3 1. Fill out the sections and upload the required document a. Contact details b. Enrollment form c. Confirmation payment d. Passport size picture and authentic signature of the student e. Scholarship acceptance form (only if you have been awarded with a scholarship) f. Language prerequisite g. Bachelor diploma h. Official GMAT score report (only if not already provided during the selection process) 3

4 1a. Contact details This section shows the details (e mail, mobile phone number, permanent home address) you entered during the selection process, check if they are correct and save the section. 4

5 1b. Enrollment form The Enrollment form has to be signed in each spot: 3 signatures of the student. The form needs to be signed on the blank space above the «signature of the student» indication. The Enrollment form has 2 pages, you can choose whether to make a unique scan or to upload the single pages in the specific areas (Enrollment form PAGE 1 and Enrollment form PAGE 2) Please authorize the treatment of your personal data and insert the date. The ID card or passport : please upload a valid copy of your passport/id Result undersigned: pleaseprinttheresultpaper,signand upload it 5

6 1c. Payment of the first installment In the section «Upload» you need to upload the payment confirmation receipt, NOT the MAV slip automatically generated by the system. If you decide to pay by bank transfer, you can find all the bank details by clicking on the section Confirmation payment (if the payment is by bank transfer, please upload the scan or.pdf of the confirmation received by your bank) 6

Passport size picture and authentic signature of the student After printing and signing the document «Official signature» (please note: Max size allowed 5 Mb; Allowed extensions:.jpg or.jpeg; Image must be at least 400px width and 150px height), in the Upload section click on «Authentic signature of the student», then click on «Click or drag an Image» and select the rectangle with your signature. Finally, for completing the upload, click on «upload». For uploading your «Passport size photo» (please note: Max size allowed 5 Mb; Allowed extensions:.jpg. ; Image must be at least: 235px width and 295px height), in the Upload section click on «Passport size photo», then click on «Click or drag an Image» and select the rectangle with your face. In conclusion, for completing the upload, click on «upload». 7

8 1e. Scholarship acceptance form This section will appear ONLY if you have been awarded with a scholarship. Please print, sign, date and upload the document in order to accept the scholarship offer. 8

9 1f. Language prerequisite Check on our website the list of valid language prerequisites and minimum scores required; Flag the language prerequisites you already have or you will obtain within June 30 th 2017; Remember to upload your language prerequisite by and no later than June 30 th

10 1g. Bachelor Diploma and Declaration of value A valid final Bachelor Diploma is compulsory to enroll. Please check the deadlines: 30th June 2017: deadline for the submission of a valid Bachelor diploma for CHINA MIM admitted students 31st October 2017: deadline for the submission of a valid Bachelor diploma Please verify the conditions on the validity of your Bachelor diploma on the Bocconi website. If you are required to provide a Declaration of value (DoV), you will be informed in your selection results. In order to obtain this document, please get in contact with the Italian Embassy/Consulate which has competence upon the diploma asap. The DoV must clearly state that your Bachelor diploma is valid to pursue studies in a second cycle degree program (i.e. Master of Science programs). 10

11 1g. Official GMAT score report The Official GMAT score report is requested ONLY if not already provided during the selection process. 11

12 2. Confirm data and submit application Your enrollment is submitted only once you click on the following button: 1. Confirm data 2. Submit application In order to know if your application has been submitted properly, please verify this messages (on the right of the page): You have confirmed data on gg/mm/aa ; You have submitted application on gg/mm/aa Please remind that, after the enrollment confirmation, you will be able to print an Acceptance letter as receipt of your enrollment. In case you decide to withdraw, you will be required to send a withdrawal form (available online) and a copy of your ID card or passport. 12

Check your personal enrollment webpage frequently in order to get any message from the Office Please enter frequently your «My Application» e check the communications from the Admissions Office. Please note that you will find a message only in case of the documents are NOT valid. Otherwise NO specific message will appear. If you have any doubt about the validity of diploma or English prerequisites please get in contact as soon as possible with the Admissions Office writing to admissionsmsc@unibocconi.it 13
