3 DynaMed DynaMed is a clinical reference tool created by physicians for physicians and other health care professionals for use at the point-of-care. With clinically-organized summaries for more than 3,200 topics, DynaMed balances the latest content and resources with validity, relevance and convenience, making DynaMed an indispensable resource for answering most clinical questions during practice. Updated daily, DynaMed editors monitor the content of over 500 medical journals on a daily basis. Each article is evaluated for clinical relevance scientific validity. The new evidence is then integrated with existing content, and overall conclusions are changed as appropriate, representing a synthesis of the best available evidence. Through this process of Systematic Literature Surveillance, the best available evidence determines the content of DynaMed. DynaMed is available as a mobile application, compatible with devices such as Android, BlackBerry, iphone, ipad, and ipod touch. 4 DynaMed Home Page The DynaMed Home Page provides search and browse options to locate topics, and offers a menu on the top of the page that provides access to other DynaMed features. Note: To quickly access the Home Page from any location in DynaMed, click the Home link located in the top left corner or click the DynaMed logo. DynaMed User Guide 4 March 2013
5 DynaMed Screen Text Size There are three text sizes available in DynaMed small, normal, and large. The default text size is normal. The text size is managed by a series of controls in the top right section of the Result List, Category Browse, Topic page, and Recent Updates pages. To change the DynaMed text size: The default text size is normal the middle text size control selection. The following steps assume normal text size selected. Note: 1. Locate the text size controls in the top right area of the DynaMed screen. 2. Do one of the following: To adjust the DynaMed text to a smaller size click the leftmost, smallest A link. To adjust the DynaMed text to a larger size click the rightmost, largest A link. DynaMed adjusts the text size immediately. To modify the text size again, click the applicable A link. 6 Level of Evidence Labeling in DynaMed DynaMed provides easy-to-interpret Level of Evidence labels so users can quickly find the best available evidence and determine the quality of the best available evidence. Evidence may be labeled in one of three levels: Level 1 Level 1 (likely reliable) Evidence - representing research results addressing clinical outcomes and meeting an extensive set of quality criteria which minimizes bias. DynaMed criteria for level 1 (likely reliable) evidence labels DynaMed criteria for level 1 (likely reliable) evidence for conclusions from a systematic review 1. Full-text report available in English (or language well understood by participating editor) 2. Clinical outcome (also called patient-oriented outcomes) 3. Systematic search 4. Explicit inclusion criteria 5. Systematic selection of included studies 6. Evaluation of study quality 7. Additional criteria if meta-analysis Studies are clinically appropriate for pooled analysis (reasonably similar populations, interventions, methodology and outcomes) Meta-analysis not limited by statistically significant heterogeneity 8. No other factors contributing substantial bias Level 2 Level 2 (mid-level) Evidence - representing research results addressing clinical outcomes, and using some method of scientific investigation, but not meeting the quality criteria to achieve level 1 evidence labeling. Level 3 Level 3 (lacking direct) Evidence - representing reports that are not based on scientific analysis of clinical outcomes. Examples include case series, case reports, expert opinion, and conclusions extrapolated indirectly from scientific studies. Grades of Recommendation Guideline producers are more frequently using classification approaches for their evidence and recommendations, and these classifications are recognized and requested by guideline users. When summarizing guideline recommendations for DynaMed users, the DynaMed Editors are more frequently using the guideline-specific classifications and providing the guideline classification approach when this is done. Recommendations without grading systems in the underlying source (or predating this change in editorial policy) may be labeled as one of the following: Grade A recommendation (consistent high-quality evidence) Grade B recommendation (inconsistent or limited evidence) Grade C recommendation (lacking direct evidence) This labeling scheme is formally named the Strength Of Recommendation Taxonomy (SORT) and is described in detail, along with the algorithms used for its application, in Am Fam Physician 2004 Feb 1;69(3): DynaMed User Guide 8 March 2013
9 Searching in DynaMed To perform a basic search in DynaMed: 1. Enter your search term(s). Click the Search button or press the Enter key. DynaMed displays a Result List. The topics in the Result List are ordered by relevance, based on the term you entered. Note that the term(s) might not exist in the topic title. 2. Click a DynaMed title to view the topic. For Example 1. Enter the term (or select from autocomplete list) Abdominal Aortic Aneurysm (AAA) and then click the Search button. 11 DynaMed Result List The DynaMed Result List allows you to preview the sections/subsections of a topic and navigate directly to a section from the result list. To use the DynaMed Result List: 1. Enter your search terms and click Search or press the Enter key. The Result List is displayed. 2. Hover your pointer over a topic title or click the arrow icon to preview the available sections in the column on the right. 3. Click the plus icon to expand a section in the column on the right. 4. 12 Using the Rx, Dx, and Tx Links When browsing DynaMed by category, you can access a specific part of a topic by clicking on the available Rx, Dx, and Tx links found next to the topic title in the result list. Rx Clicking on an Rx link will open the topic with the Dosage and Administration section automatically expanded. Dx Clicking on a Dx link will open the topic with the Diagnosis section automatically expanded. Tx Clicking on a Tx link will open the topic with the Treatment section automatically expanded. 13 Accessing Related Search Results from additional EBSCO Databases Organizations who subscribe to additional EBSCO medical full text databases have the option to provide access to search results from those databases directly from DynaMed. When this option is configured, results related to search terms from those resources will appear in the right column of the result list. To view related search results: 1. Enter your search terms and click Search or press the Enter key. A result list is displayed with related search results from other EBSCO databases in the right column. 2. 15 Using Persistent Links DynaMed offers persistent links to topics, searches, and browse categories. You can use persistent links to share DynaMed topics or searches with your colleagues. Simply copy and paste the persistent link into an , a Web page that you have created, or into an EMR application. To view a persistent link: 1. Click the persistent link icon to the left of the Text Size icons. Note: In a DynaMed Topic, the persistent link icon is available in the tool bar on the right side of the screen. 2. The persistent link to the page (or the search) is displayed in a pop-up box. 3. Click in this field, and the link text is highlighted. Right-click with your mouse and select Copy. 4. To close the persistent link drop-down field, click the X in the right-hand corner. 5. You can paste the persistent link URL into an , a Web page you are creating, or an electronic record. NOTE: Only authorized users will be able to access the article. Appending a Persistent Link Users can append a persistent link to a DynaMed record with code that will link to the record and open a specific section header. To append a DynaMed persistent link: 1. Find a topic in DynaMed. 2. Click on the persistent link icon in the upper right hand corner to display the persistent link to the topic. 3. 17 Browsing Topics Using the A - Z Links To browse topics using the A - Z links: 1. Click a letter in the A-Z links. DynaMed displays a Result List of topics that start with the letter you clicked. 2. Click a DynaMed topic title to view the topic. For Example 1. Click the letter R. DynaMed displays an alphabetical Result List of topics titles that begin with the letter R. The Result List should look similar to the following: 2. Click a topic title to view the DynaMed topic. 18 Browsing DynaMed by Category Name To browse topics by category name: 1. Click the Browse by Category link. 2. Click a category and review the list of subcategories and/or topics. 3. Click either the plus-sign located to the left of a category or the category to view the topics contained in the category. DynaMed displays the sub-categories and topics that belong to the selected category. Note: To close the category list, click the minus-sign icon located to the left of the category title. DynaMed closes the expanded category and returns to the main list of categories. 4. Do one of the following: For example To view a topic, click a topic title. DynaMed displays the topic. To view a sub-category, click either the plus-sign located to the left of a subcategory name or the sub-category title to view the topics and/or sub-categories contained in the selected category. 20 Recent Updates DynaMed lets you view the most recent updates to topics by clicking Recent Updates in the top toolbar. The list includes the updated Topic, an explanation of the Update Reason, and the Date the topic was updated. The most recent update will display at the top and they are arranged in date/time descending order. You can customize your view on this page by selecting a specific category of interest and only display updates from that specialty (i.e. Cardiovascular disorders) and you can also display only those updates that have been determined to be practice changing. 21 DynaMed Topic Page Most DynaMed topics are organized into standard sections to allow easy navigation. These sections vary depending on whether the topic is a disease or drug related. The DynaMed Disease topic sections include: General Information Causes and Risk Factors Complications and Associated Conditions History and Physical Diagnosis Treatment References Prognosis Prevention and Screening Quality Improvement Guidelines and Resources Patient Information ICD-9/ICD-10 Codes The DynaMed AHFS Drug topic sections include: Warnings (if applicable) General Information Uses and Efficacy Dosage and Administration Cautions and Adverse Effects Interactions Mechanism of Action/Pharmacokinetics Stability and Compatibility Preparations Patient Information References Acknowledgements Upon opening a DynaMed Disease topic, all sections are collapsed (not viewable) by default. When opening a DynaMed AHFS topic, the Warning section is expanded (viewable) by default. To view the information in a collapsed section, click any of the following to expand the section: Click the blue triangle image located to the left of the section heading title. Click the hyperlinked section heading title. Click the plus-icon (+) next to a section heading in the column on the left and then click a sub-heading to view the information. 22 To open all the sections simultaneously, use the Expand All link located above the topic. To close all sections simultaneously, use the Collapse All link. You can use the scroll bar, arrow keys, or Page Up / Page Down buttons to navigate the topic page. 23 Search Within Text When viewing a DynaMed topic, you have the ability to search for specific terms using the Search Within Text tool located in the tool bar. To use Search Within Text: 1. On the Topic page, enter a search term in the Search Within Text field and click the magnifying glass icon or press enter. The topic sections are expanded and the first instance of the term is highlighted in green. Additional instances are highlighted in yellow. 2. 24 Creating a Topic Alert DynaMed topic alerts allow you receive notification when a DynaMed topic has been updated. To set up a topic alert in DynaMed: 1. From a DynaMed topic, click the Alert icon in the right column. The alert pop-up box is displayed 2. Select the alert frequency from the How often drop-down menu and enter your address in the field provided. 3. Click Create Alert. 4. Once the alert is created, you will receive an to verify that you set up the alert. (a one step process). Once verified, you ll receive alerts based on the frequency that you requested. 25 Creating an Alert from the Recent Updates page On the Recent Updates page, you can set up alerts to be notified when all topics change or when specific topics within a category change. To set up a topic alert in DynaMed: 1. Use the Limit by Category drop down to select a category default is All. 2. Click the Practice Changing Updates Only box if you would like to only receive updates that are Practice Changing. 3. Once you have the display showing the way you would like to view the topics in your alert, click the Alert icon in the right column. The alert pop-up box is displayed 4. Select the alert frequency from the How often drop-down menu and enter your address in the field provided. 5. Click Create Alert. 6. Once the alert is created, you will receive an to verify that you set up the alert. Once verified, you ll receive alerts based on the frequency that you requested. 26 Printing DynaMed Topic Summaries You can print DynaMed topics directly from the topic page. To print a topic: 1. Access a topic and then click the Print icon in the tool bar on the right of the screen DynaMed displays the topic in the Print Preview screen allowing you to view the content prior to printing. 2. Click the print icon located in the top left section of the screen. The browser Print dialog box appears. 3. Click Print to send the topic to your printer. 4. Access the Topic and then click the icon in the tool bar on the right of the screen DynaMed displays the topic in a new window. 2. Enter your name and your address in the Your Name and Your Address text boxes. 3. Enter the address of the person to whom you want to send the Topic detail in the Send to Address text box. If you are sending to more than one person, separate each address with a semicolon (for example, name1@address.net; name2@address.net). 4. Use the Delivery Format option to select the format in which you want the Topic to display: HTML or Plain Text. 5. Use the Message text box to include a message to the recipient in the . (This is optional.) 6. Click the Send icon located in the top left corner of the screen. DynaMed displays a confirmation message. 7. Click Close to return to the Topic page. 28 Search Other Services The Search Other Services area provides access to medical resources outside of DynaMed. These resources might include other EBSCO services (such as an EBSCOhost profile or the SMART Imagebase), a hospital guidelines site, a hospital formulary site, or any other resources that are beneficial to medical professionals. When you click a link in the Search Other Services area, DynaMed displays the service. To use Search Other Services: 1. Access the DynaMed Home Page. The Search Other Services area is located below the Search box. 2. Review the list of options in the Search Other Services area. Notes: 3. Click a link to access the service. DynaMed opens the link in a new browser window. Links to Search Other Services resources are configured in EBSCOadmin by your DynaMed administrator. When selecting Search Other Services from within a DynaMed topic, you will be able to execute a search for that topic by clicking a link to one of the listed services. If there are no additional services to display, DynaMed does not display the Search Other Services area on the Home Page. Up to twenty links can be displayed in the Search Other Services section. 29 EBSCOhost Full Text links in DynaMed Some DynaMed topics contain links to supporting articles. Clicking these links displays an article abstract or full text via an online medical journal resource. If your institution subscribes to specific EBSCO databases, additional links might appear in a DynaMed topic. EBSCOhost full text links appear when the full text of a cited article is available via an EBSCOhost database to which your institution subscribes. EBSCOhost full text links appear in a DynaMed topic when all of the following are true: The full text of an article cited in DynaMed exists in an EBSCO database accessible by DynaMed. Your institution subscribes to the DynaMed-supported EBSCO database in which the full text resides. Your EBSCOhost interface profile and DynaMed interface profile belong to the same EBSCOadmin group. For more information on EBSCOadmin requirements to use EBSCOhost full text links in DynaMed, see the EBSCOadmin User Guide. Clicking an EBSCOhost Full Text link in DynaMed launches a new browser window displaying the full text of the article according to the following rules: If the Full Text exists in. PDF format only Both PDF and HTML format HTML format only DynaMed displays the article in. PDF format PDF format HTML format Notes: For information on which databases DynaMed accesses for full text content, see the Knowledge Base on the EBSCO Support Site: If you link out to a PubMed citation you can continue to add PubMed LinkOuts (via the PubMed Submission utility) to access linkable EBSCOhost full text and linkable EJS content. PubMed LinkOuts to other non-ebscohost journal content can be added as well. Standard EBSCOhost authentication rules apply. If an EBSCOhost cookie does not exist on the computer in use, you are prompted to supply EBSCOhost login credentials prior to viewing the EBSCOhost full text. Clicking the EBSCO database name link at the top of the EBSCOhost full text record opens a new browser window initializing an EBSCOhost session accessing that database. 30 Continuing Medical Education (CME) Earn CME credit for searches in DynaMed DynaMed users can earn AMA PRA Category 1 Credit, AAFP Prescribed credit and AANP contact hours for using DynaMed to research clinical questions. There are a number of ways in which institutions can provide CME to DynaMed users: CME Basic: This option is for institutions that are accredited CME providers. User searches are submitted via DynaMed to the institution s CME coordinator, who issues the credit. CME Plus: This option is also for institutions that are accredited CME providers. User searches are submitted via DynaMed to a specified CME system or form, and the institution issues the credit. CME Complete: User searches are submitted via DynaMed to our CME partner, Tufts Healthcare Institute. Tufts issues the credits and users can log in to access, print and track credits earned. CME for Canadian Users: DynaMed meets the accreditation criteria of The College of Family Physicians of Canada (CFPC) and has been accredited for up to.5 CFPC Mainpro - M1 Credits for each search submitted from DynaMed. DynaMed Weekly Update: Readers of DynaMed Weekly Update can receive.25 AMA PRA Category 1 Credit,.25 AAFP Prescribed credits, or.25 AANP contact hours. 31 DynaMed Calculators DynaMed includes over 500 interactive calculators, equations and decision trees that allow you to enter values in commonly used formulas to obtain numerical data, such as urinary protein excretion estimation or estimate the severity of community acquired pneumonia in a given patient based upon the presence or absence of major risk factors. To access the DynaMed Calculators: 1. From any screen, click Calculators link in the top toolbar of the DynaMed interface. Note: You can click Calculators to view all calculators or select a specific calculator category from the drop-down menu. 2. The DynaMed calculators screen will appear in a new browser window or tab. 32 3. Enter your figures into the selected calculator. A result will automatically be generated as you input your numbers. 4. When finished with the calculator, you can return to the DynaMed interface by closing the calculator window. DynaMed editors monitor a large volume of research information on a daily basis through Systematic Literature Surveillance and frequently find articles that are of significant and timely interest to health care providers. Based on the criteria of choosing "articles most likely to change clinical practice," DMWU provides a brief and concise overview of these articles. Armed with the awareness of clutter and information overload, editors work to keep the copy in DMWU to a minimum while providing sufficient information as well as links to additional information if the reader wants to know more. DynaMed subscribers can take advantage of persistent links to click from an article directly to the topic in DynaMed. 35 Comments to the Editor DynaMed provides an option that allows you to send comments to the DynaMed editorial team. You can send general comments and questions, as well as comments associated with a specific DynaMed topic. To send a general comment to the DynaMed editors: 1. From any DynaMed screen, click the Send Comment link in the upper right of the top toolbar. DynaMed opens a new browser window displaying the Send Comment to Editor form. 2. Complete all the fields on the form and then click the Submit button. Your comments will be sent to the DynaMed editorial team. To send a topic-specific comment to the DynaMed editors: 1. Access a topic and then click the Send Comment to Editor link at the bottom of the column on the left side of the page. DynaMed opens a new browser window displaying the Send Comment to Editor form. The topic name is displayed at the top of the form. 2. Complete all the fields on the form and then click the Submit button. Your comments will be sent to the DynaMed editorial team. 36 Branding in DynaMed (for administrators) When users are logged into DynaMed, the account name appears in the upper right-hand corner of the screen. Administrators can modify the institution's name or display a logo. Administrators can also choose to have the library name or logo hyperlinked. This allows users to go to the library web page when they exit DynaMed. To display your institution's name or logo in DynaMed: 1. Log in to EBSCOadmin at 2. Click the Customize Services tab. 3. Select your DynaMed profile from the Choose Profiles drop-down menu. 4. Choose the Branding sub-tab. 5. Display Top Right Branding image (enter URL) - Enter the URL to your library's home page image or logo. Note: The recommended maximum size is Width 180 pixels, Height 70 pixels -OR- Display Top Right Branding Text - Enter the desired text to appear in the branding area of DynaMed. 6. Top Right Branding HyperLink (enter URL) - Enter the desired web address to the site you would like users to be directed to when clicking the branding text or image. 7. Click Submit.
