The West Park Hospital Patient Portal is a convenient and secure health-management tool you can use anywhere you have access to the Internet. Through the portal, you can: View upcoming appointments View accessible results Review medication list Update and view profile information View previous appointments View your provider practice and hospital health summaries Send and receive messages with out-patient physician clinics View and pay billing statements How do I sign up for the West Park Hospital Patient Portal? You can enroll for West Park Hospital Patient Portal while you are a patient in our facility. You will need to provide a valid email address to our staff. If enrolling, after you have left the hospital, your last page of your discharge summary will include your medical records number (W#) and step-by-step instructions on how to enroll on the MyWPH Patient Portal. When your registration has been completed, you will receive an email invitation to create your account. Once you receive your email invitation, click the first link you see in the email and follow the directions to create a Login ID and password. You will then be asked to choose a three (3) security question(s) and answer in case you ever forget your password. Please note your Login ID, password and security answers are case sensitive. Can I enroll to view my children s medical information? Yes, parents or guardians of children up to age 18 can access their children s medical information on West Park Hospital Patient Portal. You will need to provide a valid email address during your child s visit to the hospital or visit the HIM department with valid identification after your visit to the hospital. You will need to sign a Release of Information form prior to being given access. Parental access is terminated just before the child s 18th birthday. Can my spouse and I use the same email address to view our Medical Health Records? Yes, you can use the same email to access both parties medical records. However, both parties must have a signed release of information on file within West Park Hospital Health Information Management Department.
*Please note the first person to sign up for the MyWPH Patient Portal will have the ability to add their spouse or children to their portal account under the same email address. Can I give permission for someone else to view my medical information, such as a spouse or my adult child? Yes, any patient age 18 or older can give proxy access to another adult. Both the patient and the proxy will need to present to West Park Hospital s Health Information Management Department to sign appropriate paperwork. How do I log into West Park Hospital Patient Portal? Please make sure you have a valid email address on file with West Park Hospital s Health Information Management Department: portal@wphcody.org or 307.578.2784. * Please have your medical record number (W#) on file before emailing or calling us for we are not allowed to give this over the phone. If you don t have this you will need to visit us in person with a valid ID. Once your account has been set up, you can access your MyWPH Patient Portal account anywhere that you have access to the Internet at home, work, school, library, etc. Just visit westparkhospital.org and click the MyWPH button at the top of our homepage. You can login using your username and password you created during registration process. What if I forget my password? You can reset your password by clicking on the Rest Password on the portal page. What if I forget my Login ID? You will need to contact our office at portal@wphcody.org or 307-578-2784. Is my confidentiality and privacy maintained on the Patient Portal? Yes, your privacy is a priority for us. The Patient Portal connects to a secure web site and utilizes encryption for all communications. Only limited information is available through the Patient Portal and your medical record is not displayed or accessible. In addition, the system will automatically lock your account if there are three (3) failed attempts to log in. You may also ask us to deactivate your access at any time. Please contact the Health Information Management Department at 307-578-2780 for assistance. How do I notify West Park Hospital if any of my personal information changes? The Patient Portal offers you the convenience of viewing the demographic information we have on record for you and making changes to that information. Contact information such as your address, phone numbers, and email may be changed when needed as well as personal information such as name of employer or insurance provider. This information will come to our Health Information Management Department where it will be verified and accepted.
Where does my health information come from? All of your information is pulled directly from your electronic health record. You will see information from your visits to West Park Hospital locations. It does not contain information from any visits to independent physicians offices outside of our organization, even if they are physicians who work with West Park Hospital. Also, information from Behavioral Health visits is not displayed, due to strict privacy laws regarding that information. What is included on the MyWPH Patient Portal? The West Park Hospital Patient Portal includes a view of clinical data from your West Park Hospital Electronic Medical Record. Your Health Record includes: Health Records Medications Allergies & Conditions Lab Results Visit History Health Summary Billing What is my Heath Summary and Continuity of Care Document (CCD)? Your Health Summary provides a snapshot in time of the most current medical information. This information is called a Continuity of Care Document (CCD) which can be viewed or downloaded and saved by you for future use or transmitted to your physician, by Direct Messaging, for follow up care. What are Direct Messages? The transmission of personal health information is performed using direct email messaging. Direct messaging is an easy and secure clinical messaging system within the patient portal that allows patients to send clinical information (CCD) to their primary physician office for follow up care. A direct email address must be obtained from the office which you are sending the information too. Please contact your physician office for additional information. What lab results can I view on the portal? You are able to view all of your lab results, 96 hours after the tests have been performed, except HIV, Genetics results, Pathology reports and Health Check labs. We are currently working on getting the Health Check labs available on the portal. Will I be able to see my radiology or pathology results? Due to the sensitive nature of these types of results you will be unable to view radiology or pathology results on the patient portal. A copy of these results can be obtained through West Park Hospitals Health Information Department. West Park Hospital is currently working on getting these additional test results available. What should I do if I have questions or concerns about a test result?
You should contact your physician s office if you have concerns about a result you review on the portal. What if my medical information is incorrect? If you have any questions about the medical information you see in West Park Hospitals Patient Portal, please contact the Health Information Department at 307-578-2784. What if I can t find what I need to on the MyWPH Patient Portal? Contact West Park Hospital Health Information Management Department if you need additional information that is currently not available in our Portal at 307-578-2784. How can I obtain a copy of my entire medical record? To have a complete copy of your medical records sent to another medical facility or to obtain a copy for personal reasons, click here to download a copy of our Release of Information Form http://westparkhospital.org/patients-health-information-management.aspx or call 307-578- 2784. Can I communicate with my physician through the Patient Portal? You can use the Portal to send and receive non-emergency communication if you are a current patient to following listed physicians. All incoming communication is received by your physician s office where it is distributed to the appropriate person for response. Please remember that communication through the Patient Portal is not a substitute for clinical appointments and will not be used to make a diagnosis or issue medical advice for conditions not currently being treated by your physician. Communications are monitored only during our regular physician office hours so the portal should only be used for non-urgent questions. We will try to get you a response within 24 hours, but depending on the nature of your question, more time may be needed before we are able to reply. You will receive an email alert advising you to log in to the Patient Portal to
