1 Graphics Interface 2006 Release Form Canadian Human-Computer Communications Society This form must be completed and signed by the contact author of every accepted paper that is to appear in the proceedings. Only papers for which this form has been signed and sent physically to the following address, accompanied by any necessary payment and hardcopies, and for which an electronic (PDF) form of the paper has been uploaded, will be printed. As we have to divide up the sheets in this release and send them to different people, please print it single-sided and sign everywhere a signature is requested. This document includes BOTH a CHCCS copyright release form and an ACM copyright release form. The CHCCS release lets us print your paper and distribute it via AK Peters. 5 PAGE CHARGES My paper contains pages, of which are colour pages. The following charges are in Canadian dollars. Personal or institutional (employer) cheques are preferred (in Canadian OR U.S. dollars). It is MUCH easier for us to handle cheques than VISA or MasterCard payments, even for cheques in U.S. dollars. However, cheques must be drawn on a Canadian or U.S. bank. If you wish to pay in U.S. dollars when paying by cheque please use an exchange rate of $1.20 CAN to $1.00 USD. You may also pay by a money order or bank draft drawn from a Canadian or U.S. bank. 2. Cheques should be made out to the Canadian Human Computer Communications Society, and should mailed to the same address as manuscript (see above), in the same package. Please put your paper number in the memo field of your cheque. 3. The only credit cards accepted are VISA and Mastercard. Sorry, we cannot accept American Express. 4. The name of our merchant account (for both VISA and Mastercard) is Canadian Human Computer Communications Society. Check here if you are paying by cheque, bank draft, or money order drawn on a Canadian or U.S. bank:.
