1 HOW TO ACCEED TO AN FIA ENTRY FORM Select the link Or the FIA website, select FIA SPORT, select COMPETITORS AREA, select FORMS, select Online Entry Form for all the FIA championships. STEP 1 : USER S NAME AND PASSWORD Application for the USER name ( User/utilisateur ) and for the PASSWORD ( Password/mot de passe ) in order to access the ENTRY FORMS. LOGIN Password/mot de passe Connect yourself by indicating your User name as well as your Password. If you do not have a user name and a password yet, please apply for them by clicking on the «Cliquer ici /Click here» link at the bottom of the page (see yellow arrow). You will then accede to the Login information. Login information / Demande de création de compte Complete the application by filling out as a minimum the mandatory fields indicated in red. Validate your application. You will receive by the notification of your application as well as the necessary data to complete the forms. 1/ First from the FIA = notification of your application for a User name and Password. 2/ Second from the FIA = Communication of your User name and of your Password. Necessary data to draw up Entry Forms included in a PDF file. 1/5

2 After receiving this 2nd , it is necessary to wait approximately one hour while the system validates your User name and your Password. STEP 2 : ENTRY APPLICATION In the event of a problem regarding the Entry Form, you must always quote the application code and number ( Request Number ) which has been allocated to you (see yellow arrow). Select the chosen Championship by clicking on the Select link (see green arrow). Select Rallies on the Discipline by clicking on the scroll down menu (see blue arrow). Select the chosen Championship by clicking on the scroll down menu (see green arrow). And then validate your application with OK (see yellow arrow). COMPLETE THE LOGIN INFORMATION *at least the Mandatory fields in red 2/5

3 COMPLETE THE ENTRANT INFORMATION Complete the Entrant part by filling out as a minimum the mandatory fields indicated in red. The name of the competitor is the one indicated on your competitor licence. If the Entrant data is identical to the Login/account information data, click on Initialisation (see yellow arrow, bellow). If you are waiting for the issue of your licence, please write TBA COMPLETE THE DRIVER S INFORMATION Add one Driver by clicking on the Add driver link (see green arrow). If the Driver data is identical to the Login information data, click on Initialisation (see yellow arrow). Complete the Driver part by filling out a maximum of fields. Validate your application with the OK button. 3/5

4 COMPLETE THE CO-DRIVER S INFORMATION, IF NECESSARY Add one or two Co-Driver(s) by clicking on the Add co-driver link (see blue arrow). Complete the Co-Driver part by filling out a maximum of fields. Validate your application with the OK button. COMPLETE THE TEAM S INFORMATION, IF NECESSARY Add a team by clicking on the Add team link (see yellow arrow). Complete the team s part by filling out a maximum of fields. Validate your application with the OK button. COMPLETE THE VEHICLE S INFORMATION Add one or two vehicle(s) by clicking on the Add vehicle link (see red arrow). Complete the vehicle part by filling out a maximum of fields. Validate your application with the OK button ATTACHMENT FILES You must attach at least: - A copy of the FIA Historic Technical Passport (HTP) - A photograph of the Driver (picture in JPG format) - A photograph of the Co-Driver (picture in JPG format) - A photograph of the Vehicle (picture in JPG format) Add the attachments by clicking on the chosen insert links (see green arrows). By clicking on the Parcourir / search for link you will find your document in your computer. 4/5

5 CLAUSES AND DECLARATION Click on Accept the Inscription clauses and on the Accept the Common Declaration. (see above, yellow arrows). Please carefully read the statement and the clause and accept them by clicking in the white square (see under, blue arrows). PAYMENT You can choose by paying or by Credit card or by Bank transfer. VALIDATION Finally validate the entry form, by clicking on Validate (see yellow arrow, below). You will first receive by an acknowledgment of receipt of your entry application. Then you after our validation, you will receive by our confirmation and a PDF version of your entry application form. 5/5
