2 Lab Registration Procedure Step 1. From your Internet browser, go to: www.beaconlbs.com Step 2. Click the Login button and select Lab Login. Step 3. Click the Lab Registration link. 3 Help Tools PDF User Guide and Video Tutorial available Learn More Links that appear toward the bottom of the initial Login screen can also be found during the registration process by clicking the Learn More link at the top of each screen. 4 2 5 Step 4. Enter the following fields: Last Name, First Name, Title, Phone Number and Email address. Note: Required fields marked by *. Select your Security Question and enter the Answer. (Note: Answer is case sensitive.) The Login ID will automatically be populated with your Email address. You can modify the Login ID. It must have a minimum of 5 characters and is not case sensitive. Special characters allowed are [ _@. -]. The Display Name will automatically be populated with your First and Last Name. This field can be modified. Step 5. Click the Continue button.
6 7 Step 6. Enter the NPI # associated with your lab (if your laboratory has multiple facilities enter the Main or Headquarters NPI for your lab). These fields will be automatically populated upon clicking the Retrieve button. Step 7. Click the Retrieve button. The Laboratory Name, Address Information, and Billing Information will automatically be populated. These fields will be automatically populated upon clicking the Retrieve button. If the Billing address is different than the Laboratory address, this information can be modified. 8 Step 8. Modify the Display Name, if the NPI name does not reflect the Lab name. Note: This field will be what the provider sees when using Physician Decision Support (PDS). Enter the Lab URL (if appropriate) and Tax ID #. 9 10 Step 9. Check the box to verify the Laboratory information. Step 10. Click the Submit button. 3
11 name@domain.com Step 11. You will receive an Email from BeaconLBS to your registered Email address containing instructions for completing your Lab setup information. 12 Step 12. This is a sample content of the Email. Click the link in the Email where it says Login here. This will guide the user back to the main Login screen. 13 Step 13. Enter your User ID and Temporary Password contained in the Email. (Note: The User ID is not case sensitive. The Password is case sensitive.) 14 Step 14. Click the Enter Portal button. 4
15 Step 15. Enter your Security Answer (which is case sensitive) that you created during the beginning of the registration process. 16 Step 16. Click the Submit button. 17 Step 17. Create a password which must be a minimum of 7 characters, include at least 1 number and 1 upper and lower case letter. Step 18. Setup three additional Security Questions and Answers. (Note: Answers are case sensitive.) Step 19. Click the Continue button. 18 19 5
20 Step 20. Read the Terms of Use. Note: You may also print and keep for your records. 21 Step 21. Check the box at the bottom stating that you agree to the terms and conditions. 22 Step 22. Click the Continue button. You will be brought to the Summary screen. Using the tabs in the upper right area, the tabs represent areas which require completion, click each one to complete the following: Facility, Service Area, User and Map Tests. name@domain.com 6
24 25 Step 24. Click the Facility tab. Step 25. Click the Add New Facility button. At least one Facility has to be listed on the Facility tab. 26 27 Step 26. Either click the Use NPI # for Corporate Headquarters or enter the NPI # for the facility you are adding, if it is different from the Corporate Headquarters NPI #. 29 31 28 These fields will be automatically populated upon clicking the Retrieve button. 30 32 Step 27. Click the Retrieve button. Step 28. The Facility Name and contact information will be automatically populated, except for the Email address. Enter your Labs customer service email address, if appropriate. Step 29. Check the box to verify the Facility information. Step 30. Enter the CLIA #, select the Certification Level and enter the Expiration Date. Step 31. Check the box to certify the accuracy of the CLIA information. Step 32. Answer the Technology Profile questions. 7
33 Step 33. Click the Add Facility button to add additional facility, or Example Lab, Anchorage, AK 1234567890 34 Step 34. Click the Continue button, if all facilities have been added. 35 Step 35. You will be brought to the Service Area tab. 36 Step 36. Define the Service Area by selecting the check box for Service Area in all states or select a single State or multiple States. The County and Zip Code are optional and only need to be defined if your lab services a particular geographic region or within a state or states. 37 Step 37. Click the Continue button. 8
38 39 Step 38. You will be brought to the User tab. Step 39. To add additional users, click the Add New User button. 40 Step 40. Enter the Last Name, First Name and select a Role. The User Roles are Lab Admin User and Lab Outcome Manager. The Lab Admin User has the ability to modify the Lab information in the Lab Portal and the Lab Outcome Manager is only able to search, view, and print Outcome Summaries. Enter Phone Number, Email Address and click the Email User setup notification box. This allows the Username/Temporary password to be sent to the new user via the email address that was entered. Check this box to send notification email to new user. The Login ID will automatically be populated with the Email address. You can modify the Login ID. It must have a minimum of 5 characters and is not case sensitive. Special characters allowed are [ _@. -]. 41 The Display Name will automatically be populated with the users First and Last Name. This field can be modified. Step 41. Click the Add User button to add additional user, or 42 Step 42. Click the Continue button if all users have been added. 9
47 43 Step 43. Download the appropriate Guides. These are instructions for completing the template. 44 45 46 Step 44. Download the Test Mapping Template. Step 45. Download the Test Compendium Template. Step 46. If Laboratory of Choice, download the AOE Template. Can be completed at a later date. 47 Step 47. The initial registration process is now complete. Click the Log Out link in the upper right corner to log out of your account. The Submit for Approval button will not be active until at least one Template has been uploaded. You can use either the Guide and instructions that are included in the templates to complete the Templates on your own, or a BeaconLBS Representative will walk you through the process of completing the Templates. 10
48 Step 48. Once the Templates have been completed, Log back into the PDS Lab Portal, and go to the Map Tests tab and upload the completed files. Upload the completed files by clicking Choose File. 49 Step 49. After the Templates have been uploaded, click the Submit for Approval button. Note: The Lab Admin User can update any of the information contained in the Laboratory Setup by logging into the account and using the tabs in the upper right section of the screen. 11
