1 MiSiS My Integrated Student Information System CREATIING A COUNSELIING AND//OR DIISCIIPLIINE REFERRAL This job aid is designed to provide instruction in the process to Create a Counseling and/or Discipline Referral. JJOB AID 99/ /2233/ /1144 Users at all school levels with various roles including, but not limited to Principal, Discipline Designee(S), Counselor and Office Manager will be able to perform the tasks outlined in this document. This job aid was created using the Discipline Designee (S) role at a middle school. Step 1 Log into MiSiS with the following URL: from your internet browser, using your single sign-on (SSO) user ID and password. 1 sample.name Step 2 Select the correct user role from the landing page, as required [Example: Discipline Designee (S)]. 2 Page 1

Step 4 Click the search icon. 4 3 To Enter A New Referral: Users can access the Student Support module multiple ways. Step 5 Option 1 Student s First Name link: a. Click the student s first name link. 5a b. At the student s profile page, hover the mouse over Support on the module selection bar. c. Click the Referrals option from the list. Option 2 Student s Action button : ANY LAUSD MIDDLE SCHOOL 5b, c 1234 ANY LAUSD MIDDLE SCHOOL MRS. VARGAS a. At the search results window, use the horizontal scroll bar at the bottom of the screen to scroll to the right. 5b b. Click the Action button for the desired student. 5a Page 2

Your list view may differ from this picture, but the step is the same.) 5c ** The Student Search Results and Student Profile screens display with the Support option highlighted. To collapse the results window and view only the Student Profile screen,. Step 6 Click the collapse button. 6 Step 7 Click the Add new record button. 7 Page 3

Step 8 Use the vertical scroll bar to view the full input screen. 8 ** All required field are represented by the Red Asterisk. ** The Date and Time fields are defaulted to the current date and time according to the computer settings. These fields are required and should represent the date and time the incident occurred. Users can click and type over the default data or the icons can be used. The next few steps will demonstrate using the calendar and clock icons. Step 9 To change select a date from the calendar, click the Calendar icon Step 10 Click the desired date option on the calendar. (For this example was selected.) Step 11 To change the time, click the Clock icon. 11 Step 12 Click the desired option from the list. (For this example, 10:00 AM was selected.) Page 4

To add minutes after selecting an available option Step 13 Double-click the minute field (00) and type the desired minutes ** The Follow-up Required/Resolved fields are not required, but should be used to track follow-up information. Step 14 Click the Location/Context field drop down list arrow. Step 15 Click the desired option on the list. (For this example, Classroom was selected.) 15 ** The Referred By: field default to you, the logged in user. To select someone else Step 16 Click the Other radio button if the student was referred by a non-staff member. Step 17 Type the name and title of the person who referred the incident to you Page 5

6 Creating a Referral (Admin Portal) September 23, 2014 rev. Step 18 To select who will receive this referral electronically, click the Referred To: field drop down list arrow. Step 19 Click the desired name from the list. 19 ( ) 18 ** The Possible Motivation field is optional, and may not be applicable to Counseling referrals. Step 20 For Discipline Referrals, click the Possible Motivation: field dropdown list arrow. Step 21 Click the checkbox(es) for the desired option. (For this example, Seek Attention was selected.) 21 10:15 AM Amber Jones, Teacher s Aid ACOSTA, SOLEDAD ( ) Step 22 Click in the Incident Description: (Max 500) field and type a detailed description of what occurred. Type Incident Description here ** If other comments, besides the description of the incident are needed, click in the Comments/Notes (Max 500) field and type any text to be documented along with this particular referral submittal. 7 September rev. Creating a Referral (Admin Portal) Step 23 Click the Prior Interventions dropdown list arrow. All previously entered interventions will display. Clicking the checkbox(es) will indicate those that apply to the referral. (For this example, no options were selected.) 10:15 AM Amber Jones, Teacher s Aid ACOSTA, SOLEDAD ( ) Type Incident Description here 23 ** Student Support Referrals may be entered for either a Counseling -or- a Discipline reason, but not both at the same time. Selecting a Referral Reason: Some parent Referral Reasons listed will have child list options available. MiSiS will automatically display the lists in an expanded view. Counseling if the desired option has a parent to child relationship, click the checkbox for the child first (ex. Grade average is going down), then select the parent (ex. Academic Concerns). Discipline if the desired option has a parent to child relationship, clicking the child first (ex. Alcohol) will trigger the system to automatically select the parent (Substitute of a controlled substance). For Counseling Referral Reasons, follow steps 24c and 25c examples. Step 24c Click the Grade average is going down checkbox under the Counseling: area first. Step 25c Click the Academic Concerns checkbox. 24c 25c Page 7
