1 Introduction Thank you for agreeing to perform as a sterile compounding and aseptic technique Evaluator. Though this program has been evaluated to ensure it meets the criteria determined by USP <797> and the TSBP Rule ; if you practice pharmacy outside of Texas, it is your responsibility to ensure the requirements of this educational program meet your specific state s requirements. To ensure you receive all communications, please add the as a safe-sender in your system. The general process for evaluators: 1. Evaluator gets approval letter from TSHP to do perform evaluation services 2. Evaluator requests login for access to testing portal via and received the access granted notice 3. *Participant studies material* 4. Evaluator validates and supervises participant (forms available on testing portal after login or as part of this document) 5. Evaluator submits forms via the online form link in Step 3 Submit Process Validation Evaluation Checklists 6. After verifying all forms have been submitted correctly, filled out and the participant has been validated by PER, TSHP sends a forms confirmation to Evaluator that participant may be tested 7. Evaluator logs in, then lets participant set up their login and proctors exam Step 1 Textbook Materials Assist the participant as they read and work through the training materials. Evaluators are strongly encouraged to require participants to read the entire textbook, and complete the Chapter Self-Assessment quizzes. Evaluators may add additional content (i.e. facility specific information, policy and procedures, etc.). The specific content and format of additional coursework is up to the Evaluator/Facility, provided that it is relevant to the participant s learning experience in the course, prepares them to pass the required written exams, and equals up to a minimum of 18 hours of didactic experience. Step 2 Supervise the Participant Once you have been approved as an evaluator, you ll need arrange to supervise the participant for: Technician Participants: A minimum of 22 hours of process validations Pharmacist Participants: A minimum of 2 hours process validations While it is preferred that you oversee the participant s training, that is not always feasible. Therefore, it is acceptable for you to assign the participant to train with another pharmacy staff member provided that they meet the required qualifications and have been previously approved by TSHP as an Evaluator. Evaluators must complete the following forms with the assistance of the participant: 1. Supervised Practice Documentation Form. Evaluators must: I. PRINT all information required at the top of the form. This information must be legible to be accepted. II. Participant must be 100% competent at each procedure being evaluated. 2. Process Validation Evaluation Checklists (Note: Checklists I-VIII are required for all participants; IX-XII are optional if tasks are NOT performed at your facility) I. Aseptic Handwashing II. Gowning & Gloving III. Ampule Preparation IV. Vial Preparation V. TPN Preparation 1

2 VI. Horizontal Airflow Hood Cleaning VII. Vertical Airflow Hood Cleaning VIII. Sterile Solution Label Preparation IX. Biological Safety Cabinet Cleaning X. Hazardous Drug Ampule Preparation XI. Hazardous Drug Vial Preparation XII. Sterile Ophthalmic Solution Label Preparation Step 3 Submit Process Validation Evaluation Checklists Scan the Supervised Practice Documentation Form and all twelve (12) Process Validation Evaluation Checklists as file type pdf, jpg, gif, or doc. For optional Process Validation Checklists IX-XII, if your facility does not perform those tasks, please write NOT APPLICABLE on the form and submit with the other checklists. Submit all paperwork via the online form. COMPLETE THE SUPERVISED PRACTICE DOCUMENTATION & EVALUATION CHECKLIST FORMS HERE: Step 4 Proctor Exam Exam access details are provided below. Once you ve received your access as an Evaluator, you will see the PER Education Portal 3. The student will need to create a profile or log into an existing profile for the PER Education Portal 4. Student may then proceed to complete the appropriate exam, to be proctored or overseen by evaluator. 5. At the exam completion, the student is instantly displayed their exam grade and given the opportunity to review exam questions they have missed. 6. The system automatically transfers the earned credit to the My CPE Monitor; displayed within 72 hours. Schedule a time for you to supervise the participant as they complete the 100 question electronic exam. Please note: The online exam must be completed in a single sitting. There is no time limit imposed by the online exam, however should the internet connection get lost or other technical error occur during the exam, contact technical support through the Contact link at the top of the portal. The participant may find it helpful to make note of their answers as they go in case an examination re-take is necessary. The results of the exam are displayed instantly when completed. Participants that successfully complete the exam and have submitted all required documentation to TSHP will be issued credit via the My CPE Monitor system. Official statements of credit may be downloaded via the participant s My CPE Monitor profile within 72 hours. To receive credit for this activity the participant must provide all relevant participant registration materials, documentation of training hours, 9 Process Validation Evaluation Checklists with 100% accuracy supervised by a TSHP approved Evaluator, and pass one (1) written exam, via the PER Education Portal, with a score of 75% or greater. Participants will have one (1) online exam attempt. Participants that fail the exam will be provided with 2 additional opportunities to successfully pass with a minimum of 75% accuracy. Exam re-attempts are billed at $20 per exam and must be completed manually via a hard-copy form. Required materials will be mailed to the participant s Evaluator. Participants that fail to pass the exam within 3 attempts will be denied credit and will have to re-purchase the program. All credit is issued through the PER Education Portal and transferred to the My CPE Monitor system. It is the responsibility of the participant to ensure their PER Education Portal profile is complete and that the My CPE Monitor Details provided are accurate. Participants that fail to provide this information (or provide inaccurate information) at the initial completion of the exam will not be issued credit and have a maximum of 60-days from exam completion to update their profile with their NABP epid and date of birth. Participants that fail to update their information within the 60-day deadline will not be issued credit, regardless of program completion, and will not receive a refund for their 2

3 program materials. Participants that provide inaccurate or no data will be contacted via (evaluator copied) twice (2 times), no additional notifications will be sent. Please note: Participants that complete the exam process without authorization will be deleted and any credit issued revoked. Step 5 Obtain Statement of Credit Participants should download their official statement of credit for TSBP verification purposes via the participant s My CPE Monitor profile. Credit typically appears in a participant s profile within 72 hours of program completion. Please note, TSHP and Pharmacy Education Resources have no access to the My CPE Monitor system. 4 Instructions for Completing the Supervised Practice Documentation Form 1. The Supervised Practice Documentation Form must be properly completed and submitted with the Process Validation Evaluation Checklists for Pharmacy Education Resources National Sterile Compounding Manual in order for Participants to receive ACPE credit. Incomplete documentation forms or forms not properly completed will result in no ACPE credit being issued. Forms should be scanned and submitted electronically via the link: 2. The form must document a minimum of 2 hours of hands-on practice in the 8 areas listed below for Pharmacists. Or, this form must document a minimum of 22 hours of hands-on practice in the 8 areas listed below for Pharmacy Technicians. 3. This hands-on practice must be supervised by a TSHP approved IV Certified Evaluator. 4. Clearly print all information requested. All fields are required. 5. Date: Enter the date the practice takes place. Practice may take place over multiple dates and times. 6. Start time: Enter the time the practice starts. Practice may take place over multiple dates and times. 7. Activities Performed: During practice, the participant must perform a minimum of 2hr (RPH) or 22 hrs (Tech) of small volume and large volume IV preparation, the participant must adhere to the following process validation checklists included in this packet and training manual. The participant must have practice in the following areas (A-H REQUIRED): A. Aseptic Handwashing (pg. 430) B. Gowning & Gloving (pg. 431) C. Ampule Preparation (pg. 432) D. Vial Preparation (pg. 433) E. TPN Preparation (pg. 434) F. Horizontal Airflow Hood Cleaning (pg. 435) G. Vertical Airflow Hood Cleaning (pg. 436) H. Sterile Solution Label Preparation (pg. 440) I. Biological Safety Cabinet Cleaning (pg. 437) J. Hazardous Drug Ampule Preparation: Optional- if not applicable to site write Not Applicable on form (pg. 438) K. Total Number of Hours: Add the number of hours from the Total column. (Must be a minimum of 2hr (RPH) or 22 hrs (Tech) divided between small and large volume IV Admixture preparation.) 12. Evaluator Signature: The signature of the TSHP approved IV Certified Evaluator supervising the practice.
