CHW Profile MiCHWA s CHW Registry serves as a statewide database of all Community Health Workers (CHWs) in Michigan. It also serves as the mechanism through which MiCHWA certifies CHWs throughout the state. All CHWs seeking CHW certification through MiCHWA are required to join the registry in order to verify successful completion of the MiCHWA CHW Training Program or complete the grandparenting application process. CHWs not seeking CHW certification through MiCHWA are also welcome to join and gain access to the registry. All registry profiles are private and protected by password. Information can only be made viewable to other registry participants if the CHW or employer chooses to make their profile viewable. Profiles will not be viewable to people outside the registry. Step 1. Creating an account Go to and review materials available regarding MiCHWA CHW Certification, Registry Benefits, and CHW Registry Payments. Click the link to register or log in on the MiCHWA CHW Registry and you will be redirected to the MiCHWA CHW Registry Login Page ( 1
2 On the MiCHWA CHW Registry Login Page, click create new account under User Login and provide a Username and address. To submit your request, click the green button create new account. You will receive an automated message from the MiCHWA CHW Registry that your request was successfully submitted and there will be a link to Eventbrite to complete your payment. 2
3 Step 2. Payment Process After clicking the green Create new account button, you will receive an automated follow-up with a link to Eventbrite to complete the $30 payment for the MiCHWA CHW Registry. Once you have submitted the initial $30 payment, through Eventbrite, MiCHWA will verify the payment and provide access to the CHW Registry. Please use the same address for registration and the Eventbrite payment so that MiCHWA staff can match the payment with the new account. Also, please maintain your receipt from Eventbrite until you receive a verification from MiCHWA. The verification will provide your one-time, temporary password to gain access to your CHW Registry Account. The renewal date for CHWs is based on the date of registration. The renewal payment must be completed annually to maintain access to the MiCHWA CHW Registry, even though MiCHWA Certification renewal is every two (2) years. The CHW will receive a reminder 30-days prior to the renewal date. There are two dates for MiCHWA CHW Registry membership renewal, February 1st and August 1st. Please see the chart available here for details: MiCHWA Registry Renewal Table Year If you register between February 1st and July 31st then your annual MiCHWA CHW Registry fee will be due August 1st of the following year. If you register between August 1st and January 31st then your annual MiCHWA CHW Registry fee will be due February 1st of the following year. Please note that MiCHWA is using Eventbrite through the University of Michigan. This is the most secure payment method available to ensure that your information is protected. Step 3. CHW Dashboard Once your payment has been verified and your account is approved, you are able to log in on the MiCHWA CHW Registry Login Page and enter your Username and temporary password. Once logged in for the first time, please be sure to change the temporary password immediately as it is only available for one-time login. If you forget to change the password or you forget the password you created, you can request a new password on the MiCHWA CHW Registry Login Page. You will first see your CHW Dashboard. In the left column, the CHW Dashboard contains links to essential pages such as Certification, where you will find more information 3
4 about MiCHWA CHW Certification, grandparenting, MiCHWA CHW Certificate verification, and necessary applications. There are also helpful documents here to get you started with setting up your CHW Profile (see Getting Started in the right column). The top navigation bar, is visible on each MiCHWA CHW Registry page (see green box). You will need to return to the home page to view the links available on the Dashboard (see blue box). You may also click the MiCHWA logo at the top of the page to return to the Dashboard at any time (see red box). To edit your CHW Profile go to the Dashboard links or the navigation bar and choose view my profile. Step. 4. Creating your CHW Profile 4
5 When you first go to view your CHW Profile, it will look pretty bare as content needs to be added here! First go to the edit tab above your name and complete all fields in this section. This is also where you will change your password after your first-time logging in. On the edit page you can also hide your profile if you click the box under Last Name. If you hide your profile, no other registry users (i.e., Employers, Instructors, CHWs) will be able to search for or view your profile. Please do not de-select the box for Administrative Overlay. Select View Profile and complete information for Employment, Education, Volunteer Work, MiCHWA Training, Additional Training, Other Certifications. To add information here, please select the Add link in the bottom right corner of each profile box. You can also edit and delete items from your profile by using the Delete and Edit links on th bottom right corner of each profile box. 5
6 Step 5. Applying for CHW Certification Return to your Dashboard by clicking the MiCHWA Logo at the top of the page or clicking Home above your profile picture. In the left column of the Dashboard, select Certification and you will be redirected to the MiCHWA Certification Page. Please read through the MiCHWA Certification Page carefully as this page outlines the processes for MiCHWA CHW Training Certificate Verification and Grandparenting. It also describes MiCHWA CHW Certification Renewal requirements and tracking. On this page, CHWs will also find the schedule for renewal payments and certification renewal. The MiCHWA Certification Page has a link to the MiCHWA CHW Training Certificate verification application so that MiCHWA staff can verify that a CHW successfully completed the MiCHWA CHW Training Program. MiCHWA staff will then provide verification that the CHW is MiCHWA Certified on the CHW s profile. The MiCHWA Certification Page also has a link to the Grandparenting Application. If a CHW has not completed a MiCHWA CHW Training Program, but meets requirements for grandparenting then the CHW may complete the grandparenting application via the registry and upload supporting documents to the application. Once the grandparenting application is submitted, the CHW will receive an notification with a link for 6
7 payment of the one-time, non-refundable $50 processing fee. Once payment is verified, MiCHWA will review the grandparenting application and send a follow-up to the CHW regarding the CHW s grandparenting status. 7
8 Step 6. Recording CEUs on the CHW Registry Once MiCHWA Certification status is verified and approved then the CHW may use the My Training Page to record Continuing Education Units (CEUs). MiCHWA also recommends keeping paper copies of CEU documents (syllabus, agendas, certificates, supervisor letters, etc.) in case you are audited (see MiCHWA Certification Page for details about CEUs and renewals). MiCHWA Certified CHWs must complete 20 CEUs every two (2) years. The My Training Page provides a useful tool for keeping track of CEUs and related documents. CHWs can also search for CEUs that were completed in the past. Step 7. Search for CHW Trainings On the Search Trainings Page, CHWs can search for Continuing Education (CE) activities and opportunities available by city or region. All MiCHWA Certified CHWs will need to complete 20 Continuing Education Units (CEUs) every two (2) years. More information can be found about MiCHWA Certification Renewal and CEUs on the MiCHWA Certification Page (see Step 5). These may include CHW training offered by MiCHWA as well as other partnering organizations. Select the name of the training to see more information about the training and how to register with the organization offering the CHW training opportunity. 8
9 Step 8. Search for Job Postings and CHW Employer Profiles By selecting Search Job Postings in the navigation bar, CHWs will be able to view CHW job postings from Employers on the CHW Registry and search for CHW job postings by region. Employers must have a profile on the CHW Registry in order to make a job posting. To view Employer profiles in your region, you can return to the Dashboard page (click home or click the MiCHWA logo at the top of the page) and select the Search Employers link. To learn more about the job opportunity and how to apply, select the name of the job posting or click to the link to the job posting. 9
10 Please note that by clicking the MiCHWA logo at the top of each page you will be redirected to the Dashboard page. Selecting the Home link will also bring you to the Dashboard page. To view the Employer Profiles, click the name in the link column. 10

Search Training Partners If you want to learn more about MiCHWA partnering organizations and agencies that offer the MiCHWA CHW Training Program, you can return to the Dashboard and select the Search Training Partners link. Employer Profile The CHW Employer profile is a useful tool for CHW Employers to search for MiCHWA Certified CHWs by region/location, post job opportunities for CHWs, and search for CHW training opportunities for staff. CHW Employer profiles are also a great way to network with CHWs and CHW Employers around the state. By joining the MiCHWA CHW Registry, CHW Employers are supporting MiCHWA s mission to promote and sustain the integration of community health workers (CHWs) into health and human services organizations throughout Michigan through coordinated changes in policy and workforce development. Step 1. Employers have the option of paying for a CHW Employer Profile only or choosing a package that includes CHW Profiles for CHW staff. Please see details here: Registry Membership Fees. 13

If the Employer is purchasing a package, they will need to provide names, s, and addresses of the CHW staff that will need access to the MiCHWA CHW Registry. This information should be provided on Eventbrite or ed to info@michwa.org. MiCHWA will set up accounts for the CHW staff identified and these CHWs will not need to go through the steps for setting up their own accounts. The CHWs will receive an with their username and temporary, one-time password when their account is activated. Please maintain your receipt from Eventbrite until you receive a verification from MiCHWA. The verification will provide your one-time, temporary password to gain access to your CHW Registry Account. Please note that a renewal fee is due each year to maintain access to the Employer profile. The renewal date for employers is based on the date of registration. To edit your Employer Profile, go to the Dashboard links or the navigation bar and choose View My Profile. Step. 4. Creating your Employer Profile To add content to your Employer Profile, first go to the edit tab above your name and complete all fields in this section. This is also where you will change your password after your first-time logging in. On the edit page, you can also hide your profile if you click the box under Time Zone. If you hide your profile, no other registry users (i.e., Employers, Instructors, CHWs) will be able to search for or view your profile. Please do not de-select the box for Administrative Overlay. Complete all fields on the Edit Page to ensure that the Employer Profile has a professional look and feel. You may also upload your organization s logo on the Edit Page. 16
17 Step 5. Add and View Job Postings By selecting Add Job Posting in the Navigation Bar, Employers will be able to post CHW job openings at their organization. CHWs on the registry will be able to search for these opportunities in their regions and employers will be able to view the CHW profiles of applicants. 17
18 Here Employers can designate dates when you want the posting to be viewable on the CHW Registry. Employers will also be able to provide the job description and a link to apply. Please note that the job listing will not be viewable until the start date entered for the job posting. The job posting will be automatically removed after the given end date. However, Employers can edit the job posting during that time by searching for their job posting and viewing it on the registry. 18
19 To view the posting once it is live, go to Search Job Postings and search by region. Click the name of the job posting and you will be able to view and edit the job description accordingly. 19
20 Step 6. Search CHWs By selecting Search CHWs in the navigation bar, Employers will be able to view CHWs in your region and location. Here you can view the CHW s experience, education/training, and if they are MiCHWA Certified. MiCHWA Certified CHWs have either completed the MiCHWA CHW Training Program OR qualified for grandparenting. To view the CHW s profile, click the name of the CHW. If the CHW has completed the MiCHWA CHW Training Program and has opted to have their MiCHWA Certificate of Completion available on their profile, employers may click MiCHWA Certified below the name of the CHW to view the certificate. 20

Search MiCHWA-trained CHW Instructors and Training Partners To learn more about MiCHWA partnering organizations and agencies that offer the MiCHWA CHW Training Program for MiCHWA Certificate, select the Search Training Partners from the Navigation Bar or Dashboard. Here Employers may view profiles of MiCHWA-trained CHW Instructors and their organizations/agencies by region. The Training Partner profile will show the MiCHWA-trained instructors background and experience in offer the MiCHWA CHW Training Program. It will also have more information about their employer/organization. All Training Partners listed on the registry have MiCHWA-trained instructors that are certified by MiCHWA to offer the MiCHWA CHW Training Program. Training Partners are also able to post other CHW training opportunities available through their organization. These opportunities have CEUs available for MiCHWA-Certified CHWs needing to complete their 20 CEUs every two [2] years. 22
23 Step 8: Search Training Opportunities for CHWs MiCHWA and Training Partners are able to post additional training opportunities for CHWs via the CHW Registry. MiCHWA will continue to post opportunities for the MiCHWA CHW Training Program to the MiCHWA website ( as well as the MiCHWA CHW Registry. As mentioned in Step 7, Training Partners may also provide information about Continuing Education (CE) and training opportunities for CHWs via the MiCHWA CHW registry. Select Search Trainings from the Navigation Bar to search for CHW training and CE opportunities by region. Select the name of the training to view further details. 23

25 Step 9. Search CHW Employers The MiCHWA CHW Registry is also a great way to network with other CHW Employers in your area. To search for CHW Employers by region/location, return to the Dashboard Page by clicking the MiCHWA logo above the Navigation Bar or clicking Home on any CHW Registry page. On the Dashboard Page, choose the Search Employers link in the left column. The other Dashboard links are also available in the Navigation Bar. On the Search Employers page, Employers may click the link to view profiles of other CHW Employers in Michigan and search by region/location. Employer Profiles will look much like your organization s profile. 25

27 III. Training Partner Profile The Training Partner Profile is for MiCHWA-trained Instructors and their organizations/employers offering the MiCHWA CHW Training Program. The Training Partner Profile is a useful tool to promote CHW Training offered by the Training Partner to CHWs and CHW Employers throughout Michigan. The MiCHWA CHW Registry is also useful for networking with CHWs, CHW Employers, and other Training Partners by region/location. Please note that MiCHWA-trained Instructors that are CHWs must also have a CHW profile for MiCHWA CHW Certification. Step 1. You may also upload your organization s logo or a professional photo of the MiCHWA-trained Instructor on the Edit Page. Select View to return to the Training Partner Profile. To add information about the Instructor s educational background, Upcoming MiCHWA CHW Training, and Previous MiCHWA CHW Training, select the Add button below each profile box and edit these sections accordingly. These sections may also be edited or deleted using buttons available in each profile box. 31
32 Step 5. Add and View Training Opportunities By selecting Add Training Opportunity in the Navigation Bar, you will be able to post CHW training opportunities at your organization. CHWs seeking additional training and Continuing Education Unites (CEUs) on the registry will be able to search for these opportunities in their regions/locations. CHW Employers are also able to search for these opportunities for their CHWs. Please note that MiCHWA will be posting MiCHWA 32
33 CHW Training Program Opportunities here and on the MiCHWA website ( Training Partners are asked to only post additional CHW training or Continuing Education opportunities available to CHWs through their Instructor and/or organization. Here Training Partners may describe the training opportunity available and registration details. Training Partners may designate dates (start date/end date) when posting should be viewable on the CHW Registry. Please note that the training opportunity will not be viewable until the start date entered for the posting. The training opportunity will be automatically removed after the given end date. However, Training Partners may edit the posting during that time by searching for the training opportunity and viewing it on the registry. Training Partners are encouraged to provide the number of CEUs available for CHWs. One [1] hour of training (not including breaks) is equivalent to one [1] CEU. A training opportunity may not be less than 30 minutes (0.5 CEUs). 33
34 To view your posting once it is live, go to Search Training Opportunities and enter your region. Click the name of the job posting and you will be able to view and edit the job description accordingly. 34
35 Step 6. Search CHWs By selecting Search CHWs in the navigation bar, Training Partners will be able to view CHWs in their region and location. On each CHW profile, Training Partners may view the CHW s experience, education/training, and if they are MiCHWA Certified. MiCHWA Certified CHWs have either completed the MiCHWA CHW Training Program OR qualified for grandparenting. To view the CHW s profile, click the name of the CHW. If the CHW has completed the MiCHWA CHW Training Program and has opted to have their MiCHWA Certificate of Completion available on their profile, Training Partners may click MiCHWA Certified below the name of the CHW to view the certificate. 35

37 Step 7. Search CHW Employers The MiCHWA CHW Registry is also a great way to network with CHW Employers throughout Michigan or in a particular region/location. To search for CHW Employers in a region/location, select Search Employers in the Navigation Bar. Click the link for the Employer Profile you would like to view. The profile will describe the Employer, how many CHWs they have, and what roles they fill. 37

Search MiCHWA-trained CHW Instructors and Training Partners To learn more about other MiCHWA partnering organizations and agencies that offer the MiCHWA CHW Training Program for MiCHWA Certificate, select the Search Training Partners from the Dashboard. Training Partners may view profiles of MiCHWA-trained CHW Instructors and their organizations/agencies by region. To first access your Dashboard, select Home or click the MiCHWA logo at the top of any page. 38
39 Step 9. Search CHW Job Postings Training Partners may also search CHW Job Postings by region. This may be helpful to see if there may be upcoming need for a MiCHWA CHW Training in that region. Only CHW Employers on the MiCHWA CHW Registry are able to post CHW job opportunities via the registry. To search for CHW Job Postings select the Search Job Postings link on your Dashboard.
