1 Graduate Award Application Worksheet Instructions This worksheet is provided for you to collect and organize your information in preparation for submission on the official on-line application form for ALL School of Information Technology graduate student awards. Only students who complete and submit the official on-line application will be considered for School awards. All of the information requested on this form is required in order to assure your full consideration for all eligible awards. Applications must be submitted on-line by Friday, February 9, Eligibility: The School of Information Technology graduate awards are limited. Only students currently enrolled as Information Systems graduate students planning to graduate in May, August, or December 2018 are eligible to apply. The following awards will be given: Outstanding Graduate Student Distinguished Graduate Student Outstanding Graduate Assistant Awards Reception: All School Awards will be presented at the School of Information Technology IT Celebration on April 27, Award recipients are required to attend the award presentation. Award recipients will be notified in advance of the event.
