Accessing the Directory Apple Devices The SJ Provider Directory can be accessed by going to https://pd.sjhsyr.org in your standard browser. Android Devices The SJ Provider Directory can be accessed by going to https://pd.sjhsyr.org in your standard browser or can be downloaded from the Google Play store by searching for SJ Provider Directory. Other Devices or Desktop Computers The SJ Provider Directory can be accessed by going to https://pd.sjhsyr.org in your standard browser. Browsers The SJ Provider Directory can be used on almost any browser but works best with Internet Explorer, Chrome and Firefox. Requesting a New Account Before signing into the directory, you must request a new account by clicking on the Request New Account link at the bottom of the Login page. The Request New Account page captures the information needed to create your account and your directory listing. You will receive an email once your account has been approved. Logging In After your account has been created, you can login to the directory using your username and the password you entered when you requested your account. You username is the first 4 letters of your last name and the last 4 digits of your SSN. The application will automatically save your information so the next time you access the SJ Provider Directory, you will not have to login. Forgetting Your Password If you forget your password, you can use the Forgot Password? link on the Login screen. Enter your email address and a link will be emailed to you that will allow you to change your password. Navigating the Directory The directory menu can be accessed by clicking on the page in the directory. The options listed are: image in the upper left-hand corner of any Home: Home page which allows you to set your availability in the directory Providers A-Z: Displays all members of the directory in an alphabetical list Specialties: Allows you to filter the Providers A-Z list by Specialty Favorites: Lists those members of the directory that you have identified as a favorite My Profile: Allows you to change your information 2
Logoff: Logs you off of the application Your Availability (Home) The first screen of the application shows your current availability. By default, you are Available. If you are Unavailable, then your contact information will not be displayed in the directory. To change your availability, click on the +1 Hour, +24 Hours, +5 Days or Set Date button to extend your unavailability. The Set Date button will allow you to set a specific day and time that you will be unavailable until. Clicking on the I m Available button will clear your unavailability and display your contact information in the directory. Providers A-Z The Providers A-Z page displays all members of the directory in an alphabetical list. The availability of each member of the directory is displayed to the very right of their listing. A green dot means they are available which a red dot means that their contact information will not be displayed. The Providers A-Z page can also be searched by using the search bar at the top of the screen. 3
Provider Profile Clicking on a member in the Providers A-Z Page will bring you to that provider s profile page. The profile page contains the contact information for the provider and includes buttons that will CALL, TEXT or EMAIL the provider if they have entered that information into the directory. The +FAV button will add them to your Favorites list. Specialties The Specialties page allows you to filter the Providers A-Z list by Specialty. Favorites The Favorites option filters the Providers A-Z page by those members of the directory that you have identified as a favorite. My Profile The My Profile page allows you to change your information or change your password. 4
Logoff The Logoff option will log you out of the application and disabled the automatic login. You will need to enter your username and password the next time you access the directory. If you want to be automatically logged in to the application, do not logoff. 5
