Currently supported browsers are: Safari Android Web Browser The elogs App does not currently run on Internet Explorer or Mozilla Firefox, as these browsers do not support the local database storage component of HTML5. Do not use the elogs App on Public Computers. All residents wishing to use a public computer must use the elogs Direct. Residents should use the elogs Direct on all Public Computers. 2 American College of Osteopathic Obstetricians andgynecologists OBGYN elogs App Quick Reference Guide READ ME FIRST (Back to Links) Introduction: elogs is a valuable software tool that allows Residents and their corresponding institutions to meet mandated and AOA log requirements in a safe, HIPAA compliant, legible and efficient manner. elogs are a required part of your residency and should be a part of your daily routine. elogs printed reports are due to your DME no later than 14 days past the last day of your Rotation 1. General Procedure, three main types of logs. a. Contract year: This log is completed at the beginning of your contract year. This reflects the resident s contract dates and not the academic year. b. Master Schedule This log is completed at the beginning of each rotation. This log reflects the schedule dates and service assignment. c. Case Logs. These logs are completed daily. First year residents/interns are required to log all patient encounters on non-obgyn rotations and just procedures on OBGYN rotations; senior residents are required to log just procedures. 2. All elogs must be synced to the master database. First year residents must submit a signed copy of their logs to Medical Education no later than 14 days after the end of their rotation. 3. First year residents: a. OBGYN rotations: Log ALL procedures b. NON-OBGYN rotations: (off rotations such as IM, FM, Surgery etc.) Log ALL patient encounters and ALL procedures c. ALL rotations: Log reading and Education d. All elogs must be synced to the master database. First year residents must submit a signed copy of their logs to Medical Education no later than 14 days after the end of their rotation. 4. NON-First year Residents a. Log ALL procedures b. All elogs must be synced to the master database. Residents must submit a signed copy of their logs to Medical Education and/or Program Directors no later than 14 days after the end of their rotation. 5. All residents are required to generate an Annual report using the ACOOG elogs system. Annual reports must be saved by the resident and locked by the program director no later than 30 days past the end of the residents OGME year. You can also run the handheld component of the SCS elogs on a PC, notebook or tablet device with a supported web browser. Currently supported browsers are: Safari Android Web Browser The elogs App does not currently run on Internet Explorer or Mozilla Firefox, as these browsers do not support the local database storage component of HTML5. An alternative elogs client elogs Direct will be compatible with more browsers and allow secure logging in a public setting. elogs Direct will be available Fall, WARNING: Residents should use the elogs Direct on all Public Computers. 5 Configuring elogs on a Handheld Device i (back to Links) Open the HTML5 compliant web browser (Safari, Google Chrome). For ipod touch, iphone and ipad devices, tap/click the Safari icon to open the web browser. On Android devices, tap/click the Internet icon to open the web browser on the device. 1. In the handheld web browser, enter the elogs URL: Click/tap/ on the link Install or Run elogs You will be taken to a web page for elogs. scselogs.com 2. On the starting page for the elogs Offline Logging app, tap/click INSTALL/RUN. A synchronization screen will be displayed. Tap/click the Setup button and enter your elogs User Name and Password. Then tap/click OK. 3. After entering the handheld User Name and Password, you will be returned to the Pendragon Forms Synchronization screen. You should now bookmark this web page so that you can easily return to it in the future. On ipod touch and iphone devices, tap/click the Plus (+) icon at the bottom of the screen and choose to add an icon to the Home screen. On ipad devices, the Plus (+) icon is at the top of the screen. 6 On ipad devices, tap/click on the add bookmark button (next to the URL address) Tap/click on Add Bookmark to add a book mark to the Browser. Edit the bookmark title and tap/click on Save Tap/Click on Add to Home Screen to place an icon on the ipad home screen. Edit the Icon title tap/click on Add. (back to Links) Add Bookmark Rename bookmark or icon here On Android devices, tap/click the Menu button, and choose to Add Bookmark. Forms VI will be selected as the bookmark name and the address (URL) of the Form VI Synchronization page will be pre-selected. Choose Done. Tap/click the Home button to return to the Home screen. Tap/click and hold a blank area on the Home screen, and the Add to Home screen will be displayed. Choose to add a Shortcut. Select Bookmarks and on the Bookmarks screen select Forms VI. Type elogs or Forms VI into the window and a Forms VI icon will be added to your Home screen. 4. On the Home screen of the handheld device, tap/click the Forms VI icon. The Pendragon Forms VI Synchronization screen will be displayed. Tap/click the Sync button to synchronize the device to receive any new form designs. After a successful synchronization, tap/click the Launch button to view the form designs. Tap/click on a form to fill out the form. (back to Links) Page 6 of 13

7 Entering New Records into the elogs (back to Links) Before you can use the elogs app, the program must first be installed on the handheld via the Web browser of the handheld device, and an icon should be created on the Home screen of the device to allow the handheld user to access the program even when offline. See step 1 of the instructions above to load the software to your handheld device. Each instance of filling in a form creates a record. General Procedure, three main types of logs. a. To create a new record on the handheld: 1. On the Pendragon Forms VI Synchronization screen, tap/click the Launch button. 2. Tap/click the Sync button 3. On the Forms Main Menu showing the list of available forms, tap/click the name of the form that you want to fill in. 4. Tap/click the New Record button. 8 5. Tap/click in each field in turn, to enter data in that field. On handheld devices with a popup keyboard, the keyboard will automatically pop up when you are in a Text or Numeric field. On handheld devices with an external keypad or online, the cursor will populate the field Text or Numeric field. Tap/click on a drop down list or Multiselection look up list and the list will open up and fill the screen. Longer lists will have a scroll bar to the right of the entries. Tap/click on a date field and a date selection box will fill the screen. This date is always set to current so make sure to choose the correct month and date when back logging. Tap/click on the correct month, then tap/click on the internal keypad to enter in the correct day and year. Back Sync End Previous Next 6. To move forward or backward one screen at a time, tap/click the Next or Previous buttons. Note: Do not use the Back button of the web browser when in Pendragon Forms VI. Tap/click the End button to exit the record. When you exit a record, you will be returned to the Review screen for the form. Tap/click the New Record button to create another new record, or tap/click the Back button to return to the Forms Main Menu. Note: At the end of each session tap/click the sync button to ensure that your data is synced to the master database. Page 8 of 13

9 Reviewing Records in the elogs (back to Links) An existing record is a record that you have already filled in, or a record that has been sent from the server to your handheld. To modify an existing record, you need to review the record on the handheld. Records remain on the handheld device for days depending on the log type. To review and or edit records on the handheld: 1. On the elogs App Synchronization screen, tap/click the Launch button. 2. On the Forms Main Menu showing the list of available forms, tap/click the name of the form that you want to review. 3. A list of existing records for that form will be displayed. Tap/click the record you would like to review. Scroll through the record using the got to jump boxes, or the Next, Previous and End buttons. Edit fields as necessary. Note: On the Review screen, the default is that three fields from each record are displayed; Date Seen, Medical Record Number and the Attending. Go To Dropdown End Next Previous Page 9 of 13

11 elogs OBGYN RES Forms and Descriptions (back to Links) OBGYN Gynecologic Case Log This log is used to record non-obstetric hospital & clinic procedures. OBGYN RES Gynecologic is organized by anatomical regions. The log features point and click drop down menus that include surgical and non-surgical procedures performed in the hospital or clinic. These include gynecologic, urogynecologic and gynecologic/oncologic procedures, as well as some general surgery. This log may be used for, but is not limited to diagnostic procedures (e.g. diagnostic hysteroscopy,) general surgery procedures (e.g. Appendectomies,) urogynecologic procedures (e.g. urethrovesical suspension), OMM procedures, Ultrasounds and gynecologic procedures (Oophorectomies.) OBGYN Obstetrics Case Log This log features point and click drop down menus that are used to record Obstetrical procedures performed in the hospital and clinic including but not limited to obstetric diagnostic procedures such as amniocentesis and OB ultrasounds, vaginal and cesarean births. OBGYN Other Procedures Case Log This log is used to record non-obgyn procedures often performed by the Resident that do not fall into any other activity log category e.g. circumcisions. elogs OBGYN RES First Year Additional Forms and Descriptions Case Logs DO This log is used to record all non-women s health Hospital and Ambulatory Clinic/Office visits. The log is used to record pediatric and men s health patient encounters such as but not limited to ambulatory care, H&Ps, House officer duty, and ER call. The log features point and click drop down menus that include the 50 most commonly seen diagnoses and 80 most commonly performed procedures done in an ambulatory or hospital setting by primary care practitioners. Educational Programs: Used by first year residents to log all lectures, talks or demonstrations attended or given, such as but not limited to noon lectures, weekly departmental lectures and SCS education days. Reading Program: Used by first year residents to log assigned and optional reading done during the rotation such as, but not limited to, journal club activities, textbook reading assignments and case research. Surgery: Used by first year residents to record all non-women s health operating room and bedside surgical Page 11 of 13
