4 About this Planning Manual The Manual before you answers questions related to planning for the ceiling mount Leica OHC4 and Leica OHC5 from Leica Microsystems. With a weight of 500 kg and a boom radius of 1.43 m, it places particular requirements on the room planning and structural engineering. The following pages help all involved with the necessary clarifications: hospital technicians, architects, structural engineers, and electricians find here all the information and technical requirements necessary for a successful installation. In addition to the structural requirements, personal preferences should also be taken into account. Each operating room is shaped by the way its staff works. Therefore you will also find draft proposals in this Manual that help you find the optimum location for your Leica OHC4 / Leica OHC5. But even the best manual is no substitute for personal support. 6 About this section This section helps with checking the structural requirements. The load capacity of the ceiling plays just as much of a role here as the available space or the kind of operations that are performed. If the operating room does not meet one of the requirements, it does not mean that the installation is impossible it simply means that additional clarifications must be made. Most problems can be solved by adapting standard components. 7 Structural requirements The table below shows the structural requirements. Please compare the specifications with the conditions and enter the values in the table. Please send the completed checklist by fax or to your contact person at Leica Microsystems (see page 6). We will contact you as quickly as possible to discuss how to proceed. 8 Radius of action This is a recommended standard positioning. In each case, however, the surgeon must check that proper work is ensured in this position. (Taking into consideration the patient's position, the kind of operation, operating room table, operating room lamps and the position of the assistants and nurses. Also particularly critical is the use with MRI systems.) Work is not allowed without a rotation stop, since otherwise the cables are in danger. This is also the range of movement of the arms and weights of the ceiling mount. Ideally, there should not be any secondary devices in this range. A part of the range can be protected from collisions by rotation stops on the ceiling mount. 9 Wiring Required opening in the wall for control unit and transformer The Leica OHC system requires installation of the control unit in the wall. For this reason, an opening of exactly mm must be created with a mounting hole pattern in accordance with the drawing next to this paragraph. The depth must be at least 300 mm. 600~ Floor Overview of the Control Unit location and the cabling of the system: The transformer is placed on the base, which requires a height of the CU opening of at least 600 mm. All cables needed for the system operation, incl. Leica FL400 / Leica FL800, are provided along with the Leica OHC. The cables are 20 m long. If the distance from the CU to the stand is longer than 20 m, Leica Microsystems must be notified (see contact information on page 6). The standard cables are long enough if the stand control unit and the documentation system (Leica MDRS, HDMD) are located at the same place. Some cables are too short for other placements. 12 About this section The Leica ceiling mount OHC serves one purpose only: It should make work as easy and comfortable as possible for the surgeon. Every doctor has his or her way of operating, with a unique style and an arrangement that is best suited to him or her. For these reasons, it is of great importance that the doctor's needs and desires be taken into consideration during the planning. Where is the illumination located? The anesthesiologist? The assistant? Even the difference between left and right-handedness can be significant. Unlike most other fixtures, the position of the Leica OHC cannot be changed again. The better the setup is planned, the more pleasant it will be to work with the ceiling mount. Every question has its purpose This section poses various questions that are important for the planning and setup. Please read through these questions carefully. In the column on the right, you also find a short explanation of why these questions are significant. 13 > General questions Question What kind of operations is the Leica OHC4 used for? Is the operating room table moveable? Yes No Is the operating room table fixed on a base? Yes No Explanation The response helps determine the optimum positioning. In any case, it must be established that the operating field lies within the range of the Leica OHC and that the operating room illumination is not cast in a shadow. A moveable table grants more freedom for different procedures and is very advantageous in any case. A base limits the table's movability or even prevents movement entirely. This requires exact consideration of all possible procedures. In addition, it must be judged whether the combination of the Leica OHC and the position of the operating room lamps enable these procedures. In what range is the operating room table moveable? From which direction will the surgeon operate? Example Example 90 > The operating room lamps cannot be oriented towards the operation area from the side of the Leica OHC, since otherwise collisions with the Leica OHC may result. 16 Vibrations in general Vibrations of the microscope itself are absorbed by the damping material between ceiling mount and horizontal arm. Vibrations of the ceiling are reduced, because the ceiling mount does have a patented damping system for vibrations in the range of Hz. The microscope allows a vertical amplitude of less than 0.4 mm (peak to peak value) for all used working distances and the whole zoom range with eyepiece 10. Vibration absorber The vibration absorber is used for maximum stabilization of the microscope. Reducing the vibrations ensures a stable, clear image. Only vertical vibrations are absorbed, not lateral vibrations. Extrapolated to a vibration of the ceiling, the amplitude of the ceiling related to the floor must be smaller than ±1 mm. Short wavelengths are worst, longer wavelengths of a few seconds duration will reduce the perceptible effect. For better efficiency and reduced costs we recommend to absorb vibrations directly at their source (compressors, elevator machines, etc.). amplitude x = max. 1 mm X wavelength 16 Planning Manual for the Leica OHC4 / Leica OHC5 Ceiling Mount
17 General questions Question Is the facility in question a new construction or an existing operating room? New Modification Explanation For a modification, a special solution may need to be found in some cases. Is the operating room located near the helicopter landing site? Yes No Is the clinic located by a heavily traveled road with heavy goods traffic? Yes No Are there any sources of vibration in the immediate vicinity of the mounting point or directly above it? (For example, an elevator, compressors, walkways or drivable passageways.) Yes No Are there any elements already present that must be taken into consideration? This would include, for example, ventilation equipment, electrical equipment, etc. Yes No Does the operating room in question have a concrete or steel construction? Concrete Steel Additional vibrations occur in the vicinity of the helicopter landing site, which must be reduced by additional measures, or they could potentially hinder use of a ceiling mount. If unobstructed positioning of the stand cannot be ensured due to existing fixtures, other clarifications must be made first. With a concrete construction, a standard solution is possible in most cases. A steel construction requires a custom solution (see the sketch Basics of calculation for the ceiling plate construction ). Basics of calculation for the ceiling plate construction With steel constructions, it is particularly important that sufficient rigidity is achieved in order to prevent vibrations and residual shaking. 20 Help for the 3D planning Planning aid Collisions with other fixtures are a high risk. Since Leica Microsystems does not know the local conditions, we cannot assume any responsibility. To minimize the risks, however, the existing 3D data of the hospital can be supplemented with the stand data, prepared and checked for possible collisions by Leica Microsystems. Usable formats are DXF and DWG files. The costs for integration into the existing 3D plans amount to approx Swiss francs per 3D plan. This cost estimate refers to typical data integration.
