Your retail and mail order Prescription Benefit Manager (PBM) will be CVS/Caremark. Caremark will assist participants that have open mail order prescriptions to transfer information to their system. For any prescription benefit questions, you can contact Caremark at or visit them online at To locate a participating Caremark pharmacy near you, please contact Caremark at the toll free number and/or website listed above. Caremark is one of the largest PBMs in the country and contracts with all major chain pharmacies and more than 98% of the independently-owned pharmacy stores nationwide. If you have a current mail order prescription, Caremark will assist you in transferring your information over to their system. If you have any questions, please contact your CoreSource Customer Service Department Monday through Friday, 8:00 AM to 6:00 PM Central Standard Time with any claims, eligibility or benefit questions. Better yet, you can visit the portal to your health plan when it fits your busy schedule at anytime of the day or night. Take advantage of all that mycoresource.com has to offer: Claim Lookup and Account Balances View claim detail. Use a variety of filtering and sorting capabilities to help you find specific claims faster, including the ability to sort by patient status, type or service date. Site Security and Login Intense security protects members information. Create separate logins for family members, and have the ability to block certain information from other members of the household. Online Message Center Gain quick, direct access to Customer Service. Immediately send questions about a specific claim while viewing it. 5 A Member s Guide To Registering on mycoresource.com To register as a member on mycoresource.com for the first time, you will need to follow these steps: 1. Go to mycoresource.com and select the Create My Account button in the I am a Participant box. Each plan member will need to create their own account.
6 2. The Account Creation security page is a feature that protects members important information. a. Type the numbers shown. b. Select the Next button. a. b.. Create an account: a. Enter a username and password. The system will tell you immediately if the username has already been taken. Usernames must be four or more characters and contain at least one non-alpha character (not including an *, &, %, (), = or + sign). Passwords are case-sensitive and must be 6 to 2 characters long with at least one non-alpha character. b. Enter your name and the address that you want to use to recover your username, in the event you forget it. c. Select and answer three security questions. d. Select the Next button. d. a. b. c. Note: If at any time, you forget your username or password, you can select the appropriate link under the login button on the home page, and then follow the prompts.

CONVENIENT ECO-FRIENDLY SECURE How to sign up for electronic EOBs How to view your electronic EOBs Visit mycoresource.com and log in to your account. Not registered yet? Select Create an Account to get started. Select either the Update Contact Information link, or the Enrollment tab at the top left. You will be directed to the Personal Information page. Add or edit your address using the Electronic Contact Information section. In the Preferred Communication Method table, select the Explanation of Benefits box under your preferred address, and select the Save button to save your updates. You ll receive an at the address listed on your mycoresource.com account letting you know when you have a new electronic EOB available. The provides the instructions to access your electronic EOB: Select the link provided in the and enter your password (your five-digit ZIP code and the last four digits of your Social Security number). If you need to access a previously issued EOB, simply log into your account at mycoresource.com and click on the Claims tab at the top. Once you begin receiving electronic EOBs, you will no longer be mailed a paper version. However, you can easily print your electronic EOB for your records. 13 understanding your EXPLANATION OF BENEFITS Members receive easy-to-understand Explanation of Benefits (EOB) with consolidated information on medical claims and payments made for each family member. In this guide, CoreSource provides definitions of EOB terms and a sample EOB. Each numbered definition below corresponds to one of the numbers on the sample EOB on the following pages Group Number Number assigned to the employer by CoreSource Print Date Date the check was issued Patient Name Name of person who received the service Type of Service Description of the visit (e.g., physician visit) Claim Number This number identifies the claim in our system Other Adjustments Negotiated or ineligible amounts that are not a member s responsibility Other Plan Payment A payment made by another health plan due to coordination of benefits Ineligible Amount of submitted charges not covered by the plan Copay A predetermined charge that the provider can collect from you at the time of service Reason Codes Used to explain why a portion of submitted charges is not covered by the plan. A number, or reason code, shown on the EOB corresponds with an explanation. (See page 2 of example.) Patient Account Number Account number assigned by the facility or professional provider that rendered the service Provider Name of facility or professional provider that rendered the service Description of Service A brief description of the services for which the provider billed Service Date The date the provider indicated the services billed for were received or rendered Billed Charges Services for the member that have been billed to the member s health plan Discount Amount The amount that has been reduced from the provider Deductible The amount of the covered charge that the patient is responsible for before health plan payment begins Co-Insurance A percentage of the submitted charges not paid by the health plan for which the member is responsible Plan Benefit Total amount that will be paid by the plan for the submitted charge(s) Plan Paid At Percentage of the covered expense paid by the plan, after any applicable deductible Issued Date the claim was released and sent to processing to send payment or an EOB statement Patient Responsibility Portion of total submitted expenses for which the member is responsible Family Dollars applied toward the employee and covered dependents Current Year Benefit payments made during this year Prior Year Benefit payments made last year * Depending on how the claim was paid, these columns may appear differently. The claim will be paid with in-network, out-of-network or PPO rates. These columns will include payments toward your deductible, out-of-pocket costs and lifetime medical maximum allowance.
