6 TESTING APPLICATION FORM OPERATION FLOW Testing Application Form Operation Flow: Suppliers fill in online test request form in InterLink Lab verify submitted form and create job Once the report is in progress, cancellation or editing is not allowed for Applicants(Suppliers). Suppliers submit online test request form Suppliers may edit submit /cancel submit the Test Request Form before the Lab start the testing (i.e. in-progress status) Suppliers print out form, attach with sample and submit to lab To modify the printed submission form, user can mark on it Lab conduct test Lab update form Status in Interlink 6

7 ONLINE TEST REQUEST FORM (EFORM) System Status: Suppliers fill in and save online test request form Suppliers submit test request form Lab updates status when samples are received Draft Submitted In Progress Job completed in Labs Completed 7

8 SUBMIT A TESTING APPLICATION FORM Submit a Testing Application Form: 1. Go to the Testing Application Form section of the Main page. Click on the form you select. The respective testing application form will pop up. 2. Fill out the form. Refer to the section Testing Application Form Form Functions for details of different field options and functions. 3. Click Print & Submit when done. Note: Once the report was in progress by the lab, the form is no longer editable. 4. You will be prompt to the Terms and Conditions. Check mark Understand and Accept and click Continue. 5. A pdf version for the form will be generated. You may save the file as your copy. Print the form and send it with samples to the respective laboratory

10 TESTING APPLICATION FORM TEMPLATE Create and save a New Testing Application Form Template: 1. Open new online form. 2. Enter value to the desired field(s) and click Save As Template button. 3. In the pop-up box, enter your preferred template name. 4. Click Submit to save. 5. The new template will display in the User Saved Template section of the main page. 2 Manage Search Template: To edit or delete a template, click

11 REVIEW A TESTING APPLICATION FORM Review a Testing Application Form : 1. Users (Supplier/Vendor) can review the Testing Application Form record by using Search function. 2. Users can find report records with at column TRF. Click and you can review the Testing Application Form Note: Supplier/Vendor search can only review the form they apply where as buyer search can overview all Supplier/Vendor forms. 2 11

14 TESTING APPLICATION FORM (CHECKLIST) Invoice & Agent Information Section If you click Yes, you do not need to fill in this section. If you click No, you need to fill in whole section again. 14

15 TESTING APPLICATION FORM (CHECKLIST) Sample Description Section 1. Please ensure to put, & space to separate the information in the same field 2. Season fields: Allow multiple selection. 3. Input the unit of fabric / garment weight, like oz/yd², g/m² Type in the information by clicking the box Style # format: 5 digital number, example: One letter + 4 digital number, example A1234 Suggested input style # format: B8521 (Missy), C2767 (Petite), C2768 (Tall) G2515, D355481, G2526, D D+ 6 digital number, example: D

18 TESTING APPLICATION FORM (CHECKLIST) Test(s) Required Section 1. Package test: Select appropriate the test package. 2. For garment / accessories submission, all related trims / fabric test reports / test report number (limited to Intertek test report only) shall be given. 3. Re-test: Ensure to provide the previous test report number. 4. Individual test: Always use the drop down menu to select the physical / Restricted substance tests

19 TESTING APPLICATION FORM 1. Double check the information is correct or not. 2. Missing mandatory information, pop up box by highlighted related missing fields in yellow to remind you to add back. 3. Print & submit the form, click the box Understand and accept before continue (Ensure you read and accept) 4. The form is generated, please print out and submit with the test samples + related test reports (if needed)

20 EDIT THE TESTING APPLICATION FORM Edit a Testing Application Form: Only form status in draft or submitted mode can be edited online. In case you need to update the record after submission, you can mark it on the print out or inform our local CS team by Retrieve application form in the search. 2. Key in online form number (A#########) and search it. 3. Click the E icon and application form display. 4. Click Edit or Edit Submitted button to edit the content. 20

21 ONLINE TEST REQUEST FORM (EFORM) Edit and Update a Saved Online Test Request Form with Draft status: 1. Retrieve Online Test Request Form Search 2. Key in online form number (A#########) and search it. 3. Click the E icon and online form display 4. Click Edit button 5. Update the content in the form. 6. Click Save to save your update. The form will remain in Draft status. When ready to submit the form, make sure to select Print & Submit and accept terms and conditions

Key in online form number (A#########) and search it. 3. Click the E icon and online form display. 4. Click Edit Submitted button. 5. Edit the content. 6. Click Print & Submit

23 ONLINE TEST REQUEST FORM (EFORM) Edit and Update an Online Test Request Form with Submitted status (continued) 7. Once clicked the Print and Submit button, you have to read and accept the Terms and Conditions before continue. 8. The form will be saved with submitted status again, and the revised PDF file will be generated as well. 9. Then the revised TRF #AXXXXXXXXX-REV01 can be found in the search

24 ONLINE TEST REQUEST FORM (EFORM) Cancel a Submitted Online Test Request Form 1. Retrieve Online Test Request Form Search 2. Key in online form number (A#########) and search it. 3. Click the E icon and online form display. 4. Click Cancel Submitted button, then click OK to confirm the cancellation. 5. The record has been removed from the search

27 EXPORT SEARCH RESULT Export search result to Excel: Open respective search in the main page. 1. Enter search criteria. 2. Click Search. All records within the search scope will display if no criterion was entered. 3. Further filter the column results or rearrange the columns as you wish. 4. Click the Export to Excel button. 5. You will have options to save or open the export

28 PERSONAL SEARCH TEMPLATE Create a New Search Template: Open respective search in the main page Enter search criteria and click Save Query. 2. In the pop-up box, enter your preferred template name. 3. Click Submit to save. 4. 29 MY FAVORITE WEBSITE Users can keep their favourite website or URL links for easy access to other web pages when using InterLink. Other users are not able to view your favorite website links. Add a document: 1. In the My Favorite Website section, click. 2. Click New. 3. Complete the website information. 4. Click Update when done. Edit the website link or change the display order: 1. In the My Favorite Website section, click. 2. Click Edit for the selected website. 3. Update the website description, URL or change the display order. 4. Click Update when done. Delete a website link: 1. In the My Favorite section, click. 2. Mark the document to be deleted. 3. Click Delete. URL format must be start with or 29
