1 AP41a Guidance Document Elmhurst Energy Assessor Accreditation Application Section 2 This is where you must indicate which strand e.g. NDEA, OCDEA or DEA you are applying to be accredited on and which region you will want to work in. Please note that you can apply for multiple strands and regions at the same time. Section 3 To be completed by the applicant, with your personal information (not your employers).

2 Section 4 This section must be completed with information of who will be invoiced for the lodgement charge. E.g. If you are a self employed DEA but work for a company who will be paying for lodgements. You would need to complete this with their company details and someone from the company would need to sign the declaration. However, If you were lodging EPCs for a company and they are not paying the bill this would be completed with your details. If different to section 3. Section 5 For Domestic Energy Assessors only. You must have a criminal record Disclosure dated within the last 12 months. It has to be either the original or a authenticated colour copy.

3 Section 6 Elmhurst Insure all strands and does not offer discounts for those insured else where. Section 7 This section must be completed if you have ever been or you are currently accredited with another accreditation scheme. As this will determine what QA rate you go on with Elmhurst Energy. If this is not completed you may be put on a higher QA rate than you should be on. Section 8 This section states which qualifications we accept to be accredited via this application. If you do not have a qualification from one of these Awarding Bodies please contact Elmhurst to discuss which route you need to be accredited by.

4 Section 9 For NDEA, DEA and OCDEA applications you must send a colour copy of either your passport or photo card driving licence. For NDEA and DEA accreditations you will also need to provide a passport size photo. Section 10 DEA When you are accredited you will be sent an ID and password to Access Elmhurst where you will be able to lodge EPCs using our RdSAP software. OCDEA To lodge OCDEA EPCs you will need to purchase the Elmhurst SAP software, please click here for more information and the application form. NDEA and DEC You will need to tick which software you will be using to lodge NDEA EPCs or DEC s. Currently this list is the only software that Elmhurst receive assessment data from.

5 Section 11 This section must be read by you and if you are working/lodging EPCs on behalf of a company they must also read the bill payers authorisation. You must sign in the applicants signature section. Bill payers signature must be signed if you are lodging EPCs on behalf of a company. Bill payers full name and position must be completed by a Manager or Director level. Section 12 Please use the checklist to ensure that you have included everything in the checklist when sending your application in for accreditation. If all the information from the checklist is not complete this will delay your accreditation.
