1 2018 Spring - Membership Application This application is for all prospective candidates, affiliates and returning members. Returning members must apply each semester to confirm continuing eligibility for Beta Alpha Psi. This application takes approximately 5 minutes to complete. If you have any problems with this form, contact reporter@bapusfsp.org. 2 All applicants must upload a copy of their Degreeworks transcripts so that the appropriate eligibility requirements can be reviewed and confirmed by the chapter reporter. No other transcripts will be accepted. To download your Degreeworks transcript, follow these steps: (if option 1 fails, please follow instructions for option 2) Option 1: Go to Login using your USF NetID, if necessary. Click "Save as PDF." A pop-up window will display your transcript. Click the save icon in the lower-right corner of the transcript. VERY IMPORTANT: Change the name of the file and type ".pdf" at the end of the filename (ex: "mytranscript.pdf"). Don't forget where you save the file. The file has now been saved. Skip option 2 and upload this file using the upload button below. Option 2 (only follow these directions if option 1 failed): If you already have PDF printing software installed on your computer, please skip to step 2. If you received an error while trying to save your transcripts you will need to install a PDF printer to complete the process. This software will allow you to convert any printable document/webpage into a PDF file. Understand that by following the instructions below you will be installing new software onto your computer that has been tested and is virus free. These instructions were prepared using internet explorer, but should work similarly with other browsers. Step 1: Install PDF Printer Follow this link and click the download button. When prompted to Run, Save or cancel, click "Run". When the install window appears click "Install Now" and the program will automatically install. After the install completes, click the close button on the install window and continue to Step 2. Step 2: Print transcripts to a PDF file. Go to Login using your USF NetID, if necessary. Click "Print" (located on the menu bar at the top of the web page). When the printer dialog box appears, select "dopdf 8" from the list of available printers and click "Print" You will be prompted to save you transcripts as a PDF file. VERY IMPORTANT: Select the location where you would like to save the file, then change the name of the file and type ".pdf" at the end of the filename (ex: "mytranscript.pdf"). Don't forget where you save the file. The file has now been saved. Upload this file using the upload button below DegreeWorks Transcript [Required] Florida Shines Unofficial Transcript To get the unofficial transcripts: 1. Go to Floridashines.org 2. Click on "check your progress" under "Succeed in College" 3. Then there will be a link to get your transcript, click on it. 4. A box will pop up where you choose University of South Florida and that will prompt you to put in your student information in "netid and password" 5. Your unofficial transcript should come up. 6. Save your transcript as a PDF file and upload it here. 7. Information about the dues is provided here for your consideration. Once you have reviewed the dues, click next to continue with the application. Membership Dues List Affiliates - $150 ($100 will be applied to your candidate dues next semester) New Candidates - $ Returning Candidates & Members - $40.00 Transfers from Other Chapters - $40.00 How Do I Pay Dues Links will be made available to you to pay your dues online toward the end of the application. Alternatively, you may pay by check, credit card, or money order in person at the next meeting. Cash is never an acceptable form of payment to the chapter according to our chapter constitution. If you are unsure about your membership status and do not know what amount of dues to pay, please reporter@bapusfsp.org for confirmation, or ask an officer at the next meeting. Committee Selection Every Beta Alpha Psi candidate and member is required to serve on a committee. How involved you are is up to you, but we do require everyone to support the chapter in some small way. Here is a short description about each committee. Competitive Engagements Committee - Every year our chapter participates in the Best Practices competition at the Regional and Annual Beta Alpha Page 3 of Spring - Membership Application

4 Psi national meetings. This committee designs, plans, and executes a project that will be entered into the competition. We need creative, imaginative, out-of-the box thinkers! Media Committee - This committee is responsible for documenting the chapter's activities through photos, videos, and narratives. The committee posts updates to our website and other social media sites. Service & Socials Committee - This committee plans and promotes the chapter's service and social events. If you like to plan fun events or just enjoy coming up with creative activities, this is the committee for you. Special Events Committee - This committee plans our annual Induction Banquet which takes place every fall semester. We will need several people to help with sending invitations, preparing decorations, organizing table assignments, etc... If you enjoy planning large events this is the committee for you. Set Up Your PeteSync Profile All communications and chapter information goes through the our PeteSync portal. If you haven't already done so, take a moment to update your PeteSync profile with your mobile phone number. If something changes last minute at an event (room change, etc.), we will text the entire chapter to let them know. You can access your profile here. Once you are done with that, click on Notification Settings (or click here). Choose how you want to receive communications from Beta Alpha Psi. Please note that if you uncheck any of the notification settings you run the risk of not receiving important chapter updates. If they fail to meet the requirements while on probation, they must be expelled from the chapter.
