3 1 Introduction LibreView will provide people with diabetes and their health care professionals with a common portal for sharing, monitoring and analyzing historical blood glucose readings. Patients and health care professionals can upload, monitor and share historical readings from supported blood glucose monitoring devices. Patients that upload their data will be able to easily share the information with their health care professionals for monitoring, analysis and review. Patients can also have their health care professional upload their data on their behalf. Patients and health care professionals will have access to a set of detailed reports with many graphical views of blood glucose levels, patterns, and trends. 1.1 Scope The purpose of this document is to provide information on how to use the LibreView software product. The document focuses on an introductory level of information to get the user started. 1.2 Audience The intended readers of this document are end users of the LibreView application. 1.3 Goals and Objectives The objective of this document is to gain an adequate understanding of the software product and be able to utilize the platform. By reading this document users should be able to understand how to create a LibreView account, manage user data, and upload readings. 2 System Requirements Minimum Computer Requirements Windows Vista with IE10 running on a 550MHz Pentium 3, 512MB DRAM, 2G Hard Drive, USB 2.0, LCD Screen with resolution of 1024x768. Browser Requirements: LibreView will be supported in the following browsers on Windows Vista or higher computers: o o o Microsoft Internet Explorer 10 or higher Google Chrome release 37.0 or higher Mozilla Firefox release 32.0 or higher NOTE: LibreView web application is supported on Safari Browser on Mac OS X Mountain Lion or higher computers and Safari Browser on ios 6 or higher mobile devices. LibreView Device Drivers upload utility is supported on Windows computers only.

5 4 About LibreView LibreView Data Management Software version [1.0] LibreView is developed and distributed by Numera, Inc. headquartered at: rd Ave Suite 700 Seattle, WA The LibreView Data Management Software is intended for use by both patients and healthcare professionals to assist people with diabetes and their healthcare professionals in the review, analysis and evaluation of historical glucose meter data to support effective diabetes management. The LibreView Software is not intended to provide treatment decisions or to be used as a substitute for professional healthcare advice. 5 CAUTION: Do not use LibreView to upload data from any device that has stored data for more than one person. LibreView cannot identify if data on a device has ever been recorded for more than one person.

6 LibreView HCP Quick Start Guide REGISTRATION (1) Welcome to LibreView. By utilizing this application you will be able to create clear, easy- to- understand reports from your patients glucose monitors. 1.1 Your first step will be to create a LibreView account. You may do this by going to Please select Healthcare Professionals under the title Start Here at the bottom of the webpage. (If you have already registered please log in with your address and password used during registration.) (1.1) Enter your address, and click Sign Up Now (1.2) 1.2 You will now be presented with the professional account registration form. Please fill out all required fields. Once finished select Next. (1.3) Your next step in registration will be to enter your practice name, address, phone number and add an optional logo. Once finished select Next. (1.4) Your last step in registration will be to accept the End User License Agreement and Terms of Use. If you agree with the license agreement and terms, select "Agree" and proceed in your use of the software Numera Proprietary

7 TOUR (2) You have now successfully registered your LibreView account and will be prompted for a quick tour to get started. Stepping through the tour is recommended the first time you use the application. (2.1) 2.1 When you start the tour you will first be shown your my patients dashboard. This is where you go to see a dashboard of all your patients saved glucose history, including the most recent uploads. (2.2) 2.2 Settings are located in the menu in the top right corner of the application. This is where you can access your account settings and report preferences. It is also where you go to set up and manage practice- level features. (2.3) 2.3 The Learn more button at the bottom of this screen, has links to some important support articles. More support articles can be found at pro.libreview.com/support. (2.4) 2.4 Once you have finished the tour, you can invite a patient directly from the dashboard by selecting Invite Patient, and filling out the invitation form. (2.5) 2.5

8 DOWNLOAD & INSTALL LibreView Device Drivers APPLICATION (3) In order to upload a meter, you will need to install the LibreView uploader ("LDD"). You can download LibreView Device Drivers by selecting the meter upload icon. (3.1) 3.1 The LibreView Device Drivers application will be used every time you want to upload data from your glucose meter. You will now be presented with a that will prompt you to download the LibreView Device Drivers. Please select download. (3.2) Once the LibreView Device Drivers has been downloaded you will need to install it. To install, double click on the downloaded file and you will be presented with the install wizard. (3.3) Simply follow the steps and select next throughout the wizard, and select Finish at the end of installation. (3.4) 3.4

9 LibreView HCP Quick Start Guide UPLOAD DATA FROM METER (4) Once you have successfully installed the LibreView Device Driversplease click the upload icon. LibreView. Since LibreView Device Drivers is installed you will now be prompted to connect your patient s meter. (4.1) 4.1 First, connect your device to the correct cable for the device you would like to upload, and then connect your cable to the computer. Upload will begin when the other end of that cable is connected to the computer. (4.2) 4.2 If your device is not assigned to a patient you will be prompted to generate an anonymous report or link the meter to a patient. (4.3) 4.3 If you click Link meter to a patient, you will be presented with a screen where you can search for the patient. (4.4.) 4.4 Once you have found the appropriate patient, select the name and you will be presented with a confirmation. If you cannot find the patient, select Back. You can generate an anonymous report, and then invite the patient from the my patients dashboard screen. (4.5) Numera Proprietary

10 VIEW REPORTS (5) After a successful upload, you will be directed to the my patients dashboard. From here you double click the patient to view more details about that patient. (5.1) 5.1 You will now be presented with that patient s dashboard. (5.2) The default view is of your patient s most recent 2- week period of data. This data is compared against the previous 2- week data period. You can change the timeframe view by selecting the dropdown filter at the top of the dashboard To view your patient s full set of LibreView reports select View Full Reports. It will take a few seconds (depending upon your internet connection) to generate your patient s full set of reports. Once they have finished generating you will see the reports on the screen. (5.3) If you would like to change the settings for your patient s reports select Reports at the bottom of the screen. You will be presented with your settings menu where you can select (5.4): time frame target ranges and thresholds meter preferences (e.g. selecting a primary meter) 5.4

11 To navigate through your patient s reports, select the dropdown menu with the report name. (5.5) 5.5 You can or save your patient s reports by clicking the " " or "Save" buttons at the bottom of the page. If you choose to the reports you will be prompted with a pop up to fill out whom you would like to send them to. Once finished select Submit. (5.6) 5.6

12 PREFERENCES, SETTINGS, AND SUPPORT (6) To see your settings and preferences, click the menu icon in the top right corner of the screen. From here you can access Report Preferences, Account Settings, Practice Info, Switch Practices and the LibreView Support website. (6.1) 6.1 In "Report Preferences", you can change the default settings for your patients reports e.g. time frame, target ranges and thresholds. (6.2) 6.2 In "Account Settings, you can change settings that include profile information and preferences. (6.3) 6.3 In Practice Info, you will have the ability to edit your practice information. (6.4) In Care Team, you will be able to invite other health care professionals to your LibreView practice and set practice administrator permissions. (6.5) 6.4 In Practice Records, you will be able to create new practice records to show what types of information will be displayed for your patients. 6.5

13 In the Switch Practices view you can see what practices you currently belong to, create a new one, and switch between practices.
