Important Information Iranian Scholarship Foundation 2018-19 Application Form Please take the time to read over the entire application and fill all pages out completely, taking careful note of the additional information that is required. All pages need to be submitted, in order, to be considered for the scholarship. Incomplete applications will not be considered. You may mail the application or send a scanned copy in PDF format via email. However, academic transcripts must be official from the university sent directly or in an officially sealed envelope and all tax documents must be sent via mail not email for your security. Faxed applications are no longer accepted. Please email the essay requirements even if you plan on mailing your application. Income Information Please provide complete copies of your parents and your income tax forms filed with the IRS. These include any 1040 s, all schedules if any, W2 s and other relevant tax forms. You must also complete IRS Form 4506. Do not send any money with this form, should we decide to send the form to the IRS, ISF will pay the fee. IMPORTANT: Tax documents must be mailed. For your security, do not email any tax documents. We will treat your financial records as confidential and only members of the Selection Committee will have access to them for review. Note that we may request other financial forms from you after reviewing your application. Letters of Recommendation Please submit two letters of recommendation. One must be from a current or former teacher and the other from an organization where you have worked/volunteered. Personal Essays Write an essay in response to each of the following topics: 1.) Write about yourself, your goals, and the challenges you have overcome and why you should be a recipient of an ISF scholarship. This essay should be about 750 words. 2.) Please tell us about someone you wish to emulate in terms of values, accomplishments, or other attributes and why. The individual can be either someone close to you personally or a public figure. This essay should be about 500 words.
