2 PIM(PDO Incident Management) Creating First Report PIM (PDO Incident Management) is a powerful enterprise risk management (ERM) solution for environment, health, safety and sustainability management through below modules:- a) IMPACT ERM Incident Management IMPACT Incident Management is configured as per PDO business workflows. The incident data captured and other raw data behind the system is used to generate leading indicators of performance that can be used to reduce the likelihood of future events. b) IMPACT ERM Assessment Management IMPACT Assessment Management effectively addresses the proactive side of business and is configured to handle both Closed-format (with checklists) and Open-format audits and assessments. It allows business to use data to identify areas of risk and make adjustments in business processes and behavioural observations to actually lower risk and the potential of an event. The user is expected to enter details of an incident witnessed by him/her, into PIM, which will in turn send an e - mail notification to the Responsible Supervisor. He then passes on additional information to the Incident Owner, who assigns responsibilities to the other individuals in the hierarchy. Depending on the severity of the incident, investigation and corrective/preventive measures are taken up until the incident status is closed. Steps to be followed by the User(s) while entering details of an incident: 1. Browse the PIM from HSE Home Page by clicking on PDO Incident Management (PIM) link on the top-left corner of the HSE home page under Tools. This will take you directly to PIM home page Page 2 of 8

3 2. Creating a First Report Anyone witnessing an event can enter basic information about the event as a First Report of Incident. This information is stored as an incident record and the appropriate people are notified about the event. To create a first report: 1. Do one of the following: From the Track section of the Main Menu, click First Report From the My Tasks section of the Tasks menu on the Home page, click Add First Report Page 3 of 8

4 The First Report of Incident/Near Miss window opens. 2. Select the lowest department level for the site in the Responsible Dept/Unit list. (The site where the incident happened and the department belong to) Note : The selection that you make in this field determines the data that is displayed in other sections and lists on the First Report record. 3. Select the Event type (for example, Incident or Near Miss). Event type definitions Page 4 of 8

5 Any sub types applicable to the selected level of consequence are displayed. 4. Select the relevant check boxes in the Incident Sub Types field. The incident sub types appear only when a event type is selected. 5. Click Next.. The First Report of Incident is displayed, depending on the previous selection. 6. Complete all required fields, identified by an asterisk (*), and complete any other additional fields with information pertinent to the incident. Page 5 of 8

6 7. Click Review. Alternatively, click Back to change the Responsible Dept/Unit, event type, or Incident Sub Types on the previous page. The First Report of Incident preview page is displayed summarizing the data entered into the First Report. 8. Click Save to save the first report. Alternatively, click Back to change any necessary information on the previous page. 9. The incident will be in Draft 1 status and can be seen a n d e d i t e d by the reported by only. Page 6 of 8

7 The Incident Read Only page opens. This page contains the new Incident ID number and an Options Menu to help you perform additional actions for the record. This page also contains a section for the recommended consequences based on the incident sub types selected. Click the Options menu then Edit to complete any additional actions for the incident record or send it to the responsible supervisor. Page 7 of 8

8 10. Click the Options menu then Submit to responsible supervisor to allow the responsible supervisor to see the incident and complete it. The incident status will move to Draft 2 and it can be seen by the reported by and responsible supervisor only. And it can be edited by the responsible supervisor only. 11. The Responsible supervisor can review the incident and add more information if needed. Then he should open the incident by Clicking the Options menu and Open. This will open the visibility of the incident to all the PIM users. 12. He can change the incident owner if he is not the one and the incident owner will be able to add the consequences, investigations, action items and close the incident if there is no incident approver, otherwise he should submit it to the incident approver by clicking the Options menu and Submit to approver who can close the incident or reject it. Note : An notification will be generated automatically by the system and sent to the individuals for any responsible user or notify only during the incident progression Page 8 of 8
