1 American College of Osteopathic Obstetricians and Gynecologists OBGYN elogs Direct Quick Reference Guide READ ME FIRST System Requirements: The SCS elogs Online Logging and Reports Application can run on any almost any Web browser and on any computer or handheld device with Internet access. Data is stored at the MSU server and not on the computer or the handheld. Introduction: elogs is a valuable software tool that allows Residents and their corresponding institutions to meet mandated and AOA log requirements in a safe, HIPAA compliant, legible and efficient manner. elogs are a required part of your residency and should be a part of your daily routine. elogs printed monthly reports are due to your DME no later than 7 days past the last day of your Rotation. The program director may require a printed copy too. Check with Med ED Coordinator. The Annual Report must be saved by the resident no later than 7 days past the last day of the Resident s Contract year. Logging Requirements 1. General Procedure, three main types of logs. a. Contract year: This log is completed at the beginning of your contract year. This reflects the resident s contract dates and not the academic year. b. Master Schedule This log is completed at the beginning of each rotation. This log reflects the schedule dates and service assignment. c. Case Logs. These logs are completed daily. First year residents/interns are required to log all patient encounters on non OBGYN rotations and just procedures on OBGYN rotations; senior residents are required to log just procedures. 2. All elogs must be synced to the master database. First year residents must submit a signed copy of their logs to Medical Education no later than 7 days after the end of their rotation. 3. First year residents: a. OBGYN rotations: Log ALL procedures b. NON OBGYN rotations: (off rotations such as IM, FM, Surgery etc.) Log ALL patient encounters and ALL procedures c. ALL rotations: Log reading and Education d. All elogs must be synced to the master database. First year residents must submit a signed copy of their logs to Medical Education no later than 14 days after the end of their rotation. 4. NON First year Residents a. Log ALL procedures b. All elogs must be synced to the master database. Residents must submit a signed copy of their logs to Medical Education and/or Program Directors no later than 14 days after the end of their rotation. 5. All residents are required to generate an Annual report using the ACOOG elogs system. Annual reports must be saved by the resident and locked by the program director no later than 7 days past the end of the residents contract year. Page 1 of 13
2 Entering logs into elogs Direct Open web browser and go to elogs homepage tab bar including: Main Page: Program description and updates Login: elogs login info for Online and Offline access Program Info: elogs purpose and description of forms and uses Support: Documentation, Training videos, elogs mailing list, and FAQs Contact Us: elogs helpdesk staff contact information Program description Website Search box Login to Reports and Online Logging application Retrieve ID and Password Offline access to be used on Page 2 of 13
3 Entering Logs Open web browser and go to Using your SCS supplied ID and password log into the elogs Direct Online Logging system click on submit; click on Enter Logs elogs Direct a list of forms will appear. Step 1: Contract Year At the beginning of their contract year, generally on July 1 st, all residents are required to fill out a Contract Year log form for each OGME/Training year. Depending on the Resident, at the end of his or her program, they may have as few as one contract year or as many as eight contract year forms entered into the system. The contract year form must be filled out or the elogs reports will not print properly. Only the forms that are required by the residency will appear on this screen. Please note that in this example all the forms are displayed System logout, Switch to elogs Reports From this screen the user may logout of the system and switch to the elogs Reports module. 1. Click on contract year, click on the New Record button. All the fields in Contract Year are required 2. Click in the Base Hospital/Institution box, a drop down list will appear, either type in the name of the base hospital or scroll down to choose the name. This click or type feature is part of all the new elogs Direct Online forms. Page 3 of 13
4 3. Click in the Contract Year Start Date box, a calendar will appear, make sure to choose the correct month, day and Year. This calendar feature is part of all the new elogs forms 4. Click in the Contract Year End Date box, a calendar will appear, make sure to choose the correct month, day and Year. Special Note: In most instances the resident s contract year runs with the academic year, July 1 June 30. Many residents are off cycle. These dates must match the resident s actual contract. If the resident is unsure of his or her contract dates check with medical Education for confirmation. 5. OGME Year: Click on the drop down arrow and choose the correct OGME year. Note that the OGME year and the Resident Training Year generally match, but not always. For example, it is not uncommon for a resident to be an OGME 2 and a TR Training Year: Click on the drop down arrow and choose the correct Resident Training year. 7. Check that all of the information entered is correct and click on the Save button 8. Click on the Back button 9. Click on the Return To Forms List button Step 2: Master Schedule At the beginning of each rotation all residents are required to fill out a Master Schedule log form. Rotation numbering starts with July =1 and sequential after that. Every July rotation numbering restarts with 1. Rotations may be monthly or block periods of 1 4 weeks. The master schedule reflects the exact schedule given to the resident at the beginning of the academic year. All residents log all cases under their current assignment/service regardless of call or case activity. For example if an OB resident is assigned to Internal Medicine rotation/service and that resident does an OB weekend call; all those OB/GYN cases are logged under the Internal Medicine service. 1. 5 2. Using your SCS supplied ID and password log into the elogs Direct Online Logging system click on submit; click on Enter Logs elogs Direct a list of forms will appear. 3. Click on Master Schedule. 4. Click on the New Record button. 5. Fill in the form paying close attention that the information matches the Resident s exact schedule including location, dates of service and type of service. 6. Click on the Save button 7. After the record is saved the system automatically brings up another blank form. At this point the resident may add an additional rotation by repeating steps To get back to the main menu a. Click on the Back button b. Click on the Return To Forms List button Step 3: Case Logs First year residents/interns are required to log patient encounters. After the first year residents must check with their program directors to determine logging requirements. See page 1 for clarification. Otherwise residents are required to log all their procedures regardless of type of rotation. Note: Each resident will only see the log forms pertenant to their residency. The case to be logged, not the rotation or service, determines which elogs form to be used. See appendix A for examples of cases and forms used. 1. Click on the log form Page 5 of 13

In elogs Direct the resident may use all standard website navigation tools such as the tab key, scroll up and down, find, and page up and down, to maneuver through the log form, generally the first 5 7 fields in every form are required. Enter in the basic patient statistical information 4. Scroll down and enter in DX and procedures 5. Click on the save button 6. After the record is saved the system automatically brings up another blank form. At this point the resident may add an additional rotation by repeating steps To get back to the main menu a. Click on the Back button b. Click on the Return to Forms List Button Editing and Deleting Records Open web browser and go to Using your SCS supplied ID and password log into the elogs Direct Online Logging system click on submit; click on Enter Logs elogs Direct a list of forms will appear a list of forms, tailored to the resident s account will appear. Editing Records 1. Click on the log form 2. Search for the record by a. Scrolling up and down or click on the <<Prev or >> Next buttons to move through the list or b. Enter a search term, such as a medical record number or a date seen in the search box click on the search button Page 6 of 13
7 3. Click on the record to be edited 4. Edit the record as neccessary 5. Click on the Save button 6. To edit more records repeat steps To go back to the main screen click on the Return to Forms List button Deleting Records IMPORTANT NOTE: when using the elogs Direct the resident is using a live editor in the MSU master database. In all cases when deleting records in the elogs Direct, if the resident is also using the offline elogs App, and if the record appears on the elogs App list, the unwanted record must be deleted from the offline App device. See Deleting Records in the elogs App instructions on the website under the Support tab Support Documents for further instructions. Deleting Records in elogs Direct 1. Click on the log form 2. Search for the record by a. Scrolling up and down or click on the <<Prev or >> Next buttons to move through the list or b. Enter a search term, such as a medical record number or a date seen in the search box click on the Search button 3. Click on the record to be deleted 4. Scroll through the record and make sure this is the record to be deleted 5. Click on the Delete This Record button 6. Click on the Confirm Deletion button 7. Click on the Return to Forms List button Page 7 of 13
8 Special features Missing data: All required fields must be filled in before the record is saved. If the record is saved and these fields are blank the system highlights the required fields in red. The resident must fill in these fields and resave the record. Text box features: Click in a text box and start typing or use the drop A down arrow and scroll to find the correct response. Click on the response to enter it into the field. A B Click on the + to add a field for additional case information B Multiselection Lookup Lists Residents may click and un click as many selections as applicable for the case. These selections are toggle buttons click on the button to darken it and make a selection. Click on the darken button to deselect. Page 8 of 13
9 elogs Record Inspection Utility This feature allows the user to view the records that were added to the database. Log into the elogs reports and click on the Enter Logs tab Record Inspection Utility The resident may view how many cases were added and the date they were updated. Residents may click on a form to view the individual records. Click on the individual record to view a log. Page 9 of 13
10 elogs Support Go to and click on Support FAQs for fast answers on many common issues and procedures. Anyone who forgets their ID or password may retrieve it automatically by clicking on the Forgot My Password or Forgot my ID link on the elogs home page. elogs support staff information may be found on the elogs home page under the Contact Us tab elogs OBGYN RES Forms and Descriptions (back to Links) OBGYN Gynecologic Case Log The log features point and click drop down menus that include surgical and non-surgical procedures performed in the hospital or clinic. These include gynecologic, gynecologic/oncologic and urogynecologic procedures, as well as some general surgery. This log may be used for, but is not limited to diagnostic procedures (e.g. diagnostic hysteroscopy,) gynecologic/oncologic procedures (e.g. Intraperitoneal Catheter Placement,) general surgery procedures (e.g. Appendectomies,) urogynecologic procedures (e.g. urethrovesical suspension), OMM procedures, Ultrasounds and gynecologic procedures (Oophorectomies.) OBGYN Obstetrics Case Log This log features point and click drop down menus that are used to record Obstetrical procedures performed in the hospital and clinic including but not limited to obstetric diagnostic procedures such as amniocentesis and OB ultrasounds, vaginal and cesarean births. OBGYN Other Procedures Case Log This log features point and click drop down menus that are used record non-obgyn procedures often performed by the Resident that do not fall into any other activity log category e.g. circumcisions. OBGYN MFM Procedures: This log features point and click drop down menus that are used record procedures performed by MFM Fellows and Residents doing an MFM rotation. These procedures include Specialized Amniocentesis (e.g. Fetal Lung Maturity Amniocentesis,) Specialized Doppler (e.g. Doppler, Ductus Venosus,) and Specialized Ultrasound (e.g. Ultrasound, Fetal Echocardiograms.) 11 elogs OBGYN RES First Year Additional Forms and Descriptions Case Logs DO This log is used to record all non-women s health Hospital and Ambulatory Clinic/Office visits. The log is used to record pediatric and men s health patient encounters such as but not limited to ambulatory care, H&Ps, House officer duty, and ER call. The log features point and click drop down menus that include the 50 most commonly seen diagnoses and 80 most commonly performed procedures done in an ambulatory or hospital setting by primary care practitioners. Educational Programs: Used by first year residents to log all lectures, talks or demonstrations attended or given, such as but not limited to noon lectures, weekly departmental lectures and SCS education days. Reading Program: Used by first year residents to log assigned and optional reading done during the rotation such as, but not limited to, journal club activities, textbook reading assignments and case research. Surgery: Used by first year residents to record all non-women s health operating room and bedside surgical Page 11 of 13
