You will receive an telling you an incident has been reported and that you are the investigator If at any stage you get stuck with this please contact the Health and Safety Unit for Help. Below is an example of the you will receive Incident Reported This is a confirmation to say that the following Incident has been logged in which you are assigned as the Investigator. Regards, Health & Safety Department You are require to click on the link above which will take You to the planon system which is illustrated on the next page 2
3 Screen Shot of preliminary Investigation The page will contain all the incidents assigned to the manager logging in, those incidents that belong to them. The latest incident will be at the top. Click on that incident to open it which will take you to the screen on the next page. 3
4 This page presents the incident details and gives two options at the bottom the page. Click on investigation in progress if you are going to investigate the incident Click on reassign the investigation if you are not going to investigate but assign to someone else to investigate. 4
5 When you click on the arrow investigation in progress you will get this screen. You can enter details of what is happening in the communication log then submit. The investigation is now in progress 5
6 After you have submitted the incident when you go back into it there are three options. Update with further details Put on hold because you are waiting for other information Or close the incident because the report is complete 6
7 Screen shot of what it looks like when you click update investigation add detail. You get a range of questions to answer as part of the investigation. You need to work through them. After the questions there is a free text area called communications logs where you can put any further details. Once this complete you press submit and the investigation at this stage is finished. There are 5 sections of yes no questions please answer those that are relevant you can leave those that are not relevant blank 7
8 Once you have gone through the questions you can also add any further information in the communication log. You can also use the browse button to upload any documents, photos etc to support the investigation. 8
9 If you choose to reassign the investigation you will get the following page seen below, you need to pick the manager you what to reassign the investigation too. You can also had notes via the communication log. If the incident is reassigned new investigator will get the same information as at start of process and go through the investigation. See page 1 9
10 If you pick Investigation on hold you will get the following menu and drop down list from which you need to pick one and also add any further information in the communication log and submit 10
11 Report Complete Any additional information can be submitted in the communication log, If you are happy that your investigation is complete press submit. If you need further information you can press cancel and go back to the previous page and put the investigation on hold. Once the form is submitted the health and safety team will review the form and either close the incident or ask for further information. If there is a request for further information the investigator will receive an to add further information. 11

13 Report Complete. Once submitted you can click onto the report in the planon self-service and it will look like this screen shot below. 13
