any other Class of Case will be rejected Reference Date All cases meeting the above criteria diagnosed on or after your Reference Date must be submitted Cases diagnosed prior to your Reference Date will be rejected Types of Cancer The types of cancer required to be accessioned and followed are listed on page 3 of FORDS. Other cancers collected by agreement at your facility should not be extracted from your hospital registry database, and will be rejected if submitted. You may combine submission years in a single data file or separate them; however, note that there is a limit of 5000 cases per submission file. New Programs Submitting for the First Time If you are a new program submitting for the very first time you will need to submit your full analytic caseload for each year from your Reference Year through Using NCDB GenEDITS Plus 2018 to Edit Your NCDB Data, December 2017 Page 1
5 Resubmissions of Corrected Data Resubmissions to correct edit problems that may exist in your initial submission are generated using the same steps as used for initial submissions, and for the Call for Data 2018, must be made by June 1, It is desirable to submit only cases that required correction, but full diagnosis year resubmissions are acceptable. The initial NCDB submission deadline for the 2018 NCDB Call for Data is midnight Central time the night of June 1, Acceptance of NCDB resubmissions to correct survey deficiencies will be by appointment only. For deficiency resolution, contact Kathleen Thoburn via at Downloading and Installing NCDB GenEDITS Plus GenEDITS Plus is included in the EDITS Tools developed by the Centers for Disease Control and Prevention (CDC). The EDITS Tools are used for defining standards for data quality and for checking data quality during collection processes. These programs have been in widespread use by facility and central registries for over two decades and can be incorporated for use within registry software. GenEDITS Plus is an application that registrars use to edit facility data. The NCDB GenEDITS Plus 2018 installation was generated using the generic program. However, this NCDB-specific software includes all required edits metafiles and configuration files and is pre-configured to run the NCDB Call for Data edits on your data submission file. To download NCDB GenEDITS Plus 2018, complete the following steps: 1. 6 2. On the left-hand side of the page click click National Cancer Database. Result: The National Cancer Database sub-options are displayed. 3. Click Data Submission Information. Result: You are directed to the Data Submission Information page, where you are provided access to information for registrars, instructions for editing your data, instructions for file transmission, and information for software providers. 7 Read and become familiar with the Data Submission Information for Registries before you prepare to submit your data. Installing NCDB GenEDITS Plus 2018 To install and run data quality edits on your NAACCR Version 16.0 data submission file using the NCDB GenEDITS Plus 2018 software, complete the following steps: 1. On the Data Submission Information page, click Editing Information or go directly to the following URL: Result: You are directed to the NCDB Edits Software and Metafile page. You can only use the NCDB GenEDITS Plus 2018 software if your registry is currently using NAACCR version Note: Prior to submission to NCDB, the NCDB GenEDITS Plus 2018 should be run on the actual data file to be submitted. 2. Click NCDB GenEDITS Plus 2018 Software Download Installation. Using NCDB GenEDITS Plus 2018 to Edit Your NCDB Data, December 2017 Page 4
8 Result: You are prompted to Run or Save setup_geplus4_ncdb_2018.exe. If your facility has security policies in place that prevent you from downloaded an.exe file directly from the WWW, click NCDB GenEDITS Plus Software Download Zipped to download the installation file in zipped format. 3. Click Run. You can also download the installation file to your computer and install it later by double-clicking on the.exe file. If a User Account Control dialog box opens and prompts you for an administrator password, you do not have Administrator rights to your computer and cannot install programs yourself. You will need to have someone with Administrative rights to your computer install the NCDB GenEDITS Plus 2018 program. Result: The User Account Control dialog box opens. 9 4. Click Yes. Result: The GenEDITS Plus NCDB 2018 Setup Wizard opens to the Select Destination dialog, which tells you where the application will be installed on your computer. 5. Leave the default location of C:\RegPlus\GEPlus4_NCDB_2018, and click Next. Result: The Select Start Menu Folder dialog opens, which tells you the name of the program shortcut that will be listed in your Start Menu folder. 10 6. Leave the default name of GenEDITS Plus NCDB 2018, and click Next. Result: The Select Additional Tasks dialog opens, which tells you the name of the program shortcut that will be listed in your Start Menu folder. 7. Leave the default of Create a desktop icon checked, and click Next. The desktop icon is a shortcut to GenEDITS Plus NCDB 2018 that is placed on your computer s desktop: Using NCDB GenEDITS Plus 2018 to Edit Your NCDB Data, December 2017 Page 7
11 Result: The Ready to Install dialog opens, displaying all of the installation settings. 8. Click Install. Result: When the installation process is complete, the Completing the GenEDITS Plus - NCDB 2018 Setup Wizard dialog opens. 9. Click Finish. Result: NCDB GenEDITS Plus is now installed. You are provided with the option of running the program immediately by checking the Launch GenEDITS Plus 2018 checkbox (prior to clicking Finish), or closing the installation window. Running NCDB GenEDITS Plus Once you have NCDB GenEDITS Plus 2018 installed on your computer, you will need to apply data quality edits to your data submission file to identify any edit errors that may exist within your data file. If you do not already have the application open, you will need to launch NCDB GenEDITS Plus. GenEDITS can be launched in 2 ways: i. Double-click on the GenEDITS Plus NCDB 2018 icon on your desktop ii. Go to Start All Programs GenEDITS Plus - NCDB 2018 GenEDITS Plus - NCDB 2018 Result: The application opens to the main window of the program, defaulted to the Run Edits tab. 13 NCDB GenEDITS Plus comes pre-configured to apply data quality edits to your NCDB Call for Data submission data file: Metafile = NCDB_28_v16E_ rmf, Record Layout = NAACCR EXCHANGE VS 16 for NCD, Edit set = NCDB_28_v16E_ Config File = NCDB_28_v16E_ ini Edit set Options = Skip Failed (if a case fails a single item edit (e.g., for Primary Site), then no inter-item edits that use that item are run (e.g., Primary Site, Laterality). Report Configuration has all case identifiers specified: Reporting Facility (FIN) Accession Number Sequence Number Primary Site ICD-O-3 Histology Date of Diagnosis Abstracted By Vendor Name The only task left before you can run the edits is to specify the actual data file against which you would like to apply the edits. If you are pre-editing cases for RQRS, use the configuration file RQRS_28_v16E_ ini Both the NCDB and RQRS configuration files will select the same metafile, record layout, and options, but they will pick different edit sets to be run on your data. RQRS has fewer edits than NCDB, so when pre-editing for the Call for Data be careful to use the NCDB configuration file. Identifying your Data File and Running the Edits Now you are ready to identify where your data to be edited are stored. To run the edits complete these steps: 1. Return to the Run EDITS tab by clicking it. Result: The application returns to the Run Edits tab where the NCDB configuration file, metafile, record layout, and edit set have already been specified. 14 2. To the right of the Data File Name box, Click Browse to navigate to the data file you saved from your registry database. Select the file and click Open. In the example shown, the data file named TestFacility.XCD is being selected. Result: When you select your data file, the boxes on the Run EDITS tab in the User Options section will automatically be filled in with information based on your data file. The Record Type must equal I, the NAACCR Record Version must equal 160, the number of records displayed in the Stop Editing after Record box should correspond with what you believe the number of cases included in your data file. Use this information to confirm that you have selected the right file. In the example shown the data file has 2759 records. Using NCDB GenEDITS Plus 2018 to Edit Your NCDB Data, December 2017 Page 11
15 3. Once you have confirmed that you have selected the right file, click Run EDITS. Result: The four boxes located along the bottom of the Run EDITS tab will display the number of Records Processed, the number of Records with Errors, and the number of edit errors found (Messages Reported). An indicator bar will indicate the progress of the editing process on your file, and the Time Remaining until the file is completely processed is also displayed: When your file has completed processing, the Report Options window will open. Three types of error reports are offered. The Detailed Report describes each edit failed for each case report. The Summary Report provides only the number of cases failed for each edit. The Combined Report provides both. By default, the Summary Report omits edits with no edit failures. The NCDB edit set include over 700 edits so this option makes the Summary Report easier to read and saves paper if you print it. You can view the results from every edit by unchecking the Omit Zero Counts box. For the most efficient page output, select Page Filled. Note that the name of the data file is automatically appended to the name of the type of error report selected. You can view each of the reports, but be aware that the last one selected is the one that will be generated and saved for future reference. If the file has no edit errors (Total Exceptions = 0), the Summary report version will be sufficient. NCDB recommends selecting the Combined report if any edit errors are present. 16 For registry use, select the Text Report Format. The CSV format can be read by some software or loaded into Excel for further review. Edit Reports 1. To generate a report, select the type of report you wish to view, and click Generate Report. Result: The specified report is generated and automatically opened in Microsoft Word or Notepad. In the example shown, the combined report has been generated. The Summary Report Cover Page lists the report file name and location, the amount of time it took to process the data file, the version of GenEDITS Plus software that was used, the data file name, the metafile name and edit sets run, the number of records in the file and the number that were processed (Processed Records), the total number of records having errors (Total Error Records), the total number of edit errors across all records processed (Total Messages), the total number of warnings (Total Warning Messages), as well as any global editing options that were specified prior to running the edits. As mentioned earlier, in the NCDB configuration, the option of Skip Failed is specified. Result: The Detailed Report section of the report is displayed. The edit report lists the following data items as case identifiers for each errant record in the report: Reporting Facility (FIN) Accession Number Sequence Number Primary Site ICD-O-3 Histology Date of Diagnosis Abstracted By Vendor Name The Detailed Report includes the case identifying information listed above, the edit name, the error or warning message, and the values for every data item checked by the edit. 17 The Edit Name is used to identify which edit is involved if an edit error or warning occurs. The identifier in parentheses at the end of the edit name simply refers to the original source of the edit; most edits are used by multiple standard setters. The Error or Warning Message is a short description of the nature of the problem. Following the edit or warning message is a list of the data items checked by the edit and their values in the case record. Please note that the edits do not indicate which of multiple data items checked caused the problem. As a result, you should never assume that one data item being checked is always the issue and needs to be corrected. A full review of each data item should be performed to resolve the issue. Basically to resolve each edit error you should: 1. Review error message/edit description 2. Note the fields to be reviewed 3. Review the codes 4. Verify the codes through review of Text 5. Resolve the error After all edit problems have been corrected in the registry database, a new submission file should be extracted from the registry database again, and the edits should be re-run. This editing cycle should continue until no edit errors remain. Once all problems have been satisfactorily addressed, the data are ready to be submitted to NCDB. 18 The NCDB_28_v16E_ Call for Data Submission Edit Set This edit set affects program standing according to Standard 5.6: Annually, cases submitted to the National Cancer Data Base that were diagnosed in 2003 or more recently meet the established quality criteria and resubmission deadline specified in the annual Call for Data. For the Call for Data 2018 and later all cases diagnosed on or after January 1, 2004, are subject to Standard 5.6, regardless of the reasons they are submitted. NCDB s data quality submission edits check single-item validity (is the value a defined code for the item) and inter-item logic (can this combination happen according to the coding rules) for all data items collected by NCDB. NCDB s data quality submission edits are drawn from the CoC All Hospital edit set in the standard NAACCR 16E metafile, with the following adjustments: Call-specific edits written by NCDB Class of Case 00, 10, 11, 12, 13, 14, 20, 21, 22 only Diagnosis Years only; years of first contact only ICD-O-2 and ICD-O-3 versions for site and morphology (none earlier), based on date of diagnosis An edit for over-ride validity is limited to over-rides published in FORDS NPI numbers are present for the reporting facility Because physician NPI numbers are located differently in the NCDB submission, an NCDB version of the standard edit is used to check for valid NPI numbers; they are otherwise the same as the standard edits NAACCR Record Layout and Record Type Check Edits that check the layout with respect to the metafile in use (these items are not in facility registries but are added by registry software for submission) NAACCR Layout version = 160 NAACCR Record Layout = I Incidence record (only coded, non-confidential items may be submitted) Special Date Last Contact Edit The RQRS edit set also has an edit requiring that the Date of Last Contact has known year, month and day. Date of Diagnosis and Conversion Considerations Edits for data items whose requirements have changed over the years take date of diagnosis into account, along with any conversion requirements for the item over time. All submissions or resubmissions made at this time must follow the current specifications, regardless of when the case was diagnosed or initially submitted. 19 The NCDB validation system checks the file in ways the edits cannot. It checks to see that the file contains registry data and that the person who submits the data is a registrar or co-registrar for the specific facility or a software provider approved by that person. If the dropdown FIN selection at the time of submission does not match a FIN for the program whose data are being submitted, the submission will be rejected and will not process. Resolving Identified Edit Issues Most edit problems can be resolved by comparing the codes in your data with FORDS or another applicable manual (CS, MP/H, AJCC, etc.). A.pdf file posted at the applicable section linked from describes every edit in the NCDB v16e metafile. If those resources do not resolve your questions, contact ncdb@facs.org. If your edit report tells you that you have errors in the patients last names, you ran the prototype state edit set that automatically downloads with GenEDITS Plus rather than NCDB s edits. NCDB does not collect patient names and does not edit for them. Use the instructions in section V above to select the NCDB submission edit set. Running Edits in Your Registry Software NCDB recommends use of the most current CoC hospital-all edits in the edit set used by your registry software to time stamp cases as Date Case Completed --CoC as a first step in data preparation, but requires programs to run the final checks using NCDB GenEDITS Plus 2018 once your submission file has been created in order to be certain you are checking the same things NCDB will edit. 20 Appendix A: NCDB_28 v16e Edits and Scores The following pages show which edits are included in each of the edit sets selected by the NCDB and RQRS configuration files, and how NCDB scores those edits. NCDB assigns scores to each edit that is failed by a submitted case. The scores are used to direct processing and to help select the better record when more than one record applies to the same case. Multiple records are submitted when a facility updates or corrects a previously-reported item (a submission duplicate) or when a patient receives care from more than one accredited program (a clinical duplicate).
