We are requesting that you keep a credit card on file with us. If you do not provide a credit card then we will charge a 5% handling fee to process checks. By signing the credit card agreement, you agree to follow our rules along with Mandt s rules while on their property. Once we receive your signed application we can schedule a time to meet out at the site. This first meeting is required, after that you are free to come and go as you please between the hours of 7am-4:30pm Monday through Friday. The next step for you is to print out the credit application form, fill it out completely, and return it to Log Gone It either by mail or . 3 Contractor agreement for Log Gone It transfer site The contractor agrees to follow all dumping rules as laid out in this agreement. All loads are charged by the ton. All loads are subject to a 1-ton minimum charge 1. Log Gone It is located within Mandt Sandfill. The address is 2079 Hwy MM Fitchburg WI, Only the following material is accepted at this site, no exceptions. All loads must be dumped in their designated pile. Anything other than the following will be removed at the violators expense and dumping privileges may be revoked. -Chips (from trees only all species) -Wood (trunks, and limbs) from trees only and all species and all sizes -Brush -Stump Grindings -Root Balls -Mix Load containing root balls -Mix Load not containing root balls 3. All prices are subject to change and you will be notified via or phone call 4. Every load brought to the site will need to have a load ticket filled out. 1 copy is for the driver and leave 3 in the scale box. Fill out the form completely. Failure to leave completed tickets behind will jeopardize your dumping privileges. 5. When entering Mandt Sandfill all speed limits and Mandt s rules to be on their property will be obeyed and only drive to the Log Gone It site. Interfering with Mandt s operation can have your dumping privileges revoked. 6. You will be invoiced weekly and sales tax applies to this service. 7. Once you enter Mandt Sandfill and Log Gone It you will be on camera. 8. We will have the most current price list and any special dumping instructions posted in the Green Log Gone it shack. 9. Each contractor will provide Log Gone It with a current certificate of insurance with 1,000, general liability coverage. Log Gone It LLC must be listed as additionally insured. 10. Our hours of operation are from 7 am 4:30 pm Mon-Friday. NO EXECPTIONS 11. All contractors agree to follow the dumping instructions attached to this contract. 12. We will invoice weekly. 15. A completed and signed credit application means you have read, understood, and agree to follow all Contractor Agreement, and dumping instructions as laid out in this packet. 16. If you do not have a credit card on file with Log Gone It then an additional 5% processing fee will be added to your invoice to process checks. I signed credit applications still must be signed.

4 Log Gone It LLC. Dumping Instructions Hours of operation Monday-Friday 7 AM- 4:30 PM 1. Log Gone it(lgi) is located within Mandt Sandfill. The address is 2079 Hwy MM Fitchburg WI, The property has cameras that cover the entire area. 2. Enter the property using the south pit entrance located directly across from the Sunoco Gas Station. DO NOT USE THE NORTH ENTRANCE NEXT TO HOUSE 3. Leave your chipper or trailer connected to your truck when scaling. Swing wide with trailers. You will be liable for any damages you cause to the scale 4. Drive past the scale on the left-hand side then swing back to your right to enter the scale very slowly 5. Pull up to the LGI scale box so you can reach through your driver window and pull out an empty load ticket 6. Fill out the load ticket completely except for your empty weight (you will get your empty weight after you dump) KEEP YOUR TICKET WITH YOU UNTIL YOU SCALE OUT EMPTY 7. If you make a mistake write VOID on the ticket and place it in the completed ticket box. 8. DO NOT TAKE VOIDED OR BLANK TICKETS WITH YOU, LEAVE THEM. 9. Pull off the scale and turn right to enter the LGI site. There will be signs to follow 10. When you reach the transfer site you can drop your chipper or trailer in the designated area 11. Proceed to the correct material pile that you have brought in. Back up to pile and dump. 12. Proceed to trailer drop area and hook up your trailer. 13. Follow your same path back to scale. 14. Enter the scale very slow and the same way you did when you entered 15. Pull up to the LGI scale box. 16. Write down your empty weight on your load ticket. Make sure it is filled out completely 17. Keep the top copy for yourself and the bottom 3 copies go in the completed box 18. Exit the scale slowly
