6 ABOUT USER GUIDE: To improve and streamline the logistics and supply chain of the Family Planning commodities under the National Family Planning Programme, the Ministry of Health & Family Welfare, Govt. of India has developed an innovative Family Planning Logistics Management Information System (FPLMIS). This application will assist to track and manage the Family Planning commodities from the National level to ASHA level. This document will be used as guidance for using the FPLMIS application. Contents of the document will help users to understand the functions of the application and the flow between the modules. Page 6

7 Family Planning Logistics Management Information System (FPLMIS) 1. Introduction: A robust logistics system is crucial for uninterrupted supply of contraceptives for delaying pregnancy and ensuring healthy spacing between children. The system should ensure the right goods, in the right quantities, in the right condition, delivered at the right place and at the right time for client satisfaction and continued use. As per the needs of reproductive health program with specific reference to family planning commodities, LMIS has been developed with an intention to track the supply chain accurately and on real time basis from National level to ASHA level. The application would facilitate supply chain operations by reducing supply imbalances of family planning commodities and ensure uninterrupted availability to the end users. 2. Objectives: To track the supply chain from a single point To forecast annual contraceptive requirements To manage supply of contraceptives efficiently To minimize stock outs and reduce supply interruptions. 3. Salient Features: Web based, App based and SMS based application Instant access to stock information from National level to ASHA level Auto forecasting of contraceptives SMS alerts for key indicators Auto generated indicators and reports for program review Page 7

Supply chain flow chart GMSD MoHFW Supplier CMSS State SDH District DH Block/ CHC Supply Order PHC / SC Commodity ASHA Supply Chain Flow Chart The FP division submits the annual demand for each contraceptive to the SSM division in the MoHFW, who would in turn issues the purchase order to its suppliers for procurement of FP commodities. The suppliers of SSM Division delivers the commodities directly to each state warehouse through GMSD as per the state wise requirement provided by the FP Division. The suppliers of CMSS deliver the commodities to its regional warehouses only which are located at 20 different states as per the requirement provided by the FP Division. The State distributes the commodities to the district warehouses and subsequently to the Block CHC or PHC. The Block store then distributes to the Sub Centers and ASHA accordingly. Page 8

9 5. Functionality: The software can be accessed by typing the (yet to be approved) at the address bar of the internet browser (best view in Mozilla Firefox). The software is password protected, only an authorized user can access to make transactions, generate reports or can view dashboard indicators. 5.1 Login: An individual User Name and Password will be given to every user. The user has to enter User Name and Password. Step-1: Enter User Name, Password and Captcha then click on Login button after successful login, menu screen will appear Login Form: Errors and exceptions in Login Your Login may fail due to any of the following: Incorrect User Name or password User login expired Page 9

10 Captcha validation failed Network / Server failure In all cases of errors, the system will display relevant error message. Incorrect Login name or password The system will display Invalid User name and Password message:- Incorrect Captcha: The system will display Captcha Validation failed message:- Page 10

11 5.1.2 Forgot Password All the Users have been allotted user name and password. It will enable the user to log in to the Application. The password is like a digital signature. It is very imperative that one should keep one's password secured. Never disclose your Password to anyone and do not keep your name, family member s names or anything that can be guessed by people around you as Password. Note: - If the user forgets password then the following steps should be followed: Step -1: Click on Forgot Password link: Following pop-screen will appear, where select radio button as Question. Step-2: Enter User Name, Hint Question and Answer then click on next button. Following screen will appear: Page 11

12 Step-3:Enter the New password and Confirm Password then click on Save button. On clicking save button your password will be changed. 6. Annual Demand The purpose of this process is to raise the Annual Demand of contraceptives by the States and UTs Annual Demand Generation Step-1: After successful login, home screen will appear: Page 12

13 Step-2: On clicking Forecast menu under main Menu bar, a sub menu will appear. Step-3: Click on Annual Demand. Page 13

14 On clicking annual demand, following screen will appear. Step-4: Select Demand Period, Store Name and To store Name and then click on the Go button. The default values for all the fields will be selected when page appears for the first time. Note: Store Name: It defines the name of store, which is raising the demand. To Store Name: It defines the name of store, to which Annual Demand has to be raised. Step-5: On clicking the Go button following screen will appear. Page 14

15 Step-6: A User can search item by the search item box and the user can select the item for which Annual demand needs to be filled by clicking on Lock button. Step-7: When the user clicks, on Lock button that particular item becomes highlighted with blue color and lock button changes to Unlock button, then the required quantity filled will enable to enter the data. Shown in the following screen: Page 15

16 Step-8: After filling the demand quantity of particular item, user has to lock the particular item by clicking on unlocked button,the corresponding item becomes highlighted with pink color and user cannot make modification when corresponding image is locked. Page 16

17 Step-9: After filling the demand for the required items, the user can either press Draft Save button for further modification in Annual Demand, in this case Annual Demand will not be sent to Family Planning Division for compilation. Step-10: User can press Annual Demand button for raising the Annual Demand for further compilation at Family Planning Division. Note: Users can update demand for the items until it is compiled by the FP division. The Annual Demand status can be Draft, Draft or Approval Pending, Compilation Pending, or Compiled Annual Demand Modification: Step-1: To modify the Annual Demand, repeat step-1 to step-4. Following screen will appear: There are three buttons in action column: M: Modify C: Cancel V: View Page 17

18 Step-2: Click on Modify button to modify the annual demand. Step-3: follow step-6 to Step-10. Step-4: After clicking on M button the user can modify demand quantity. After modification the user can click Draft Save button for further modification or click Raise Demand button for raising Annual Demand View annual Demand: Step-1: To modify the annual demand, repeat step-1 to step-4. Following screen will appear: Page 18

19 Step-2: Click on View V button to view the existing annual demand data on the system. Step-3: After clicking on V button, User can only view the requested quantity for all the items but cannot modify. Page 19

20 6.1.4 Cancel Annual Demand: Annual Demand can be cancelled till its status is Draft, Draft or Approval Pending, Compilation Pending. Step-1: To modify the annual demand, repeat step-1 to step-4 under index 3.1. Following screen will appear: Step-2: Click on cancel C button to cancel the existing annual demand data on the system. 21 Step-3: On clicking cancel C button a popup message will appear, to confirm the cancellation of Annual Demand. Click on OK button to confirm cancellation of Annual Demand and entering remarks for cancellation of Annual Demand. Step-4: Enter the valid remarks to cancel the demand and again press OK. Page 21

22 Step-5: Annual Demand Cancelled successfully message appears after successful cancellation Annual Demand Compilation The purpose of this process is to compile the Annual Demand. Annual demand is compiled by the FP Division user taking into consideration the total consumption of the various commodities from all the States and UTs. Step-1: Click on Forecast menu under main menu bar and select Annual Demand Compilation. Following screen will appear: Page 22

23 Step-2: User (FP division) clicks on Compile button to compile the demands then following screen will appear: Step-3: The list of items will display along with the Sanctioned Quantity. FP Division user can modify the Sanctioned Quantity by clicking on Lock button. Following screen will appear. Step-4: A popup will appear for particular item. FP Division user can edit the Sanctioned quantity and then click on OK button. Page 23

24 After updating or reviewing the demand for the selected item, that particular item(s) becomes highlighted with pink color. Step-5: Click on either Draft Save or Compile. If user wants to modify data further click on Draft Save. On click of the button following screen will appear: There are three buttons in Action column: M Modify C Cancel V View Page 24

25 6.1.6 Demand Modification at HQ: Modification can be done before compilation of demands. Step-1: To modify the demand in the Draft mode, click on M button, after doing the necessary modification click on Draft Save button. Step-2: After clicking on M button, the list of items will display along with sanctioned quantity. Following screen will appear: Step-3: Head Quarter (FPLMIS) can modify the Sanctioned Quantity by clicking on Lock button. Following pop-up will appear: Page 25

26 Step-4: Edit the quantity of item then Click on the OK button. After updating or reviewing the demand for the selected item, that particular item(s) become highlighted with pink color as shown in below figure. Page 26

27 6.2 View annual demand HQ: Step-1: Click on view V button on the following screen: After clicking on V button, User can only view the sanctioned quantity for all the items but cannot modify. Page 27

28 6.3 Demand cancelation HQ: Step-1: Click on cancel C button on the following screen: Stap-2: After clicking on the C button, a popup will appear You are going to cancel the Request, Are you sure. Page 28

29 Step-3: Click on the OK button to cancel the demand then following popup will arrive: Step-4: Enter the valid remarks to cancel the demand then click on the OK button. 7. Purchase Request: This process is used to distribute Annual demand of each item to the Purchasing Agency by the Head Quarter (FP-Division). 7.1 Purchase Request Process: Step-1: Login with Valid Credentials. 30 Step-2: Click on Purchase menu under main menu and select Purchase Request from the task list following screen will appear. Note: Purchasing Agency: It defines the list of Purchasing Agency. Two purchasing Agency: a. SSM b. CMSS Page 30

31 Step-3: Select Demand Period, Purchase Agency and click on GO, following screen will appear. Note: Purchasing Agency: It defines the list of Purchasing Agency. Item Name: It defines the list of all items whose demand the FP Division has compiled. Step-4: Select the Item from the list. Page 31

32 Step-5: Click on Draft Save or Generate Order. When user click on Draft Save. A popup will appear. Step-6: Click on the OK button. To View the Records Page 32

33 Step-1: To View record, Click on the Modify V button. User can modify the quantity to be purchase. Step-2: Click on Generate Order. A popup will appear. Page 33

34 Step-3: Click on OK button, It displays the message Purchase Request Saved Successfully. Step-4: To view the saved record, Click on the checkbox align at right top. Step-5: Click on GO button. Status shown as Finalized. To View the record, Click on the view checkbox right side Page 34

35 7.1.2 Cancel Purchase Request: Step-1: Click on the cancel C button. Step-2: Enter the valid remarks. Step-3: Click on OK button. 8 Purchase Order A purchase order (PO) is a commercial document issued by a buyer to a seller, indicating types, quantities, and agreed prices for the items to be procured. 8.1 Purchase Order Generation: This process is designed to generate a Purchase Order for the supplier. The purpose for generating Purchase Order is to provide necessary information to supplier related to the item to procure various consignees and the quantity. Pre-requisite: 1. Supplier must be registered with FP-LMIS application and all relevant information about the supplier is available in the system via Masters. 2. Annual demand should be finalized. Page 35

36 3. Rate Contract must be issued/generated to supplier with all details available to him such as Rate, Unit etc Purchase Order Generation Process: Step-1: Login with Valid Credentials. Step-2: Click on Stock menu under main Menu and click on click on Purchase Order Detail from the task list. Step-2: On clicking purchase order generation following screen will appear. Purchase Order Desk will display the store name as "Head Quarter" and PO STATUS as Pending". It will show the entire PO list, which are still pending for approval. Page 36

37 Note: PO status is categorized into four categories Delivery in Process: Purchase orders, which are send to suppliers, are shows under the Delivery in Process status. Closed: Once the supplier completes the Purchase Order, it will be reflected under Closed status. Cancelled: If user cancels the Purchase Order from their desk, they will be visible under Cancelled Status in Purchase Order Desk. Step-3: Click on Generate button to create the Purchase Order. Following screen will appear: Page 37

38 Step-4: Fill all the mandatory details such as Request No, Funding Source, Item Name and Supplier. Note: PO Generation Period: By default, the current financial year should display. Purchase Order Date: It displays the date on which the purchase order is generated by default the current system date should display. Funding Source: User can select the funding source. Item Name: It defines the list of items corresponding to Request No, whose Purchase request is raised. Supplier Name: Select the supplier for which Purchase order to be generate only the suppliers with the selected item will be display in the dropdown. Step-5: Click on the Go Button then following screen will appear: User can enter the details of rate Contract like Rate/Unit, Tax (%), Rate with Tax etc. in the purchase order form. User can schedule the distribution plan into four quarters, but the total quantity of four quarters cannot be greater than requested quantity. Page 38

39 Step-6: Click on the Generate button to create a Purchase Order then following pop-up will appear for confirmation. Step-7: A popup will appear then click on OK button Cancel the existing Purchase Order Step-1: Select the purchase order by clicking check box and Click on the Cancel button to Cancel the existing purchase order. 40 By entering the remarks, Click on save button to cancel the PO. Step-1: Select the purchase order by clicking the check box and Click on the Cancel button Page 40

41 9 Distribution Plan Step-1: Login with Valid Credentials. Step-2: Click on the Stock menu under main Menu and click on click on Purchase from the task list. Select Distribution Plan. Step -3 Select the check box, and click on the distribution plan button. Page 41

42 Step-4 Create the distribution plan for the Quarter1, Quarter2, Quarter3, Quarter4, for all the store names listed. After ensuring all entries are done, click on the save plan, the below screen appear. Page 42

43 Click OK to save the records. 10 Supplier Interface Desk The purpose of this process is to create an interface for the supplier through which the supplier can enter delivery details Supplier desk Process: Step-1: Login with valid credentials. Step-2: Click on Stock menu under main Menu and click on click on Supplier Desk from the task list. Step-3: Select the record to the corresponding PO No to which batch details have to be enter. Step-4: Click on Batch Details button and select the Item name. Page 43

44 Following screen will appear: Step-5: The user, click the Save button to save the details, fills once all mandatory details. Step-6: The systems will give a pop-up for confirmation. Page 44

45 Step-7: Click OK button. A batch Detail will be shown in Previous Batch Details tab. User can add multiple batches for a single PO No. Enter new Batch No, Manufacturing Date and the Expiry Date. Page 45

46 Step-8: Click on Save button. Multiple batch detail will be shown in Previous Batch Detail tab Delivery Detail Step-1: Select the record for the corresponding PO No. to enter delivery detail. Page 46

47 Step-2: Click on Delivery button. Stap-3: Fill all the mandatory fields and Select the item from item Name Combo. Item Detail(s) tab will be shown. Page 47

48 Stap-4: Enter the No. of Batch and click on GO button. Select the batch No. from dropdown and enter the quantity to be deliver. User can deliver the quantity from different batches. Step-5: Click on Add button and made the entries. Step-6: Click on Save button. A popup will appear. Page 48

49 Step-7: Click on Ok button. Status shown as Receive Pending. In the same way, we can enter delivery details for different Schedule(s) View Detail Step-1: Select the record for the corresponding PO No. to be viewed. Step-2: Click on View button. Page 49

50 Step-3: Select on the radio button. Step-4: Click on the view V button to view the record for the corresponding schedule. Page 50

51 11 Challan The purpose of this process is to create/generate the Challan after receiving the consignment at the warehouse. Prerequisites: 1. Consignees name must be mapped with the application using masters. 2. PO No. should to be available. 3. Items should be available for the generation of Challan Challan Process: Step-1: Login with Valid Credentials. Step-2: Click on Stock menu under main Menu and click on click on Challan Process from the task list. Page 51

52 Note: Challan Status should be: Receive Pending: when the Supplier does not receive challan. Closed: When challan is received by the Supplier Step-3: Store name of Logged in users is auto selected, user have to select store name in case multiple stores were assigned to user. The Purchase Order details corresponding to selected store are populated. Page 52

53 Step-4: Select the check box corresponding to PO No. for which challan has to be received and click on Receive button. Following screen is displayed : Page 53

54 Step 5: Enter the Accepted quantity, Damaged, Sample Drawn quantity. Step 6: Click on the Verify button. Following popup message will appear. Clicking the OK button the following screen would appear and stocks would be updated automatically. Page 54

55 11.2 View Challan: Repeat steps 1, 2 and 3 of section 7.1. Step-4:Select the check box corresponding to PO No. for which challan has to be received and click on View button. Step 5: Following screen is displayed. Click on Radio button corresponding to challan number to view its details. Step 6: Following screen is displayed. To print click the Print button. Page 55

56 12 Inventory Management Item inventory process provides information regarding the stocks available in the system warehouse wise, batch wise and rack wise. The stock status of the available stock can be Active and Inactive status Inventory Management Process: Step-1: Login with Valid Credentials. Step-2: Click on Stock menu under main Menu and click on click on Ground Stock entry from the task list. Page 56

57 Following page would appear: Step-3: Click on the Add button to enter the stock. Step-4: Select the item from Item drop down list and number of batches. Page 57

58 Step-5: Enter the all mandatory details for the item then click on Save button. 13 Indent Generation The purpose of this process is to create/generate an Indent. The Warehouse user needs to generate indent Indent Generation Process Step-1: Login with Valid Credentials. Step-2: Click on Indent menu under main Manu and click on Indent Raise from the task list. Following screen would appear: Page 58

59 Indent Status can be: Approval Pending: When indent is not approved or rejected by approving authority. Issue Pending: When items are not issued by the issuing store. Ack-Pending: When items are not received by the indenting store. Completed: When items are received by the indenting store. Rejected: When indent is rejected by Approving authority. Step-3: Click on Generate button, following screen will appears. Select all the mandatory fields Page 59

60 Store Name: It defines the warehouse, who is indenting commodities. Issuing Store: It defines the warehouse(s), which will issue the commodities. Step-4: Then click on the Go button. System will display the list of all the items mapped with Family Planning Program. Unit: It defines the unit of the commodity. Stock in Hand: It defines the current stock of indenting store. Annual Requirement: It defined the annual requirement of indenting store. Total Stock Received as on date: It defines the total stock received as on date for each item during selected Financial Year. Average Monthly Consumption: It defines the monthly consumption of indenting store. Stock in Month: Stock in Hand / Average Monthly Consumption Availability (Issuing Store): It defines the availability of stock in issuing store. Yes defines that sock is available and No defines stock is not available in issuing store. Indent Qty.: In this indenting store, enter the quantity. Page 60

61 We can get the list of commoditiesindented by click on the button align right to the search items. Step-5: We can edit the Requested qty for the commodityto indent and then click on the OK button. Page 61

62 Step-6: Enter the indent quantity. Step-7: Click on Draft Save for further modification, if not click on Indent button to raise the indent. On clicking indent button following confirmation button will appear. System generates an alert You are going to raise indent. Are you sure!!!. Step-8: Click on OK button. It displays message Indent raised successfully MODIFY INDENT Step-1: Select the record having Indent status as Draft and click on Modify button. Step-2: Select the checkbox corresponding to the indent no. to modify. Page 62

63 Step-3: Click on the Modify button. We can modify the Indent Quantity and click on Draft Save or Indent. After Indent, there can be zero or more than zero level of Approval CANCEL INDENT User can cancel the raised indent by clicking on the Cancel button. The Page 63

64 indent can only be cancelled until it is not issued. Step-1: Select the checkbox corresponding to the indent no. to cancel the indent. Step-2: Click on the Cancel button. Step-3: An alert will appear to enter valid remarks. Enter valid remarks and the click on Ok button VIEW INDENT User can view the raised indent by clicking on View button. Step-1: Select the checkbox corresponding to the indent no. to cancel the indent. Step-2: Click on the view button to view the indent, following screen will appear: Page 64

65 PRINT INDENT User can print the raised indent by clicking on Print button. Step-1: Select the checkbox corresponding to the indent no. to cancel the indent. Step-2: Click on the Print button to print the indent, following print will appear in PDF format: Page 65

66 14 Approval Desk The purpose of this process is to approve the request Approval Process: Step-1: Login with Valid Credentials. Step-2: Click on Indent menu under main Menu and click on Approval Desk from the task list. Following screen will appear: Page 66

67 Note: Status can be: To be approved: If new request is arrived. Approval in Process: If there is more than one level of approving authority. Approved: If approving authority approved the request. Rejected: If approving authority reject the request. Step-3: Select the indent by the check on check box. Page 67

68 Step-4: Click on Approval button then following screen will appear. Approving authority can modify the approved quantity and can also reject or accept the request. Step-5: Approving authority accept the request then alert will appear, and then click on OK button to approve the indent. Page 68

69 Step-6: If Approving authority wants to approve the rejected request then, select status Rejected and then click on Re-approval button. Approving authority may view the request in any status then, select status and then click on View button to view the indent. 15 Issue against Indent This process to issue items against the indent/request. Warehouse officer cannot issue more than the indented or requested quantity. Page 69

70 Prerequisites: 1. Items should be mapped with Family Planning Program. 2. Indent should be generated for the items to be issued Issue against indent process: Step-1: Login with Valid Credentials. Step-2: Click on Issue menu under main Menu and click on Issue against Indent from the task list. Page 70

71 Following screen will appear: Step-3: Select the Indent No. by check the check box and click on Issue button, following screen will appears. Note: Status can be: Issue Pending: When items where not issued by issuing store. Ack- Pending: When items where received by indenting store. Processed: When items where received by the indenting store. Un-Approved Demand: When the Approving authority does not approve indenting request. Page 71
