7 Terms and Conditions Placing An Order: Orders may be placed with your Monarch Instrument Distributor or directly with Monarch Instrument in Amherst, NH via fax, , U.S. Mail or at Telephone orders must be confirmed in writing by one of these methods. The order should contain the Monarch item numbers, quantity, price, purchase order number, shipping and billing addresses, method of payment, preferred shipping method and required delivery date. Also include a contact name with a corresponding , phone and fax number. Upon receipt of the order, Monarch provides a confirmation of the order, if requested. Repair and Calibration Service Policy: All items being returned to Monarch Instrument for repair and/or calibration must have an RMA Number and be shipped prepaid by customer. Items accepted for Warranty repair are serviced at no charge in accordance with the terms of Monarch Instrument s Warranty policy. Repair charges for physically damaged or out of warranty items will be quoted at the time the RMA number is evaluated. Payment for out of warranty repairs shall be in accordance with terms stated above. All items returned to Monarch, but found to have no problem, will be subject to a $65.00 evaluation fee.
