1 How-To Guide For Obtaining Pharmacy Transcripts and GPA for PhorCAS Pharmacy Residency Applications Last updated: 11/6/17 by Julie Mandel, PharmD Candidate 2018 Step 1: Calculating your PharmD GPA You will need to upload your GPA from your pharmacy courses (P1-P4 years) This GPA is not visible on academic transcript but is visible through PeopleSoft Go to my.pitt.edu On right hand side select student center Select self-service Select student center Select my academics on upper left-hand side Click on view my advisement report Scroll all the way to the bottom where it says GPA of all pharmacy courses and click on the arrow next to it to expand the selection This is your GPA that should be entered on PhorCAS (I called PhorCAS to confirm this information and confirmed with Marcia what students in previous years have done) 1

2 Step 2: Entering your PharmD GPA into PhorCAS First, you will need to register for PhorCAS and the match through their online website Once you have an account created, you will see this screen to begin entering information You will need to enter your academic institutions o Click Non-pharmacy colleges attended to enter any other schools you attended and received non-pharmacy degrees at if you want (If you only attended Pitt leave this section blank) o Click Pharmacy colleges attended to enter in Pitt 2

3 o Once you enter the school information this screen should appear, click on the blue icon below update/delete o At the bottom of the screen select yes for did you have a calculated GPA at this institution? o Enter the GPA that you obtained in step 1 for your PharmD GPA 3

4 Step 3: Requesting a Transcript from Pitt On PhorCAS, under the screen that you went to above to enter your GPA click on the transcript request form button The form should look like this: PRINT out the form and fill it out handwritten. Scan the handwritten completed form to have a digital copy. My recommendation is to title the file First name last name PhORCAS ID #. (See more instructions on PhorCAS. 4

5 Next, you have to go through Pitt to request a transcript. Hold on to the file you just created above because it will need to be attached to the transcript request form. Go to registrar.pitt.edu, under service areas select transcripts/verifications Select order transcript on the right hand side under I am a current student Log in with your pitt credentials to be prompted to next screen which is by the Registrar service called Parchment You will now be prompted through a series of screens to select the right transcript to order Click order now Select Paper transcript- mailed $6.00 (Per my conversation with PhorCAS, our school is not listed as one of the ones which are allowed to send electronic transcripts so must do mailed version) Fill out the information as follows: 5

6 o Mailing name: PhORCAS Transcript Department o Mailing Address: P.O. Box 9124 Watertown, MA o Processing time: NOW o Mailing method: whatever you want o Attachment (optional): UPLOAD THE PHORCAS FORM THAT YOU SCANNED HERE! o Purpose for transcript: optional, I chose employment because seems most relevant, but up to you o Quantity: 1 (you only need to get ONE transcript regardless of the # of programs you are applying to) Continue through other screens to provide payment information and complete your order Of note: if you did the 2-4 program at Pitt (2 years undergrad at Pitt) the transcript that will be sent will include all 6 years of coursework. As per PhorCAS- THIS IS OKAY! If you would like to see a sample of what your transcript will look like you can go to through your people soft student center à self-service à academic records à view unofficial transcript Of note, the transcript must be sent directly from the Registrar office to PhorCAS- you cannot mail in yourself a copy of your transcript that you may receive from the Registrar because it will say student copy or issued to student, and PhorCAS will not accept those documents. *It is your responsibility to read through the instructions on PhorCAS in detail and make sure you are doing everything correctly.
