1 CORNWALL-LEBANON SCHOOL DISTRICT 105 EAST EVERGREEN ROAD LEBANON, PA Attached is a list of Fall and Spring Athletic Equipment and Supplies for the Cornwall-Lebanon School District ( School District ) for the school year. We invite a sealed bid from your firm on these supplies. The following conditions are to govern the bidding: 1. If the bidder is submitting a proposal on the items as specified, entries should be made in the appropriate line and block. If bidding an alternate and/or equal these should be entered on the same line as the base bid item. Alternate bids must include manufacturer s description of the item bid upon plus the bid must be accompanied by descriptive literature, etc. Samples of alternate bid items are to be supplied upon request of the School District. 2. An Itemized Bid Sheet is enclosed with this Invitation to Bid. Unit price and total price must be shown on the Itemized Bid Sheet for each item that is bid by your company. All bid responses must be submitted on the District s Itemized Bid Sheet to be considered. The Itemized Bid Sheet may be handwritten or may be returned in the form of an electronic spreadsheet. An Itemized Bid Sheet that is completed electronically will be accepted only by physical delivery via CD. Electronic submissions will not be accepted. Electronic versions of the Itemized Bid Sheet may be obtained by sending a request for Fall/Spring Athletic Bid Spreadsheet to rsnyder@clsd.k12.pa.us. 3. The Form of Proposal is to be executed by an authorized representative of the company submitting the bid and shall include the title of that representative and the date. 4. Sealed bids must be delivered or mailed to Mrs. Jean Hentz, Director of Business Affairs, Cornwall-Lebanon School District, Educational Service Center, 105 East Evergreen Road, Lebanon, Pennsylvania , prior to 1:00 PM on Tuesday, April 17, 2018, at which place and time there will be a public opening and reading of received bids. Bids should be clearly marked Bids for Fall and Spring Athletic Equipment and Supplies on the outside of the envelope. 5. The successful bidder(s) shall deliver the items to the Cedar Crest High School, 115 East Evergreen Road, Lebanon, PA by Friday, August 3, Any items not received by Friday, August 10, 2018 will be considered as cancelled. 6. The billing date for all equipment and supplies shall be after July 1, Bid prices are to include delivery charges to the address listed above. 8. All items when invoiced and shipped should be clearly marked as to the status of the item (i.e. backorder or cancel). FALL/SPRING ATHLETIC SUPPLIES 3/2/18

2 9. The District reserves the right to reject any items that arrive damaged or inferior or not according to specifications. Vendors will be responsible for arrangements and expenses of items shipped incorrectly to the District that need to be returned to the vendor. 10. The Cornwall-Lebanon Board of School Directors reserves the right to accept or reject any or all bids and to waive, at its discretion, any irregularities, mistakes, omissions, or informalities pertaining thereto. 11. No bid shall be withdrawn for a period of 60 days after the date of the bid opening. 12. The Cornwall-Lebanon School District is exempt from all State Sales Tax and Federal Excise Tax and these taxes shall not be included in any bid prices provided by the bidder. 13. All shipments are to be made to the Cedar Crest High School between the hours of 8:00 A.M. and 2:00 P.M., Monday through Friday, unless a school holiday is declared. 14. Present plans are for the Bid Award to take place at the meeting of the Cornwall-Lebanon Board of School Directors to be held on Monday, May 21, Contract: The Advertisement for Bids, the Bidder s Form Of Proposal and a Purchase Order or Contracts and its attachments, either attached hereto or included by reference, when received by the successful bidder shall constitute (1) a binding Contract on the terms set forth therein and (2) the entire Contract is to be interpreted, construed and given effect in all respects according to the laws of the Commonwealth of Pennsylvania. 16. A Non-Collusion Affidavit that is included with this bid packet must be signed, notarized, and returned with the bid. Failure to comply with this provision will disqualify the bidder.
