2 QS PROGRAMS GENERAL CONDITIONS MONTH OF PARTICIPATION: depends on the program you choose. The MONTH OF PARTICIPATION is written on the vial, in letters: Ex: JANUARY, FEBRUARY, MARCH, APRIL, MAY, JUNE, JULY, AUGUST, SEPTEMBER, OCTOBER, NOVEMBER OR DECEMBER. THE DEADLINE FOR RESULTS INTRODUCTION IS: the last calendar day of the PARTICIPATION MONTH, written on the vial. The DEADLINE FOR RESULTS INTRODUCTION, should be considered. Thereafter, you cannot enter the results since the SOFTWARE WILL BE CLOSED. LOT. 4 RESULTS INTRODUCTION On delivery, the laboratory receives a unique code called "USER" and a password. The USER code is written on the products box, as code. The user and the password are required for entering the software. The results will be placed ONLY via internet, using the information from the external quality control vials: MONTH OF PARTICIPATION: written with letters on the VIAL. Ex: JANUARY, JUNE, JULY etc. and LOT: written on the vial "Lot." The Results will be introduced not later than the end of the month of participation written on the external quality control vial. Warning: The MONTH OF PARTICIPATION and the LOT chosen in the software, should comply with the MONTH OF PARTICIPATION and the LOT written on the vial. The wrong choice of one of the two (month of participation or lot) may lead to wrong results. Keep the packages (box, vials and bags) of the external quality controls until you receive the report results, or write down, the months of participation and lot numbers for each external quality control. Write down your unique code "USER NAME" and your PASSWORD, they are valid forever and you'll certainly need them every time you want to have access in to the software. 7 To introduce the external control results, go to the toolbar and choose DataEntry The list of programs that you have chosen to participate, will open. Fig: 8 Select the program that you want to introduce the results of the external quality control. In the present photo we chose "BACTERIOLOGY". 8 This screen is shown. To enter the results two ways, are available. Choose the simplest way for you. On the right part of the screen, where the red arrow appears, pressing the arrow under it, you can choose your LOT. The lot number, it s written on your control vial: Lot: Fig: 10 On the left part of the screen, you can choose the MONTH OF PARTICIPATION. by pressing the + sign. The MONTH OF PARTICIPATION, also, written on the vial, in letters: Ex: JANUARY, FEBRUARY, MARCH, APRIL, MAY, JUNE, JULY, AUGUST, SEPTEMBER, OCTOBER, NOVEMBER OR DECEMBER. ATTENTION: the expiration date of the product, has nothing to do with the month of participation. Shows that the product is valid. The month of participation is the month written on the vial, NOT the month that the laboratory performs the controls. For example: Let s say that your month of participation is SEPTEMBER. You can perform your controls in August. You will enter the results choosing SEPTEMBER in the software. Because, your control vial is for SEPTEMBER. The results will be compared with the participants of September. Be sure that the LOT and the MONTH OF PARTICIPATION, that you choose in the software, are the same with those written on your vial. 8/30

10 CLINICAL CHEMISTRY SCREEN: Edit: Pressing this button, you have the possibility to enter your results. Once pressed, this button turns, in to Update and Cancel. ANALITE: 32 parameters METHOD: Choose reagent method that can be find in the insert of the reagent box. The methods can be: IFCC, DGKC, DRY CHEMISTRY, IFCC, ENZYMATIC, COLORIMETRIC, UV CINETIC, HEXOKINASE, DIRECT IMMUNOINHIBITION ISE etc. U.M: Unit, cannot change. If your result is in another unit, please convert it to the unit of the software. INSTRUMENT: Select the instrument with which you performed the control tests. VALUE: Result you found for the specific analite. After you enter the result, click Update. The result is saved. Follow the same procedure for every analite. Save: Click SAVE if you need to enter in the system, some other time, to make some changes at the results you already entered. It means that, you have to make those changes soon. Pressing SAVE, the results are not sent to the organizers to be compared. Save and Send: Pressing SAVE and SEND, the results are sent to the organizers to be compared. 14 BACTERIOLOGY SCREEN: To introduce your results, select NEW. BACTERIUM CEPP: Select the type of bacteria you found. It can be, for example: HELICOBACTER PYLORI, PROTEUS MIRABILIS, SALMONELLA ENTERICA, STAPHYLOCOCCUS AUREUS, CANDIDA ALBICANS, ESCHERICHIA COLI etc. METHOD: Choose your working method. It can be, for example: MANUAL KIRBY BAUER, AUTOMATED KIRBY BAUER, CULTURAL, etc. INSTRUMENT: Select the instrument with which you performed tests. You can choose, also MANUAL. ANTIBIOTIC: Ex: Amikacin, Amoxicillin, Augumentin, Cefaclor, Cefepime, Eritromicina, Econazol, etc. RESULT: You may enter the results as: SENSITIVE, RESISTANT or INTERMEDIATE. After you enter the result, click Update. The result is saved. Follow the same procedure for every antibiotic. If you don t want to save, the entered result, press Cancel. Fig: 17 To enter a new antibiotic result, click New, on the left of the result last introduced. Insert the result and save the information by clicking Update. Edit: You can change the results you already entered. Save the new information by clicking Update. Delete: all the information you saved for that antibiotic, will be deleted. After you enter the results, you need to click SAVE or SAVE AND SEND. Save: Click SAVE if you need to enter in the system, some other time, to make some changes at the results you already entered. It means that, you have to make dose changes soon. Pressing SAVE, the results are not sent to the organizers to be compared. ANALITE: 13 parameters STRIP: choose the strip you used. U.M: Unit, cannot change. INSTRUMENT: Select the instrument with which you performed the control tests. RESULT: It can be : POSITIVE, NEGATIVE or (-). OR VALUE: Result you found for the specific analite. If your results are expressed in numbers, you have to declare also the normal values (minimum and maximum value range), that your laboratory uses. Without the normal values your results will not be processed. This applies anyway for SPECIFIC GRAVITY and PH. V.min: minimum normal value, that your laboratory uses. V.max: maximum normal value, that your laboratory uses. After you enter the result, click Update. The result is saved. Follow the same procedure for every analite. If you don t want to save, the entered result, press Cancel. After you enter the results, you need to click SAVE or SAVE AND SEND. Save: Click SAVE if you need to enter in the system, some other time, to make some changes at the results you already entered. It means that, you have to make dose changes soon.
