1 How to Guide: Joining the SystmOne Frailty Organisation Group & Downloading the STOPP Protocol Part A) Joining the Frailty Organisation Group Step 1: In SystmOne home screen, go to Set Up, Users & Policy, then Organisation Groups : Step 2: Then, search for or scroll down to Devon PCT :
2 Step 3: Open drop down to left of Devon PCT to find South Devon & Torbay CCG, then drop down to left of here to find Frailty. Click Frailty and then select Join Group. Note 1: A pop up may appear stating you do not have the necessary permissions to join the group. This requires you asking the practice s SystmOne administrator or whoever has highest access rights to join the organisation group on behalf of the organisation/practice. Note 2: The shared content for the Frailty group will not appear straightaway (see red circle). Log off & onto SystmOne to gain access to the shared content which includes the STOPP protocol.
3 Note 3: The STOPP protocol individual searches (n=67) will automatically appear as autoconsultations (see screen below). There is no need to access the STOPP protocol via autoconsultations but it is worth knowing the number of auto-consultations available will increase.
4 Part B) Downloading the STOPP protocol to your Home Screen Toolbar The outcome of the next step depends on whether the organisation i.e.gp Practice has a default everyone setting in order to allow one person to amend what appears on the home screen for the entire team centrally; or if the organisation has different settings for different groups of clinicians and/or admin staff. Step 1: This step is necessary if you wish to add the STOPP protocol to the toolbar for everyone in the organisation this requires default everyone setting. Go to Setup, Users & Policy and then Organisation Preferences. Then search for Toolbars and select Amend Toolbar and choose Default - Everyone (on right hand side of screen). Note 4: Not everyone has permissions to make this change to make the protocol available to everyone in your Organisation you may need to ask whoever has permissions to make changes to the default settings for SystmOne at the practice. Step 2: To down load the STOPP protocol and add it the toolbar as a short-cut. Go to Setup, then Users & Policy and Organisational Preferences, search Toolbars and select Configure Toolbar
5 Step 3: In Configure Buttons screen, search for protocol, then select/launch Specific protocol from the drop down list by DOUBLE CLICKING Specific protocol. Step 4: Select STOPP Protocol from the list provided:
6 Step 5: The patient screen should now have a protocol icon called STOPP on the tool bar (it should appear on the tool bar for everyone on the organisation): Step 6: Click this icon to trigger the STOPP protocol for a patient. Triggering the STOPP protocol gives the following pop up (the criteria listed in the pop up are those which could not be searched for within the SystmOne STOPP protocol but are criteria within the O Mahoney et al (2014) STOPP/START criteria):
7 Step 7: A completed STOPP protocol lists a Polypharmacy medication review Read Code into the patient journal. A Read Code is entered for each potentially inappropriate medication found with the search and these are listed individually as shown below: Other Useful Information about the STOPP protocol in SystmOne: There is also a STOPP view in Quick Glance so it can be easily referred back to, if required. There are 10 STOPP criteria not included within the SystmOne STOPP protocol. There are a number of reasons for this, for example: a search could not be designed for criteria which is concerned with a specific dose of a medication. The missing criteria are listed in a pop up information screen within SystmOne when the STOPP protocol is triggered. The START criteria from original STOPP/START tool have not been incorporated into the SystmOne procotol. Feedback from GPs who have viewed the tool and the START criteria suggests a START protocol would be unnecessary - as many of the START criteria are already captured by in primary care via QOF. If Users see value in having a START protocol, please do let Sarah De Biase know. If you plan to test the STOPP protocol as a part of a mediation optimisation intervention for people with frailty at a GP Practice, Federation or CCG level, please do let Sarah De Biase know. Sarah can offer quality improvement support to enable new intervention(s) to be tested in such a way that any changes/impact seen can be attributed to the intervention. The learning from these tests will be shared with Healthy Ageing Collaborative partners and primary care more widely.
