My life. My healthcare. MyChart. What is MyChart? MyChart offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: request medical appointments; view your health summary from the MyChart electronic health record; view your lab and test results; request prescription renewals; access trusted health information resources; and communicate electronically and securely with your medical care team. How do I sign up? You can sign up three different ways. No matter which way you choose, you will need an activation code. This code will enable you to login and create a personal user ID and password. Please choose the method that works best for you. 1. Visit saintfrancis.com/mychart and click on the Sign Up link. Once you are on the login page, click New User and then No Activation Code on the next screen. After you enter your information, an access code will be emailed to you to complete your MyChart enrollment. 2. Sign up in-person with your medical care team during an office or hospital visit by asking for the activation code. 3. Call your physician s office to request that an activation code be mailed to you.
What do I need to use MyChart? You need access to a computer connected to the Internet and an up-to-date browser. Is there a fee to use MyChart? MyChart is a free service offered to Saint Francis Health System patients. Is MyChart secure? Access to information is controlled through secure activation codes, personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. MyChart uses the latest encryption technology to secure the integrity of your account. Unlike conventional email, all MyChart messaging is done while you are securely logged on to the website. Which physicians may I make appointments with in MyChart? You may use MyChart to schedule appointments only with physicians who have previously treated you while using our new electronic medical record. Appointments with a new physician may be made through referral, by calling your physician s office or by calling the Warren Clinic Referral Line at 918-488-6688. When can I see my test results in MyChart? Test results are released to your MyChart account within 72 hours after the test is complete. If I send a message to my physician or nurse, when can I expect a reply? You will generally receive an answer within 1 3 business days. Please note that MyChart should not be used for urgent situations. Please dial 911 if it is an emergency.
Will my entire medical history be made available in MyChart? MyChart is a direct link between you and the information in your electronic medical record, and all new medical history entries from Saint Francis Health System will be included. It will not reflect your complete medical history, though some patients may see past records in order to ensure continuity of care and access to information about ongoing treatments. Any appointment that has occurred in our new electronic medical record can be viewed under Appointments > Past Appointments. Can I ask my primary care physician for medical advice? Yes. You can send a secure message to your physician if you have seen them since enrolling in MyChart. Can I view a family member s health record in MyChart? This is called proxy access and allows a parent (or legal guardian) to log into their personal MyChart account, and then connect to information regarding their child. Adult patient information may be accessed by family members with permission of the patient, or if the patient is incapacitated, guardians, powers of attorney or other legally authorized representatives may be allowed to access information of the patient on MyChart. Some restrictions apply. Complete a Proxy Consent Form and return it to your physician s office to request access to this convenient service.
Can I ask questions regarding a family member from my MyChart account? MyChart offers direct access to your personal health record. If you asked a question about a family member, the response would appear in your health record and not in the family member s health record, potentially jeopardizing your family member s medical care. We recommend asking questions only about the patient who is currently logged in to MyChart. Can my spouse and I share one MyChart account? No, each adult must establish their own MyChart account. If some of my health information on MyChart is not correct, what should I do? Your MyChart information comes directly from your electronic medical record at your physician s office or the hospital. Contact your physician s office to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit. Is my activation code my user ID? No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
Can you send me a new activation code as I have lost it, let it expire or did not receive it? Please contact your physician s office to notify them. After we verify your information, a new code will be sent to you. My activation code does not work, what should I do? For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, contact your physician s office. Where can I update my personal information (e.g., home address, email or change my password)? Log into MyChart and go to the Preferences section and select the appropriate option. I forgot my password. What should I do? Visit the Saint Francis MyChart site and click the Forgot Password link on the sign-in page to reset your password online. What is MyChart s Privacy Policy? MyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records.
