4 Handout D Web Forms Directions: Find three different websites that contain web forms. Input the information in a word processing document using a handheld computer. Save as Handout D Remember, web forms can include any type of form on which the visitor has to input information. 5 Activity D Create Web Forms Directions: 1. Using a word processing program, create a web form survey with checkboxes, option buttons, dropdown menus, and/or textboxes. 2. Include the following information in your survey: Gender Age Grade Favorite Color Favorite Food 3. Create a submit button. 4. Save the web form as Activity D Be sure to save the survey as a web page. 5. Using your school s browser, open survey and view it as a web form. 6. Make sure the web form is set up and works correctly. Example: Gender: Male Female Age: 15 Grade: Favorite Color: Blue Favorite Food: Submit D4-5
6 Integrate MS Word and the Web Activity D Directions: Create a website for a new business using MS Word with at least three linked pages. 1. Page 1 (homepage) should include the following: Company Name Logo Address Phone and Fax Number address A link to the next two pages 2. Page 2 should include the following: A list of goods/services that your business provides Pictures and descriptive information about your goods/services A short survey inquiring about age and gender A link to the next page A link to the homepage An outside link. Note: Sites such as FedEx, UPS, etc. provide useful information about related product information. 3. Page 3 should include the following: A brief history of your business.. When did you open?. Who owns the business?. What week days are you open?. What are the hours of operation? Any other pertinent or interesting information. A link to the previous page. A link to the home page. 4. Be sure to incorporate text colors, backgrounds, graphics, and hyperlinks. 5. Be sure to proofread. Check your spelling and grammar. Do not use slang. Be professional and creative. D4-6
