2 Introduction ARDMS Semi-Interactive Console (SIC) items are designed to simulate the controls on an actual ultrasound system Each item represents a partially completed ultrasound examination. Candidates are required to assess the current system settings, evaluate the existing image(s), and adjust the console settings as needed to achieve the goal specified in the Problem Statement. Candidates can adjust as many or as few active controls as needed. System Conditions The current settings for all system controls are indicated in the simulated console interactive controls area. Controls may be Selectable or Not Selectable. Selectable controls can be On or Off. Controls must be Selectable and On to be changed. The Freeze control is set to the On position in all SIC problems. Therefore, image(s) will be displayed in a static state and will not change as the active control settings are adjusted. The M-Mode and Tools controls are currently in the Not Selectable state and are reserved for later use. The Console Layout The simulated sonogram console is arranged into the following areas: Area 1 This area will contain a static 2D image. Area 2 This area may contain a static Doppler waveform image (not all problems will have an image in this area). Area 3 This area contains the problem statement and the issue to be addressed or the objective to be achieved in the problem. Area 4 This area contains the interactive controls; the simulated controls are manipulated to achieve the objective in the problem statement. Candidates respond to the problem by manipulating the Active controls in this area. Color Bar Area This area may contain a color bar (if appropriate for the system mode). Color bars are presented in RABT format Red (Bottom) Away, Blue (Top) Towards, or BART Blue (Bottom) Away, Red (Top) Towards. Exam Controls This area contains the standard Pearson VUE examination controls plus a Reset control that allows candidates to return the problem to its original state. Since any changes made before clicking the reset button will be lost, the system includes a pop-up box asking candidates to confirm that they wish to reset the problem to its original state. Standard Pearson VUE Test Information This area contains the test name, timing, and other test information typically found on items in a Pearson VUE-administered test. Interactive Controls Area The interactive controls area is divided into four areas: Page 2
3 Primary Controls These controls establish the operational mode for the sonogram console and provide access to system secondary controls. Secondary Controls The controls in this area vary depending on the operational mode selected. Secondary controls are selected for manipulation in this area. Transducer and Select Control Value This area contains a drop-down box to select the transducer and transmit frequency. Also, when a secondary control is selected, a graphic slider appears in this area. The slider is used to change the setting for the selected secondary control. Time Gain Compensation (TGC) Controls This area contains five sliders for the TGC controls. The specific content of each subarea is as follows: Primary Controls These controls establish the operational mode for the sonogram console and provide access to Freeze and system Tools controls. The settings available in this area are: Control States PW Pulsed Wave Doppler CW Continuous Wave Doppler Color Color Doppler 2D 2 Dimensional - Gray Scale mode M-Mode Motion Mode Other Primary Controls Not Selectable Freeze Displays the image(s) as static images rather than real-time images. (For SIC items, all images are preset in freeze mode and cannot be changed.) Tools Provides access to additional system controls. Controls that are Not Selectable cannot be changed by the candidate. The control name is in gray font, outlined in a white box with no graphic On/Off switch. Not Selectable/On The Freeze control is in the Not Selectable/On state and cannot be changed by candidates. Selectable/Off Selectable controls can be manipulated by candidates. The control name is in white font, outlined in a white box with a graphic On/Off switch in the Off position. Candidates turn a control On by clicking the left mouse button on the control title or on the On/Off switch. To turn the control Off, candidates either click the left mouse button on the control a second time or click on a different control. Selectable/On Selectable On controls are identified with yellow font, outlined in a yellow box with graphic On/Off switch in the On position. Only one control in the Primary Controls area may be in this state at one time. Page 3
4 When a Primary Control is in the Selected/On state, the secondary controls that relate to this mode are displayed in the Secondary Controls Area. Secondary Controls Area The Secondary Controls Area displays the active operational mode and the secondary controls that are available in that operational mode. The Secondary Controls Area has a yellow font and is outlined in a yellow box. The secondary controls vary based on the primary control mode selected. The complete list of secondary controls is shown in the Glossary of Terms. The Secondary Controls Area serves two purposes it informs candidates of the current settings for the sonogram controls, and it allows candidates to select secondary controls to change. The Secondary Controls Area contains the name of the control, the current setting, and the units of the current setting when appropriate (e.g., cm, cm/sec, %, etc.). For most operational modes, there are too many secondary controls to fit all of the controls on the screen at once. This control area has a scroll bar to allow candidates to scroll down to see all secondary controls for the operational mode. When a secondary control is in the Selected/On state, a slider control that relates to the control appears in the Select Control Value area. Select Control Value Area When a secondary control is in the Selected/On state, a slider appears in the Select Control Value Area. The title of the control and the current value setting appear above the slider. The value of the control can be changed by placing the mouse cursor on the slider button, holding down the left mouse button, and moving the button to the left or right. Alternatively, the value can be changed by clicking the left mouse button on the number of the value to be selected. The available values appear above the tick marks on the slider. These are the only values available for the selected secondary control. The slider cannot be placed between tick marks. A control may have two to five values The selected value is highlighted in a yellow box above the slider. The selected value also appears in the Secondary Control Area next to the name of the selected control. The Select Control Value slider has only an On and an Off state. When a secondary control is selected, the slider appears in the On state. The title of the secondary control is in yellow font and highlighted in a yellow box. The current value is also highlighted in yellow font and in a yellow box. If no secondary control is selected (i.e., the Off position), nothing is shown in the Select Control Value Area. Transducer Control Area The Transducer Control Area allows candidates to see and, if active, select the type and/or the frequency of the transducer being used to generate the image. There are frequency values tied to a specific transducer type. To change the transducer type, candidates must click on one of the values in the drop-down box for Type and one of the values in the drop-down box for Frequency. Page 4
5 The Frequency value is also set in the same manner; however, it is possible to change the frequency value without changing the transducer type. TGC Controls Area The TGC Controls Area displays the TGC sliders. The control contains five sliders with five potential settings for each slider. The value of the current setting for each slider is shown to the right of that control. Candidates change the TGC slider values by clicking and holding down the left mouse button on the slider and moving the slider to the left or right. As the slider moves, the new setting value appears in the yellow box to the right of the slider. Only one slider may be selected at one time. Special Functionality for Baseline and Velocity Scale Controls for Color and PW Doppler The Baseline and Velocity Scale controls are interrelated. Changing the baseline position changes the possible settings for lower and upper velocity scale values. There are three possible baseline positions High, Center, and Low. For each of these positions, the possible velocity scale settings are as follows: Color Doppler Baseline Velocity Scale High upper/lower 3/-9 8/-24 14/-42 22/-66 30/-90 Center 6/-6 16/-16 28/-28 44/-44 60/-60 Low 9/-3 24/-8 42/-14 66/-22 90/-30 PW Doppler Baseline Velocity Scale upper/lower High 8/-32 16/-64 32/ / /-512 Center 20/-20 40/-40 80/ / /-320 Low 32/-8 64/ / / /-128 Given this relationship, candidates will change the baseline velocity scale in the same interaction as follows: Candidates can change the baseline by left mouse clicking on one of the words High, Center, or Low or by clicking and holding the left mouse button down on the yellow arrow or the yellow dotted line and dragging the arrow or line up or down. For each baseline setting, a different set of possible values appears in the Velocity Scale Area. Candidates select one of the five settings by clicking on one of the numbers in the set or the row in which the desired values are shown. The selected values are highlighted in white font on a blue background with a yellow box around both values. Candidates must select sets of values and cannot select values for lower and upper velocity scales that are not in the same row. Page 5
6 On/Off Combinations It is possible for the baseline and the velocity scales to be independently On or Off as follows: Both upper and lower velocity scales must be in the same condition On or Off. If baseline is On, both upper and lower velocity scales must also be On. (This is because a change in baseline must be accompanied by a new selection of velocity scales.) If baseline is Off, both upper and lower velocity scales can be On or Off. If velocity scales are On, the only values that can be selected for the upper and lower velocity scales are those that are associated with the given baseline setting. For example, if the baseline is set at Middle, only the velocity scale values associated with the middle baseline setting are displayed. (See table on page 5 for acceptable combinations for each baseline setting.) Invert Control The Invert Control is found in the Secondary Controls Area and has only On and Off settings. These settings impact the appearance of the color bar by changing the locations of the two portions of the color bar. With Invert = off the red/yellow portion is on the top, and the blue/green portion is on the bottom. If Invert= on the color bar is flipped, and the blue/green portion of the color bar is on the top.
