Making a referral to RAMP User Guide Self-registration If you work for an approved Family Violence agency your organisation s email domain address will enable you to self-register for the RAMP Referral Portal. You will then be able to complete and submit your referral directly to the RAMP Coordinator via the portal. You will only need to selfregister the first time you lodge a referral. Step 1. Paste this link into the address bar of your web browser and press Enter https://dhhsvic.service-now.com/ramp Step 2. Step 3. Step 4. Enter your name, area and email address into the User Registration Request page Enter and confirm a password that you will remember. This will be used to log in once your registration is completed Click Save & exit to submit your request Step 5. If your organisation s email domain address is on the approved list you will receive an email containing a link to the RAMP referral portal as well as your log in details. If your email address is not on the list, and you work for a specialist family violence agency, contact your local RAMP Coordinator.
Step 6. Using the details in the email, log in to the home screen of the RAMP Referral Portal. ** Note the referral process is broken up into Parts A and B. Part B will appear after Part A has been completed and saved. PART A Enter the Referrer, AFM, and Safety Profile details Step 7. Click on the Make a new Referral button **Note that all fields marked with a red asterisk are mandatory. If these are not completed you will see an error message when you save this section indicating which mandatory fields are missing. Step 8. Select the Area you are referring to by clicking on the icon to the right of the Area field. This is the area that the AFM resides in. The RAMP Coordinator field will populate automatically once you have selected the Area. Step 9. Click on the Referrer tab and fill in all of the relevant fields. Make sure that you include a contact number or email address for the referrer. If you know the SHIP Client ID, enter that here.
Step 10. Click on the AFM tab and fill in all the relevant information. Make sure that you include a contact number or email for the AFM. If you do not have either of these you can write Unknown in one of the fields. Step 11. Click on the Safety Profile tab and complete all of the fields Step 12. Click the Save button to save the information you ve entered and generate PART B of the referral
PART B Enter the Perpetrator and Children details, Indicate Risk factors and list Support Services **Note that there are two sets of tabs on the referral form. Part B involves the set of tabs at the bottom of the screen. Step 13. If you have details on the perpetrator scroll down to the bottom set of tabs, select the Perpetrator tab, then click on the New button. Step 14. Enter the relevant perpetrator details, then click Save & exit to return to the referral form Step 15. To enter children scroll back down to the bottom set of tabs, select the Children tab and click on the New button Step 16. Enter the relevant children details, then click Save & exit to return to the referral form. Repeat steps 15 and 16 for each child you are entering.
Step 17. Scroll back down to the bottom set of tabs and in the Risk Factors tab you will see all of the potential risks that have been associated with the case. Note that this will vary depending on whether or not you have added a perpetrator to the case at this point. The red dots in the Evaluation column indicate that the risks have not been evaluated yet. Step 18. To complete the risk assessment click on the Evaluate Risks then select a value on the right hand side of the pop up form. All risk factors need to be evaluated with either Yes or No or Unknown. Once you have completed your selections press submit to save them. The evaluations you have selected will then appear in the Risk Factors tab. Step 19. If there are other services supporting the AFM and her children click on the Support Services tab then click the New button and complete all of the mandatory fields. Step 20. Click Save & exit to return to the referral form
Step 21. If you have attachments that need to be included in the referral click on the paper clip icon at the top right of the page to open the attachments window. Click on Choose files to navigate to the attachment location on your hard drive, then double click on the file to add it to the referral as an attachment. Step 22. Once attached, the document can be accessed by clicking the link at the top of the page with the file name. You can also remove an attachment from here if it was entered in error. Step 23. To remove an attachment that was entered in error click on Manage attachments, click on the check box next to the file you wish to remove, then click on Remove button Step 24. Click on the Submit Referral button to submit the referral to the RAMP Coordinator.
***Note that if any mandatory fields have not been completed they will be listed in red at the top of the page. You will need to go to the fields listed in red and complete them before clicking Submit Referral again Step 25. Saving referrals as a draft It is possible to save a referral as a draft only if you have completed all of the mandatory fields in Part A. To save a draft referral click on Save & exit from the main part of the referral form. Step 26. To access draft referrals click on the Home link then click on List my draft referrals button and select the referral from the list.
