2 Welcome to your Infinisource COBRA Online Portal. This portal gives you anytime access to view information, manage your profile, provide COBRA information and download reports. It enables you to: Enter a new enrollee Enter a qualifying event Review a Qualified Beneficiary s information including notices and payments received Download Infinisource reports Process reports Review/change profile information Review/update plans and rates To login to your home page, follow these steps: 1. Navigate to the Infinisource login page. 2. Enter your Login/ and Password. First time users will enter your assigned User ID (Login) and Password. New users will be prompted to update security settings by entering address, new password (must contain one letter, one digit, one special symbol and be at least 8 characters) and recovery information. Note: Unique User IDs and passwords should be generated for each COBRA online user. Our online system stamps all transactions with date, time and user information. Access to online features is also governed through role-based security features. Multiple users are supported. To add users, contact Customer Service at You will be asked to change your password every 60 days for security purposes. For help retrieving a lost or forgotten password click the Forgot Your Password link below the Login. For help retrieving a lost or forgotten UserID, contact Customer Service at
3 New users will receive a prompt once all their new security settings have been saved. COBRA Online Home Page The online portal contains many features to assist you with reporting information to Infinisource along with reviewing data already submitted and checking on the status of a COBRA participant. Here s a review of what s available from this Home Page. If you report for multiple accounts, you will select your account here. COBRA Dashboard The COBRA Dashboard contains a quick overview of the most important features for you as an Infinisource customer. Customer Profile The Customer Profile is information regarding your account. Customer Contacts The contacts listed for your account.
4 Services Lists active services you have with Infinisource. Recent Notices Mailed The ability to review recent notices mailed and view those notices directly from your COBRA dashboard. Plans for Renewal This provides you with the ability to review plans that will be up for renewal within 60 days. When plans are active or have gone beyond the expires date, they will not appear in this box. If you have plans/rate information for these, you can click Go to Renewal Plans to update the information online. Recent Reports Click View to review any reports that have been provided to you in your Download Center.
5 Infinisource Account Reps This provides you with information on your Infinisource Account Representatives. If you have multiple services, you may see multiple names listed here. Active COBRA Participants If you currently have anyone on COBRA, they will appear within this box with basic information regarding their COBRA status. New Enrollee Online Form Submit this form when someone first enrolls in one of your group health plan benefits, which alerts Infinisource to generate a General Notice. The New Enrollee is a simple, three-part form: Employee: Your employee s name will always be the header on these records.
6 Enrollee: When the entire family resides at one address, submit only one Enrollee, generally the employee or the spouse. If any family member resides at a separate address, click the Add Enrollee icon, and submit that family member as an additional enrollee. A separate notice will be mailed. Submit: When you are done adding enrollees, click Submit to have the record saved to our database in real time. Qualifying Event Online Form Submit a Qualifying Event record whenever someone loses coverage due to a COBRA qualifying event. The Qualifying Event is a simple, four-part form: Employee: Your employee s name will always be the header on these records. Qualified Beneficiary: When the entire family resides at one address, submit only one Qualified Beneficiary, generally the employee (this depends on the type of qualifying event experienced). If any family member resides at a separate address, click the Add icon. Enter that family member at the appropriate address as an additional QB. A separate notice will be mailed to them at that address. For HIPAA Certificates, every family member must be submitted. However, if the additional family members all reside at the same address, you may uncheck the Generate Event Notice box to suppress additional COBRA Election Notices.
7 Offered Coverage: Using the drop-down menu, select a coverage and option combination available to the QB. Enter the Loss of Coverage date as the last date covered as an active participant. Be sure to use a four-digit year format. The Waiting Period Began and Coverage Began dates are only needed for HIPAA Portability Administration. Click the Add icon at the top of the online form and repeat these steps for each coverage and option combination available. Due to independent election rights, you should enter all available coverage and options (e.g., single medical, employee + 1 medical and family medical). Submit: When you are done adding Qualified Beneficiaries and coverages, click Submit to have the record saved to our database in real time. Qualifying Events Find Employee/QB The Find Employee/QB function allows you to view detail on individual qualifying event records. Use * as a wildcard, when necessary. Each record has multiple tabs offering you unique detail.
8 You can view recent and older records by putting a checkmark in the Include Archived box. in the You can see participant invoices and payments. To view actual notice copies, select Beneficiaries, and then go to the Notices tab. You can download actual notice copies in PDF. Reports Online Reports Allows you to process a report on demand and review now or deliver to your Download Center. Reports include notices mailed, Participant Status report, events and enrollments entered and plans/rates.
9 Data Centers Download Center Allows you access to your Infinisource reports in a secure manner. In the Download Center, you can search for reports three ways by clicking in the applicable circle: My Files: Reports addressed to you, including those you have already downloaded Account Files: All reports to all recipients My Files that have not been downloaded yet Your search results will be returned here. Click View to open your report. Reports can be saved to your local computer or network in PDF or Microsoft Excel formats. To locate files that have already been downloaded, select Account Files. This search provides you with an archive of your reports.
10 Data Centers Upload Center This allows you to submit data files directly to Infinisource for automated processing of COBRA qualifying events and new enrollments. Personal Setting Personal Profile Allows you manage your Infinisource account. You can change your password here. Personal Setting Technical Info If you experience problems while utilizing the website, you can check your technical information against our system requirements. If you continue to have difficulties, you can submit information on what you are doing and the problems you are having to Infinisource from this page.
11 Account Settings Profile The Account Profile allows you to view your account information in the Infinisource system. Account Settings Company Reports Sent To Provides information on who is receiving reports, the export format and how they are receiving them. Account Settings Carrier Reports Sent To Provides information on what carriers are receiving reports, the export format and how they are sent.
12 Guides and Documents Other Support and Resources The Source Infinisource publishes important information about compliance issues, legislative updates and enhancements to our services. These updates are sent to you through our newsletter, The Source. You can view our newsletter archive by following the Guides and Documents link on the left-hand side of any COBRA Online page, under Resources. The Newsroom The Newsroom provides timely updates on changing regulations and compliance issues on topics such as health care reform, cafeteria plans, COBRA, FMLA, payroll, time and attendance that affects your workforce management.
