If you have specific questions about how to perform a function of your responsibilities, speak with your manager. If you have a specific question about how to maneuver through the system that is not outlined in this user guide, contact: Kari Allen at the Ascend Iowa PASRR Help Desk ext IMPORTANT SYSTEM REQUIREMENTS: Ascend will always support the current and most recent versions of Internet Explorer and Mozilla Firefox. 3 Log In Visit Click Log In to access the login screen (Fig. 2) Enter Username and Password. Click Login. Ascend Management Innovations Home Page Figure 1 If you forget your password or it does not work, click the Forgot Password? link. The system will prompt you to change your password for access. You will not need to contact Ascend to assist in this process. Click My Projects. The system will bring you back to the Ascend Management Innovations Home page (Fig. 1). The Log In link becomes the My Projects menu (Fig. 3). Select Iowa Provider to open the Iowa PASRR home page (Fig. 4). Queue Options Click to add a new resident Click to submit a corrected admission notice Queues categorize and prioritize work to assist with workflow. Review Lists the names of individuals the referral source indicated are discharged to your NF Accepted Lists the names of individuals confirmed as residents in your NF Click to access the identified Queue. 2013, 2014 ASCEND MANAGEMENT INNOVATIONS LLC. 4 Queues From the Menu Bar (Fig. 7). IOWA PASRR Review Queue Lists the individuals a referral source indicated have discharged to your facility. Accept or reject the record for your census. Hover over Queues in the menu bar. Select Review to open the Review Queue (Fig. 17). Select Yes or No to indicate if the person is in your facility. If you select No, click Submit and the person will not appear in your census and will no longer be in the Review queue. If you select Yes, complete the SS# and click Submit. The person will appear in your census, available in the Accepted queue. Menu Bar Figure 8 Review Queue Figure 9 Accepted Queue Lists the individuals confirmed as residents of your facility. Hover over Queues in the menu bar. Select Accepted to open the Accepted Queue (Fig. 19). Click View to open the individual s record (Fig. 20). 5 Submit a New Level I, report tracking, and review PASRR activity for the individual. Individual Record Figure 12 Status Change Assessment Submit a status change assessment anytime an individual has a significant change in status, based on MDS criteria. From the Individual Record (Fig. 21) Click Create New Level I to access the PASRR Pro I system. Individual Record Figure 13 Complete the Level I screen. Ascend will contact you via the system with a determination, including any referral for a Level II assessment. 6 Enter the Admission Date and Admitted From location. Enter the information about the location from which the person admitted. Select a discharging facility from the dropdown, if appropriate. The system will automatically fill in any facility information. Indicate the expected length of stay. Enter your phone number. Click Submit to enter the admissions tracking. Admission Tracking Figure 15 Report New Discharge/Transfer Notice Report discharges and transfers from your facility. Access from the individual record. Click Report New Discharge/Transfer Notice to submit a tracking report. Enter the Admission Date, the Discharge/ Transfer/Deceased Date, and the disposition location and address. 7 Reviewing Submitted Tracking Access previously submitted tracking from the Individual s record Click View/Print to open a previously submitted tracking notice. The notice will open in a new window or tab. Close the new window/tab when you have finished reviewing or printing the notice. Individual Record Figure 18 Submit Corrected Notice Submit corrections to previously submitted tracking forms with errors. From the Queues menu (Fig. 7). Click Submit Corrected Notice from the menu bar to open the corrections notice page (Fig. 13). Enter the individual s last name, first name, or SSN. Click Search to run to search feature. 8 Update the form with the correct information. Click Submit to finalize. 9 Create & View Uploads Upload documents for Ascend access/review From the Individual Record Click Create Upload to open the document uploader (Fig. 30). Click Browse to select the document from your computer. Name the attachment. Indicate No for Is this attachment the Level 2. Individual Record Figure 25 To upload a document, it must be.pdf, or.doc. The system will reject any alternate format. Click Upload File to upload the document to Ascend. Click View to review uploaded information. 10 Print PASRR Outcome To obtain a copy of the PASRR outcome From the Individual Record Click Print Outcome to access the PASRR notification and screen. The Outcome will open in a new window or tab. Close the new window/tab when you have finished reviewing or printing the outcome. Individual Record Figure 28 Add Individual To report an individual who admitted to your facility From the Navigation menu Click Add Individual in the menu bar to open the SSN search option (Fig. 9). 11 Review the information to confirm the individual matches the record. Click Yes to add the individual to your NF census. Click Submit to confirm. IOWA PASRR If the Social Security Number search does not locate the individual, submit a Level I screen to Ascend. Add Individual Page Figure 31 Note: To protect the individual s information, only partial information is available until you confirm the resident is in your facility. Click OK to proceed. Add Individual Page Figure 32 Log Out Click Log out to end your session. Always Log out before closing your browser. Failure to do so can result in locked records.
