1 MPS: Behavioral Referral The Behavior Referral Form allows school staff members to complete an online referral for a student behavior event and submit it to an administrator responsible for managing behavior for further action. To access the behavioral referral, login into Infinite Campus, found in APPS, with your Milwaukee Public Schools username and password When you log into Infinite Campus, you want to be on the Campus Tools section. If you are on the Campus Instruction section, then go to the Application Switcher to select Campus Tools. The Application Switcher will open a popup window, which will allow you to select Campus Tools. Office of Student Services, DPH

2 You will now see that you have switched to Campus Tools. Once you are at Campus Tools, locate and select the Behavior tab on the left side of the page, found under the Index. Creating a Behavior Referral Select the Behavior tab, than select Behavioral Referral. To create a new referral, select New to open a new Behavior Referral and begin to enter information for the incident in the Incident Detail Editor. Office of Student Services, DPH

3 Incident Detailed Information Enter information into each field or drop down. At any point while creating a referral, by clicking Save Draft for the Behavioral Referral or Event/Participant editor, all information will saved in draft status. Leave the Title blank. It will be automatically entered later with the event you choose for the incident Context and Location MUST be filled in for PBIS. (Context Description and Location Description are optional.) In the Incident Detailed Information, only general information about what happened, where it occurred, the time/date that it occurred and the administrator/s are entered. No students should be identified in the Details section. (Ex: Student 1, Student 2, He, She, etc.) This can be seen by parent who wants to view the referral. Specific student information, concerning their responsibility in the incident is entered in Add Event/Participant. To add information concerning the incident and specific student information, select the Add Event/Participant tab at the bottom of the Incident Detailed Information. Office of Student Services, DPH

4 Add Event/Participant The Event Type is the incident or incidents in which the students were involved. There is no limit to the number of events that can be added to an incident. From the Event Type drop down, select the type of event that you believe best describes what occurred. Behavior Response is only checked if a physical restraint was used by a staff member during this incident. Participant(s) Detail Under Add Participant, choose the filter you need to use to search for and add each participant for the incident. A participant can be a student or school employee. 5 To attach a participant to an event, Search for at least the first two characters of the Participant Name or a Student Number who are part of the active enrollment or school calendar. Once each participant has been selected, choose a Role for each participant. A participant s Role can be an Offender, Participant, Victim, or Witness. For each participant, write a detailed statement in the Details section. The only name that should be used in the Details section should be for that specific participant. If a parent/guardian requests to see the referral, only their child s name should appear. To return to Incident Detailed Information click Save Draft. Office of Student Services, DPH

6 Submit Behavioral Referral Before submitting the behavioral referral, be sure that the administrator/s are selected in the Notify list. To select more than one administrator, use control-click or shift-click, and choose the administrators you want the behavior referral to be sent to. Note: Be sure to review the Events and Participants section. Is the event the one you chose? Did you attach students to the Behavior Referral? Are their roles correct? For bullying incidents, at least one offender and one victim must be attached. Printing/Saving a Copy of a Referral If the staff member wants to have a copy of their referral, there are two work arounds that will allows them to make a copy before they submit the referral, by taking a screen shot or printing a copy of the referral It is important to note, that if a staff member submits the Behavior Referral before they make a screen shot or print the referral, the referral is sent to administration and the staff member will have lost the opportunity to make a copy for their files Office of Student Services, DPH

7 Screen shot of Behavior Referral A staff member can take a screen shot by using the Print Screen key (PrtScn) found on the upper part of the keyboard or by using the Snipping Tool. The staff member would then save the image as a file on their desktop. Printing Behavior Referral A staff member that has access to a printer, may print a copy of their referral before they submit it. The staff member needs to select Print Preview. The staff member would select Only the selected frame, which is found second from the right in the top menu bar. To print the Behavior Referral, in the Print Preview, select As laid out on screen and change it to Only the selected frame. Then print the Behavior Referral Office of Student Services, DPH

8 To submit a referral, click the Submit button on the referral. Once a referral has been submitted, it can no longer be modified by the staff member submitting the referral. After the Behavior Referral has been submitted, the message Behavior referral has been submitted below the Incident Referral Editor will appear. A Process Alerts message will be sent to the administrators selected in the Notify field. Once the Behavioral Referral has been processed, an notification will be sent to the staff member that composed the referral. The Process Alerts is found on the Infinite Campus landing page, below District and School Announcements. Deleting a Behavior Referral To delete a referral, select the Behavior Referral you wish to delete, than select the Delete from the Behavior menu found above the Incident B bar. The incident will be removed, along with all event, participant and resolution information within this tool and elsewhere. Once a Behavior Referral has been submitted it cannot be deleted by the staff member. Office of Student Services, DPH
