1 Exercise 2: Early Intervention Step 1: Navigate to the Child Search page using the left menu. Once on the search page type the ETID you created in Central Coordination into the Early Track ID box. Alternatively, you can locate your record by searching with the child name, caregiver name, or child DOB. Once you have located the correct record, choose the (edit) icon to open the child record. Step 2: Fill in the missing demographic information that will be needed for children in EI. Then select Save at the bottom of the page. Page 1 of 13

2 Step 3: Navigate to the Caregivers section via the left menu. Fill in the missing caregiver information that will be needed for children in EI. Then select Save at the bottom of the page. Step 4: Assign a Service Coordinator Agency and Service Coordinator. Expand the Early Intervention dropdown on the left menu and select Service Coordinators. Click New to create the new assignment for your record. Page 2 of 13

3 Enter the Date Assigned with today s date. Choose A Wonder World for the Service Agency Name, and Test, Test as the Service Coordinator. Then click Save to add the assignment. Step 5: Navigate to the Screenings section of the left menu. Select New to add a Vision Screening. Page 3 of 13

4 Input the Vision Screening information. Click Save to save the screening. Step 6: Navigate to the Eligibility section on the left menu. From the dropdown, select Evaluation to add a developmental evaluation for the child. Page 4 of 13

5 Input the information below to make the child eligible by Informed Clinical Opinion (ICO). Click Save when you are finished. Page 5 of 13

6 Step 7: Now that the child is eligible, you will proceed with adding a Child Assessment and Family Assessment. Both are found under the Assessments tab of the left menu. Navigate to this tab and then select New. Fill in the following information to add a Child Assessment with today s date then click Save. Page 6 of 13

7 Now select New to proceed with adding a Family Assessment with today s date. Click Save when you are finished. Step 8: Navigate to the Need for Services tab to proceed with adding a Need for Services for the child. Select New to begin. Page 7 of 13

8 Enter today s date and the determination that the child does have a need for EI services. Page 8 of 13

9 After clicking Save, you will be presented with a summary page. Verify that current criteria indicates that the child has a need for services. Then click Continue. Note: If you missed any of the earlier steps current criteria would indicate that the child does not have a need for services. Step 9: Navigate to the IFSP screen and then select New. Page 9 of 13

10 Fill out the IFSP according to the screenshot below: Page 10 of 13

11 Step 10: Remain on the IFSP page and click on the Service Dates tab to add a service start date as of today. Step 11: Navigate to the Transition tab on the left menu to add a Transition Planning Conference. Input the following information and click Save to add the TPC. Page 11 of 13

12 Step 12: Add a Case Note for this record. Navigate to the Case Notes tab on the left menu and click New. Create a Case Note with the Type ALL and the subject Completed. In the narrative provide your name, county, and date you completed the training. Step 13: Finally, you will add the Exit information to close the record. Navigate to the Exit Child tab and then select New to create the Early Intervention Exit. Page 12 of 13

13 Add the exit information below and then click Save. Congratulations! You have completed the Early Intervention Early Track Self-Guided Training. You can check the History page of your record by using the tab on the left to verify that all items are included in the record. Step 14: To receive a certificate of completion (required only if seeking System Administrator access), please the ETID of the record you created to EIDS@dodd.ohio.gov. State staff will verify completion and issue your certificate. EI System Administrator accounts are created once the training has been completed and a signed user agreement has been sent to DODD.
